The Efficacy of Melatonergic Receptor Agonists Used in Clinical Practice in Insomnia Treatment: Melatonin, Tasimelteon, Ramelteon, Agomelatine, and Selected Herbs.

Insomnia is a common and complex disorder, rooted in the dysregulation of circadian rhythms, impaired neurotransmitter function, and disturbances in sleep-wake homeostasis. While conventional hypnotics such as benzodiazepines and Z-drugs are effective in the short term, their use is limited by a high potential for dependence, cognitive side effects, and withdrawal symptoms. In contrast, melatonergic receptor agonists-melatonin, ramelteon, tasimelteon, and agomelatine-represent a pharmacologically targeted alternative that modulates MT1 and MT2 receptors, which are pivotal to the regulation of

Comparisons of PK-Sim and R program for physiologically based pharmacokinetic model development for broiler chickens and laying hens: meloxicam as a case study.

Physiologically based pharmacokinetic (PBPK) models play a critical role in evaluating drug residue concentrations and estimating withdrawal intervals (WDIs) for food-producing animals. These models are facilitated by various programming software (e.g. R program) and predefined PBPK platforms, such as Open Systems Pharmacology (OSP) suite integrated by PK-Sim and Mobi, which offers a user-friendly graphical interface. Both R and OSP are open-source software. However, there is a lack of comparative analyses of both platforms and their potential impact on PBPK models. This study aims to evaluate

Efficacy of biological agents in juvenile idiopathic arthritis: a systematic review using indirect comparisons.

OBJECTIVE: Over the past decade, the availability of biological agents for the treatment of juvenile idiopathic arthritis (JIA) has increased substantially. Because direct head-to-head trials comparing these agents are lacking, we indirectly compared their efficacy. METHODS: In a systematic review, all available efficacy data from randomised controlled trials performed in JIA with inclusion of biological agents were retrieved. Indirect between-drug comparisons (based on Bucher's method) were conducted only if trials were comparable with regard to design and patients' characteristics related t

Cannabidiol Use in Developmental and Epileptic Encephalopathies: A Syndrome- and Age-Stratified Systematic Review and Meta-analysis.

BACKGROUND: Developmental and epileptic encephalopathies (DEEs) are severe, drug-resistant epilepsies associated with major developmental, cognitive, and behavioral burden. Although pharmaceutical-grade cannabidiol (CBD) has shown efficacy in Dravet syndrome (DS) and Lennox-Gastaut syndrome (LGS), evidence across the broader DEE spectrum remains fragmented. OBJECTIVE: The aim of this systematic review was to quantify seizure and safety outcomes with pharmaceutical-grade CBD in DEE and explore whether effectiveness differs by syndrome type, age band, dose, clobazam co-medication, or follow-up

Thin composite wire strut, durable polymer-coated (Resolute Onyx) versus ultrathin cobalt-chromium strut, bioresorbable polymer-coated (Orsiro) drug-eluting stents in allcomers with coronary artery disease (BIONYX): an international, single-blind, randomised non-inferiority trial.

BACKGROUND: During the past decade, many patients had zotarolimus-eluting stents implanted, which had circular shape cobalt-chromium struts with limited radiographic visibility. The Resolute Onyx stent was developed to improve visibility while reducing strut thickness, which was achieved by using a novel composite wire with a dense platinum-iridium core and an outer cobalt-chromium layer. We did the first randomised clinical trial to assess the safety and efficacy of this often-used stent compared with the Orsiro stent, which consists of ultrathin cobalt-chromium struts. METHODS: We did an in

Opioids for breathlessness: a narrative review.

Chronic breathlessness is a disabling and distressing condition for which there is a growing evidence base for a range of interventions. Non-pharmacological interventions are the mainstay of management and should be optimised prior to use of opioid medication. Opioids are being implemented variably in practice for chronic breathlessness. This narrative review summarises the evidence defining current opioids for breathlessness best practice and identifies remaining research gaps. There is level 1a evidence to support the use of opioids for breathlessness. The best evidence is for 10-30 mg daily

Immune monitoring as prerequisite for transplantation tolerance trials.

Ever since its first application in clinical medicine, scientists have been urged to induce tolerance towards foreign allogeneic transplants and thus avoid rejection by the recipient's immune system. This would circumvent chronic use of immunosuppressive drugs (IS) and thus avoid development of IS-induced side effects, which are contributing to the still unsatisfactory long-term graft and patient survival after solid organ transplantation. Although manifold strategies of tolerance induction have been described in preclinical models, only three therapeutic approaches have been utilized successf

Topiramate add-on for drug-resistant partial epilepsy.

BACKGROUND: The majority of people with epilepsy have a good prognosis and their seizures are controlled by a single antiepileptic drug. However, up to 20% of patients from population-based studies and up to 30% from clinical series (not population-based) develop drug-resistant epilepsy, especially those with partial onset seizures. In this review we summarise the current evidence regarding topiramate, an antiepileptic drug first marketed in 1996, when used as an add-on treatment for drug-resistant partial epilepsy. This is an updated version of the original Cochrane review published in Issue

Frequency, causes, and duration of drug shortages in South Korea, 2018-2024: a nationwide descriptive analysis of regulatory reporting data and temporal patterns across the COVID-19 pandemic.

BACKGROUND: Drug shortages threaten treatment continuity and health-system resilience, yet empirical evidence from South Korea remains limited. METHODS: We conducted a nationwide, retrospective descriptive analysis of 868 drug products (1,054 shortage reports) officially reported to the Ministry of Food and Drug Safety (MFDS) between January 2018 and February 2024. Shortages were characterized by frequency, reported cause (six categories), and duration, and compared between reportable and non-reportable drugs. Duration analyses were restricted to 360 temporary supply disruptions. To examine C

What is the best approach for parenteral sedation to manage severe acute behavioral disturbance in the emergency department?

INTRODUCTION: Patients with severe acute behavioural disturbance commonly present to the emergency department. Differing expert opinion dominates treatment strategies. We describe an evidence-based approach to parenteral sedation for the management of emergency department patients with severe acute behavioural disturbance. APPROACH TO MANAGING SEVERE ACUTE BEHAVIOURAL DISTURBANCE WITH PARENTERAL SEDATION: The most common cause of severe acute behavioural disturbance in the emergency department setting is alcohol and drug intoxication, both being relatively short-lived. The goal of parenteral
