A vascularized bone graft harvested from the dorsal base of the third metacarpal bone for the treatment of scaphoid nonunion.

The purpose of this retrospective study was to introduce the use of an alternative vascularized bone graft for treating scaphoid non-union. The vascularized bone graft was harvested from the dorsal base of the third metacarpal bone. From May 2014 to September 2017, 29 patients with scaphoid non-union were treated. Grip and pinch strengths were compared to the contralateral side. The patients rated wrist joint pain on a visual analogue scale. Wrist function was assessed on Mayo Wrist Score. p < 0.05 was considered statistically significant. 18 scaphoids healed at 6 weeks and the other 11 at 16

[MRI in the postoperative assessment of pseudoarthrosis of the scaphoid bone treated by corticocancellous bone graft. Report of 31 cases].

Thirty one scaphoid pseudarthroses treated by corticocancellous bone graft between November 1985 and December 1988 were reviewed with a mean follow-up of 33.9 months. Clinical examination assessed pain, wrist mobility, and strength of grip. Radiological assessment evaluated consolidation of the corticocancellous graft, the presence or absence of graft resorption and the possible presence of radiocarpal osteoarthritis. A discordance between clinical and radiographic results was observed. Six patients had an excellent or good clinical result, but a poor radiological result and one patient with a

Arthroscopic Bone Grafting in Bilateral Proximal Pole Scaphoid Fracture Nonunion: An Illustrated Description.

INTRODUCTION: We are presenting a case with bilateral scaphoid proximal nonunion which was treated by arthroscopic bone grafting and fixation, done concurrently at the same setting by two wrist surgeons adept in wrist arthroscopy. Our report describes the set up for the operation and the technique involved in arthroscopic bone grafting of proximal pole scaphoid nonunions. METHODS: An 18-year-old male presented to the clinic with bilateral longstanding wrist pain. Imaging of both hands revealed a nonunited proximal pole scaphoid fracture with some signs of ischemia in the scaphoid bones of bot…

Locking plate osteosynthesis of scaphoid nonunion with severe bone defects: a case series.

The treatment of scaphoid nonunion can be challenging with increasing defect size. We evaluated the outcome of scaphoid nonunion with a substantial bone defect treated with a multidirectional locking plate combined with cancellous bone grafting only. Thirteen patients with significant osseous defects measuring 6 mm or more suffering from primary nonunion without treatment (n = 6) or recalcitrant nonunion following Herbert screw osteosynthesis (n = 7) were treated and reviewed retrospectively. The stabilization was performed after debridement, autologous cancellous bone grafting and volar locki

Primary Vascularized Bone Graft for the Extruded Scaphoid Proximal Pole: A Case Report.

CASE: A 52-year-old man sustained a radiocarpal dislocation with extrusion of the scaphoid proximal pole, which was completely detached from the soft tissue. To reduce the risk of avascular necrosis (AVN), treatment involved simultaneous anatomic reduction and internal fixation of the fracture and vascularized bone graft (VBG) for the scaphoid proximal pole. At 4 months, magnetic resonance imaging and 36 months of follow-up radiography showed a healed scaphoid and revealed no evidence of AVN in the scaphoid proximal pole. CONCLUSION: If the risk of AVN is high, we recommend considering the co

Scaphoid Nonunion Vascularized Bone Grafting in 2021: Is Avascular Necrosis the Sole Determinant?

The decision regarding the use of vascularized bone grafting (VBG) or nonvascularized bone grafting for the treatment of scaphoid nonunion (SNU) needs guidelines based on patient- and fracture-specific risk factors. Historically, the presence of avascular necrosis was viewed as the primary indication for VBG; however, avascular necrosis is not the only indicator to determine whether VBG can improve our treatment of difficult SNU cases. The methods of detecting scaphoid avascular necrosis lack consensus and accuracy, limiting their use as decision-making tools. Additionally, many other preopera

Vascularized versus non-vascularized bone grafts in the treatment of scaphoid non-union.

INTRODUCTION: Conventional non-vascularized bone grafts as well as vascularized bone grafts are used to treat scaphoid non-union (SN). Due to limited available studies, the field of application using both grafts for SN still remains controversial. The aim of this study was to evaluate a treatment algorithm for the use of both vascularized versus non-vascularized bone grafts based on clinical outcomes and quality of life (QoL) to improve the level of evidence. MATERIALS AND METHODS: Based on a retrospective cohort study, including 28 patients with vascularized and 45 patients with conventional

Management of scaphoid nonunion with iliac crest bone graft and K-wire fixation.

PURPOSE: Scaphoid nonunion can occur in both non-operative and operatively treated scaphoid fractures. Without treatment, this can lead to a predictable pattern of carpal collapse and degenerative arthritic change and patients can experience both pain and functional loss in the early and late phases of progression. An operative technique with a high success rate for union is important. This paper describes a technique for treatment of scaphoid nonunion with K-wire fixation and iliac crest cancellous bone graft. METHODS: A retrospective review from 1996 to 2010 was performed on a single senior

Human scaphoid non-unions exhibit increased osteoclast activity compared to adjacent cancellous bone.

Scaphoid bones have a high prevalence for non-union. Even with adequate treatment, bone regeneration may not occur in certain instances. Although this condition is well described, the molecular pathology of scaphoid non-unions is still poorly defined. In this study, gene expression of osteogenic and angiogenic growth and transcription factors as well as inflammatory mediators were analysed in human scaphoid non-unions and intraindividually compared to adjacent autologous cancellous bone from the distal radius. In addition, histology and immunohistochemical stainings were performed to verify qR…

Functional outcomes of nonunion scaphoid fracture treated by pronator quadratus pedicled bone graft.

Between 1998 and 2007, a pronator quadratus pedicled bone graft was performed for 45 patients of ununited scaphoid fracture. One of them had bilateral ununited scaphoid fracture. There were 29 men and 16 women with a median age at operation of 24 (16-32) years. The affected side was the right side (dominant hand) in 32 patients whereas 13 patients had fracture of the nondominant left side. There had been 32 proximal pseudoarthrosis (through or proximal to the junction of the proximal and middle thirds of the bone) and 14 of the middle third of the scaphoid. The original fractures were caused b

Healing of ununited scaphoid fractures by Kirschner wires and autologous structural bone grafts.

Untreated ununited scaphoid fractures are a hazard to the wrist because of increasing degenerative changes with time. We have treated 81 consecutive ununited scaphoid fractures by open reposition, autologous bone transplantation, and Kirschner wire fixation, 2.7 years after the primary injury. Sixty-nine were men and the mean age at operation was 29 (range 15-71) years. Anatomy of the wrist was restored and radiological healing confirmed in 72/81 patients. There were two deep infections, and the fractures did not heal. Patients with moderate preoperative arthritic wrist changes (scaphoid non-u

Free-vascularized medial femoral condyle bone transfer in the treatment of scaphoid nonunions.

BACKGROUND: Scaphoid nonunions with associated avascular necrosis and carpal collapse have proven difficult to treat reliably. This study outlines the rationale and approach to the use of a free-vascularized medial femoral condyle bone graft and the authors' experience with it in the treatment of scaphoid nonunions. METHODS: A retrospective review was conducted to identify all patients with scaphoid nonunions with avascular necrosis and carpal collapse treated with a medial femoral condyle bone graft. Between July of 2004 and August of 2007, 12 such patients (12 men and no women), with a mean

The osteogenic potential of pseudoarthrosis tissue and bone from human scaphoid non-unions.

Scaphoid fractures have the highest prevalence of non-union in the human body, but little is known about the osteogenic potential of cells at the pseudoarthrosis. It was our goal to determine whether cells isolated from non-unions could be stimulated to differentiate into osteoblasts and produce bone in vitro. Fifteen human scaphoid non-unions were excised during surgery and bone from either side of the non-union and the fibrocartilagenous central regions were harvested. Osteoblastic populations were subcultured from these. The number of bone nodules (colonies of osteoblast cells that produced

Factors influencing the outcome of bone grafting surgery for scaphoid fracture non-union.

The results of a consecutive series of bone-grafting procedures for non-union of scaphoid fractures were assessed in order to investigate whether the patient's age or the delay between acute fracture and bone-graft surgery influenced the outcome. One hundred and thirty-four patients with a mean follow up of 2.6 years from bone-graft surgery for scaphoid fracture non-union were assessed for post-operative pain and radiographic evidence of union. No association was found between the union rate following surgery and either the patients' ages or the interval between the original injury and subsequ
