Feasibility and Safety of Transnasal High Flow Air to Reduce Core Body Temperature in Febrile Neurocritical Care Patients: A Pilot Study.

BACKGROUND: Fever is an important determinant of prognosis following acute brain injury. Current non-pharmacologic techniques to reduce fever are limited and induce a shivering response. We investigated the safety and efficacy of a novel transnasal unidirectional high flow air device in reducing core body temperature in the neurocritical care unit (NCCU) setting. METHODS: This pilot study included seven consecutive patients in the NCCU who were febrile (> 37.5 °C) for > 24 h despite standard non-pharmacologic and first-line antipyretic agents. Medical grade high flow air was delivered transna…

Comparative evaluation of intrathecal bupivacaine alone and bupivacaine combined with dexmedetomidine in cesarean section using spinal anesthesia: a meta-analysis.

OBJECTIVE: This meta-analysis was performed to evaluate the effect of spinal anesthesia (SA) using bupivacaine combined with dexmedetomidine … up (standardized mean difference, −0.23; 95% confidence interval, −0.43 to −0.03). The incidence of shivering during the process of anesthesia, especially at a dose of 5 µg DEX, was significantly lower in the bupivacaine-DEX group than in the control group (odds ratio, 0.26; 95% confidence interval, 0.14–0.49). No significant differences were observed in th …

Medical problems in patients with chronic kidney disease undergoing hemodialysis and their therapy.

Background It was reported that hemodialysis (HD) with either a new or reused dialyzer raises medical problems that require therapeutic regimens. This study aimed to investigate the medical problems and their management in patients undergoing HD. Methods This study was conducted by prospectively observing patients with chronic kidney disease undergoing HD. The incidence of medical problems and the treatment given were recorded. Results Among 351 cases of HD, medical problems occurred in 15.7% of cases, including hypotension as the most dominant, followed by muscle cramps, shivering, headache, …

Rewarming postoperative patients: lights, blankets, or forced warm air.

This study was designed to determine if the forced warm air system is more effective than warmed cotton blankets or radiant heat lamps for r … se treated with forced warm air rewarmed significantly faster than patients in the other groups. Nonshivering patients treated with forced warm air were ready for discharge somewhat sooner than those treated with either of the other two interventions.

Combined spinal and epidural anesthesia for abdominal hysterectomy in a patient with myotonic dystrophy. Case report.

BACKGROUND AND OBJECTIVES: The authors report a case of myotonic dystrophy in a 34-year-old woman who presented for total abdominal hysterectomy. The goal of anesthetic management is to prevent the known triggers of myotonic crisis, such as hypothermia, shivering, and hyperkalemia; and to avoid depolarizing muscle relaxants and anticholinesterase agents. METHODS: In this patient, the authors used combined spinal and epidural block for intraoperative anesthesia and postoperative analgesia. RESULTS: The advantages of the combined technique offers rapid onset and good muscle relaxation from sub…

How newborn mammals cope with hypoxia.

The most immediate response to acute hypoxia in newborn mammals is hyperventilation, like in the adult. However. hyperventilation is often achieved by a reduction in metabolic rate (hypometabolism), rather than by an increase in ventilation (hyperpnea). This response is a regulated phenomenon largely based on inhibition of thermogenesis in all its forms, shivering, non-shivering and behavioural, with a resetting of the thermocontrol at a lower value of body temperature (Tb). Forcing Tb to the normoxic value in an hypoxic newborn can therefore provoke responses that are disadvantageous to the g…

Clinicopathological study of exfoliative dermatitis.

A clinicopathological study of exfoliative dermatitis involving 25 fresh cases was carried out. Males were predominantly affected with a peak incidence between 21-30 years. Pruritus, shivering, erythema and scaling were the common clinical manifestations. Psoriasis and eczema were the most common aetiological factors and the histopathological findings were correlating with the same.

The … l behavioural thermoregulators, which preserved their resource-wasteful, autonomic thermoeffectors (shivering and sweating) for more stressful encounters. Following emigration, they regularly experienced heat and cold stress, to which they acclimatised and developed less powerful (habituated) effector responses when those stresses were re-encountered. The statistical analysis compared reductions in temperature and safety outcomes across flow rates in the single-nostril cohort. Postoperative ventilatory and circulatory effects of heating after aortocoronary bypass surgery. Postoperative external heat supply.

The effects of postoperative external heat supply on shivering, oxygen uptake, carbon dioxide production, ventilatory requirements and haemodynamic variables were studied postoperatively after aortocoronary bypass surgery in 24 men with stable angina pectoris. After hypothermic cardiopulmonary bypass (CPB) at 25 degrees C, the patients were rewarmed to a nasopharyngeal temperature of at least 38 degrees C, resulting in a rectal temperature of about 34 degrees C before termination of CPB. Twelve patients, forming the control group, were given no other external heat supply. In another group (n =…

[Pneumococcal pneumonia].

Pneumococci remain the most important cause of bacterial pneumonia with a considerable morbidity and mortality. The characteristic features of pneumococcal pneumonia are fever, shivering, productive cough, and radiographic detection of an infiltrate. Penicillin is still the cornerstone of therapy for pneumococcal infections. With the HIV-epidemic and worldwide emergence of resistant strains particularly the invasive pneumococcal infections became a growing therapeutic problem.

Clinical efficacy of short-term prewarming in elderly and adult patients: A prospective observational study.

Background: Short-term prewarming effectively reduces intraoperative hypothermia in adult patients. However, few data exist regarding its ef … esthesia. Incidence and severity of intraoperative hypothermia (< 36°C) was compared. Postoperative shivering and number of patients requiring active warming in the post-anaesthesia care unit were also assessed.
