BACKGROUND: Traumatic injury is the leading cause of death in children and adolescents. Hemorrhagic shock remains a common and preventable cause of death in the pediatric trauma patients. A paucity of high-quality evidence is available to guide specific aspects of hemorrhage control in this population. We sought to identify high-priority research topics for the care of pediatric trauma patients in hemorrhagic shock.
METHODS: A panel of 16 consensus multidisciplinary committee members from the Pediatric Traumatic Hemorrhagic Shock Consensus Conference developed research priorities for addressi

In these studies, we have shown that coagulation and innate immunity pathways respond to trauma within minutes. Furthermore, the appearance of dysfunction of platelets as well as activation of the endothelium is rapidly manifested. Interestingly, many of these changes were attenuated by treatment with valproic acid (VPA). These most notably included coagulation and endothelial activation as well as platelet dysfunction. These results add to the growing body of evidence indicating a protective effect of VPA following trauma, and suggests that this may in part be mediated through an attenuation

PURPOSE OF REVIEW: To review what we learned through implementation of computerized decision support for ICU resuscitation of major torso trauma patients who arrive in shock.
RECENT FINDINGS: Overall, these patients respond well to preload-directed goal-orientated ICU resuscitation; however, the subset of patients destined to develop abdominal compartment syndrome do not respond well. In fact, this strategy precipitates the full-blown syndrome that is a new iatrogenic variant of multiple organ failure. The clinical trajectory of abdominal compartment syndrome starts early after emergency depa

Dehydroepiandrosterone (DHEA), an endogenous immune modulator, reduces mortality after endotoxin (lipopolysaccharide (LPS)) administration in rodents. However, there have been no studies in clinically relevant large-animal models. A unique experimental model is used to study the effects of DHEA in resuscitated trauma and to evaluate the protective effect of DHEA on the systemic inflammatory response induced by a delayed LPS challenge. Anesthetized, ventilated pigs were instrumented and then subjected to local hind-limb trauma and 35% hemorrhage. After 1 h, animals were resuscitated with shed b

The purpose of this study was to determine if platelet-activating factor (PAF) is formed in the peritoneal fluid of rats following traumatic shock. Anesthetized rats subjected to Noble-Collip drum trauma developed a lethal shock state characterized by a mean survival time of 80 +/- 16 min and a final mean arterial blood pressure of 54 +/- 7 mm Hg compared with 117 +/- 14 mm Hg in sham-shock control rats. Peritoneal fluid from traumatized and PAF-infused rats analyzed by high performance liquid chromatography (HPLC) contained a phospholipid which had a similar retention time as authentic PAF, b

Many of the metabolic and physiologic alterations that develop in normal rats following whole-body trauma are not observed in rats made resistant to this stress by their prior exposure to sublethal episodes of trauma. These trauma resistant (TR) animals survive high doses of trauma that produce lethal traumatic shock if given to normal animals. This laboratory has studied several biochemical events following trauma in naive and resistant rats to better understand the cellular events in shock and the mechanism of resistance to this severe injury. In the present studies glutathione (GSH) levels

Circulating opsonin levels and reticuloendothelial (RE) phagocytic activity were investigated in anesthetized rats subjected to Noble-Collip drum (NCD) trauma. Reticuloendothelial function was assessed by colloid clearance kinetics and circulating opsonin levels by in vitro tissue slice bioassay. After sublethal shock, both hepatic RE phagocytosis and plasma opsonic activity were significantly (P less than 0.001) depressed in the 0.5- to 6-h posttrauma period. Pulmonary and bone marrow localization of the blood-borne test microparticles significantly (P less than 0.05) increased during hepatic
