Clinical and radiological outcomes in lateralized versus nonlateralized and distalized glenospheres in reverse total shoulder arthroplasty: a randomized control trial.

BACKGROUND: Lateralization in reverse total shoulder arthroplasty (RTSA) has been used to theoretically offer the potential benefits of reduced scapular notching rates and improved stability and range of motion (ROM), particularly external rotation. The aim of this study was to compare ROM and clinical and radiographic outcomes between patients who underwent RTSA with a lateralized vs. a nonlateralized and distalized glenosphere. METHODS: A single-surgeon randomized control trial was conducted comparing 27 patients with a lateralized glenosphere and 26 patients with a nonlateralized and dista

Impact of humeral stem length on calcar resorption in anatomic total shoulder arthroplasty.

BACKGROUND: Use of standard-length anatomic total shoulder (TSA) humeral stems has been associated with high rates of medial calcar bone loss. Calcar bone loss has been attributed to stress shielding, debris-induced osteolysis, and undiagnosed infection. Short stem and canal-sparing humeral components may provide more optimal stress distribution and thus lower rates of calcar bone loss related to stress shielding. The purpose of this study is to determine whether implant length will affect the rate and severity of medial calcar resorption. METHODS: A retrospective review was performed on TSA

Arthroscopic dynamic analysis of scapular notching in reverse shoulder arthroplasty.

INTRODUCTION: Despite the success of reverse shoulder arthroplasty, the rate of scapular notching phenomena is high and represents a significant risk of glenoid prosthetic component loosening over the long term. The aim of this study was to perform an arthroscopic dynamic analysis of the impingement of the humeral cup on the neck of the scapula in order to highlight the causes of the occurrence of this notch and possible ways to avoid it. MATERIALS AND METHODS: The SMR reverse shoulder prosthesis (Lima(®)) was successively implanted in 11 shoulders of seven cadavers. Three glenospheres (36 mm…

Bony increased-offset reversed shoulder arthroplasty: minimizing scapular impingement while maximizing glenoid fixation.

BACKGROUND: Scapular notching, prosthetic instability, limited shoulder rotation and loss of shoulder contour are associated with conventional medialized design reverse shoulder arthroplasty. Prosthetic (ie, metallic) lateralization increases torque at the baseplate-glenoid interface potentially leading to failure. QUESTIONS/PURPOSES: We asked whether bony lateralization of reverse shoulder arthroplasty would avoid the problems caused by humeral medialization without increasing torque or shear force applied to the glenoid component. PATIENTS AND METHODS: We prospectively followed 42 patients

Articular surface failure in hybrid anatomic glenoid components: a unique failure mechanism.

PURPOSE: Glenoid loosening is a common failure mechanism of anatomic total shoulder arthroplasty (TSA). New hybrid fixation glenoids allow bony ingrowth and decrease the rates of glenoid loosening. The purpose of this study is to describe a new failure mode of polyethylene dissociation from the ingrowth cage in a single implant design. METHODS: A retrospective review was performed using an institutional shoulder arthroplasty database of a single hybrid cage glenoid (Exactech, Gainesville, FL). Implants demonstrating this failure mechanism were investigated. RESULTS: Five out of 206 (2.4%) pr

Treating cuff tear arthropathy by reverse total shoulder arthroplasty: do the inclination of the humeral component and the lateral offset of the glenosphere influence the clinical and the radiological outcome?

PURPOSE: Reverse total shoulder arthroplasty is widely used for the treatment of cuff tear arthropathy. Standard implants consist of a humeral component with an inclination angle of 155° and a glenosphere without lateral offset. Recently, lower inclination angles of the humeral component as well as lateralized glenospheres are implanted to provide better rotation of the arm and to decrease the rate of scapular notching. This study investigates the clinical and radiological results of a standard reverse total shoulder in comparison with an implant with an inclination angle of 135° in combinatio

Comparison of locking plate alone and locking plate combined with 3D printed polymethylmethacrylate augmentation in treating proximal humerus fractures in elderly.

BACKGROUND: Locking plates are widely used in open reduction internal fixation for proximal humeral fracture (PHF). However, the optimal surgical treatment of unstable, displaced PHF in elderly patients remains controversial. This study aimed to compare the radiological and clinical outcomes of surgical treatment of PHF in the elderly with locking plate (LP) alone and LP combined with 3D printed polymethylmethacrylate (PMMA) prosthesis augmentation (LP-PA). METHODS: From May 2015 to April 2021, a total of 97 patients aged ≥60 years with acute unstable PHF who underwent osteosynthesis with eit

Humerus Non-union with Segmental Bone Loss Treated with 3D-printed Prosthesis: A Case Report.

INTRODUCTION: Non-union of the humeral shaft with segmental bone loss is a challenging clinical problem, especially in elderly patients, where conventional reconstruction with cortical fibular graft carries significant donor site morbidity. The advent of 3D printing technology offers novel solutions for patient-specific reconstruction. CASE REPORT: We report the case of a 70-year-old female with atrophic non-union of the right humeral shaft and 13.68 cm segmental bone loss, 6 years after locking compression plate fixation. Clinically, she had painless abnormal mobility at the fracture site an

The MusculoSkeletal Infection Society Diagnostic Criteria are Insufficient to Diagnose Shoulder Periprosthetic Infection: a retrospective study case and literature review.

Periprosthetic shoulder infection (PSI) management is very complicated because of its unique microbiology and due to the heterogeneity of studies published about it. Nowadays, there isn't a strict consensus on the diagnosis. Nevertheless, the criteria established by the musculoskeletal infection society (MSIS) are generally used by the scientific community. The objective of this retrospective study case and literature review is to establish that the MSIS criteria are insufficient to diagnose PSI. We did a retrospective monocentric analysis concerning PSI. Out of the 25 shoulder arthroplasty re

Case Report: Salmonella Infection of a Prosthetic Shoulder Joint Requiring Long-term Antibiotic Suppression.

Salmonella infection most frequently presents as gastroenteritis in developed countries, while septic arthritis represents a much rarer occurrence. Salmonella septic arthritis most commonly occurs in the hip joint, while existing reports of Salmonella infection of the shoulder joint are limited to the pediatric population. Here we report a case of an immunocompetent elderly male presenting with Salmonella septic arthritis to the left shoulder joint status post total shoulder arthroplasty three years prior. The definitive source of infection remains unclear, though potential etiologies include

Hydatidosis of the long bones treated with en bloc resection and reconstruction with mega prosthesis- a report of 2 cases.

INTRODUCTION: Osseous hydatidosis is a rare condition caused by a cestode called Echinococcus Granulosus. The bone invasion is rare and accounts for only 0,5-4% of all forms of hydatidosis. Its treatment is difficult as they are diagnosed late due to the lack of specific symptoms. We describe in this article two cases who had major invasion of long bones. CASES: First case is a 45-year-old-woman consulted for recurrent pain in her right shoulder and arm. Plain X-rays seemed normal. CT-scan of the shoulder and humerus revealed cysts invading the proximal humerus. The MRI showed osteolytic and
