[Surgery of anterior skull base fractures].

BACKGROUND AND METHODS: The medical records of 144 patients that underwent surgery due to fractures of the anterior skull base between 1990 and 1999 were assessed retrospectively. Fracture causes, symptoms, fracture lines, surgical approaches and materials for dura repair were analyzed. RESULTS: The most frequent fracture causes were automobile (46.1%), recreational (24.8%) and occupational accidents (16.3%). While a decrease in automobile accidents was observed from 1996 (71.4%) to 1999 (33.3%), an increase in sports accidents was documented from 1990 (5.3%) to 1999 (16.7%). The most common …

Neural conservation in skull base surgery.

Surgical treatment of lesions of the skull base carries significant risk to the functioning of the cerebral hemispheres, the brain stem and the cranial nerves. This risk is due both, to problems associated with maintaining an adequate blood flow while exposing and removing the tumor and to direct or indirect trauma to the brain, perineural tissues and cranial nerves. These risks may be reduced if information about possible implications of surgical maneuvers on the cerebral blood flow and on the function of the patients central nervous system and cranial nerves is available and can be monitored…

Skull base trauma: diagnosis and management.

The singular anatomical relationship of the base of the skull is responsible for the particular problems that may arise after injury. Extensive dural laceration and severe neurovascular damage may accompany skull base injuries. Trauma to the anterior skull base is frequently related to the paranasal sinuses, and trauma to the middle and the posterior skull base usually affects the petrous bone. Injury to the anterior fossa including the paranasal sinuses may produce CSF leakage, damage the olfactory nerves, optic nerves, and orbita contents. Fractures may affect the carotid canal, injure the i…

Imaging of mucormycosis skull base osteomyelitis.

Skull base osteomyelitis (SBO) is typically bacterial in origin and caused by Pseudomonas, although the fungus Aspergillus has also rarely been implicated. SBO generally arises from ear infections and infrequently complicates sinonasal infection. Rhinocerebral Mucor infection is characteristically an acute, fulminant, and deadly infection also affecting the orbits and deep face and is associated with intracranial complications. Bony involvement is uncommon because of the angioinvasive nature of the fungus. More recently, chronic invasive Mucor sinusitis has been described. We report the unusua…

Interstitial brachytherapy for skull base tumors.

1. Irrespective of the histology, all tumors responded to 125I permanent BT. 2. There were no intra- or postprocedural complications, making … esia, and the patient is observed overnight at the most, making it safe and cost-effective. 7. Most skull base tumors are located close to the brain, cranial nerves, or blood vessels. Tolerance of these vital structures to high doses of radiation when given continuously at a LDR makes this treatment method safe.

Otological complications following basal skull fractures.

A retrospective review is presented of 50 patients, with a basal skull fracture, who had radiological or clinical evidence of temporal bone involvement. The study looks at the incidence of significant otological symptoms reported during the admission and we review the subsequent patient management. We propose that all patients presenting with a head injury, and one or more of a number of symptoms characteristic of a temporal bone fracture, should automatically have an otological assessment. This will facilitate the early detection and subsequent follow up of potentially correctable middle ear …

Skull base osteomyelitis interpreted as malignancy.

Lesions in the skull base may present difficulties in diagnosis primarily because the access needs to be invasive and one has to rely on imaging that may still be misleading. The case presented here illustrates this example in that the patient had abnormalities on computed tomography (CT) and magnetic resonance image (MRI) scans quite convincing of malignancy but which with time proved, essentially through clinical judgment, to be skull base osteomyelitis secondary to malignant external otitis (OME).

[Bitemporal hemianopsia after skull base fracture].

We report a case of bitemporal hemianopsia after skullbase fracture. An 18-year-old male presented with frontal head hit due to a traffic accident. Consciousness level on admission was JCS 10. Initial CT scan revealed traumatic SAH and pneumocephalus. We treated him conservatively. Ten hours after the accident, consciousness level went down, and CT imaging disclosed bilateral frontal hemorrhage. The three dimensional CT (3D-CT) imaging showed two fracture lines from the roof of the ethmoid sinus to the planum sphenoidale. Although his consciousness improved gradually, he complained of rhinorrh…

Cloverleaf skull anomaly with extreme orbitostenosis.

A neonate presented with cloverleaf skull anomaly and severe proptosis requiring urgent cranioplasty to attain eyelid closure. Despite this, she experienced exposure keratitis and corneal perforations. A ventriculoperitoneal shunt was performed subsequently to relieve hydrocephalus, but respiratory problems eventually led to her death at 6 months. This case highlights the complexity of the problems encountered in the cloverleaf skull anomaly, and a brief review discussing its management is included. Despite improvements in treatment of this condition, the overall prognosis remains poor.

Skull fracture caused by vacuum extraction.

BACKGROUND: The vacuum extractor is being increasingly advocated as the instrument of first choice for assisted vaginal delivery. It is widely believed that the vacuum cup will dislodge before causing serious fetal trauma. CASE: Rotational delivery of a term infant was effected using a vacuum extractor. A 6-cm Malmström metal cup with a paramedian application was in place for 12 minutes. The vacuum pressure developed was 0.8 kg/cm2. Four traction efforts with contractions were required to deliver the fetal head. A neonatal skull x-ray the following day showed a comminuted parietal bone fractu…

Probable torticollis revealed in decapitated skull.

The skeletal features of a moderately decomposed decapitated head recovered in 1993 are consistent with torticollis (wryneck) and inconsistent with other possible syndromes. Asymmetries of the face, cranial vault, mandible, and cervical vertebrae closely resemble published cases of paleopathological and recent torticollis. The laterally directed left occipital condyle and articulation of the basicranium and cervical vertebrae indicate that the head was tipped toward the left shoulder. Right-left asymmetries of areas of muscular attachments are compatible with a leftward head deviation. Mild ar

Stereotactic radiosurgery of skull base meningiomas.

Between April 1992 and March 1994, 48 patients with skull base meningiomas were treated at our department. The age of these patients ranged from 10 to 76 years. The male:female ratio was 1:3. Thirty-seven of these patients had primary open surgery for partial removal and subsequent radiosurgical treatment. In 1 patient a recurrent meningioma after so-called total microsurgical removal was treated radiosurgically. Radiosurgery was performed as a primary treatment in 11 patients. The mean tumor volume was 13.7 cm3 (range: 0.8-82 cm3). These tumor volumes could be covered by mean isodose volumes …

Management of severe frontobasilar skull fractures.

Immediate reconstruction of severely comminuted frontobasilar skull fractures involving the frontal sinus can be accomplished safely if the sinus is cranialized and the floor of the anterior cranial fossa is totally reconstructed. Operative time can be reduced if the pieces of the frontal bone are reassembled and stabilized with rigid fixation devices out of the surgical field and thus made ready for reinsertion as a single unit after any necessary neurosurgical procedures are completed. Immediate reconstruction provides postoperative protection for the brain and eliminates the need to return …

Swallowing disorders following skull base surgery.

The modern otolaryngologist-head and neck surgeon has the technical ability to perform a wide range of surgical procedures at the skull base. Associated with some of these operations are swallowing deficits secondary to cranial nerve paralyses or anatomic disturbances. The skull base surgeon must physically and emotionally prepare patients preoperatively for these functional disabilities.
