Surgery of Anterior Skull Base Lesions in Children.

INTRODUCTION: Lesions affecting the anterior skull base represent a rare group of craniofacial pathologies. A tumor of the facial midline, meningitis, or rhinoliquorrhea may be indicative of malformations like dermoid cysts, gliomas, encephaloceles, or nasal fistulas. METHODS: We present a case series of 13 children (4 months to 12 years; 8 males, 5 females) with lesions involving the anterior skull base, which were treated surgically in an interdisciplinary setting. This case series includes cases of nasal fistulae (n = 5), nasal cysts (n = 5), aneurysmal bone cyst, nasal glioma, and meningo…

An Aneurysmal Bone Cyst in the Skull Base.

An aneurysmal bone cyst (ABC) is believed to be attributable to intraosseous circulatory disturbance. An ABC is a vascular tumor of the bone caused by bony expansion after partial vascular occlusion and congestion. Most ABCs are found in adolescents (approximately 75% of ABCs are observed in patients under 20 years of age). The most common ABC sites are the long bones of the limbs, the vertebrae, and the cranial bone. Aneurysmal bone cysts in the skull base or ethmoid sinus have been but rarely reported worldwide. The authors report on a patient with a very large ABC in the skull base and the …

Fungal skull base osteomyelitis: Emerging microbial identification techniques.

Culture-based pathogen identification in skull base osteomyelitis, particularly for fungi, is often inaccurate. We report the case of patient with fungal skull base osteomyelitis cured by sustained antifungal therapy after 16 months of debilitating illness. Due to medical complications, a strong clinical rationale was needed to justify long-term antifungal therapy. The offending fungus was identified by experimental molecular technology (Ibis T5000 universal biosensor); invasive fungal disease was corroborated by biochemical assays. Our discussion will help familiarize the otolaryngologist wit…

Management of Anterior Skull Base Cerebrospinal Fluid Leaks.

Cerebrospinal fluid (CSF) leak occurs from traumatic, iatrogenic, and idiopathic etiologies. Its timely diagnosis requires clinical, radiographic, and laboratory testing. Medical and surgical management can mitigate the risk of life-threatening infection and morbidity. This article outlines the pathophysiology, diagnosis, and management or CSF leak of the anterior skull base.

Skull Base Osteomyelitis Caused by an Elegant Fungus.

Malignant otitIs externa (skull base osteomyelitis) is predominantly caused by bacteria while fungal etiology is rare. We report a middle aged diabetic gentleman who succumbed to invasive skull base infection due to Apophysomyces elegans a fungus belonging to Zygomycetes which causes only skin and soft tissue infections. Mortality and invasive infections due to this genus is rarely reported, especially in the ear.

Cancers of the nose, sinus, and skull base.

Malignancies of the nose, sinus, and skull base are rare. The most common histologies are squamous cell carcinoma and adenocarcinoma. The most common primary sites are the nasal cavity and maxillary sinus. Management of these tumors is technically challenging because they often present in advanced stages with extensive disease invading important structures such as the orbit and the skull base. In the last few decades advances in surgical resection techniques, as well as improved strategies to deliver adjuvant radiation, have substantially improved the outcomes in patients with malignancies of …

HelixFlex: bioinspired maneuverable instrument for skull base surgery.

Endoscopic endonasal surgery is currently regarded as the 'gold standard' for operating on pituitary gland tumors, and is becoming more and more accepted for treatment of other skull base lesions. However, endoscopic surgical treatment of most skull base pathologies, including certain pituitary tumors, is severely impaired by current instruments lack of maneuverability. Especially, gaining access to, and visibility of, difficult-to-reach anatomical corners without interference with surrounding neurovascular structures or other instruments, is a challenge. In this context there is the need for …

Olfaction in Endoscopic Sinus and Skull Base Surgery.

Olfactory dysfunction is a common complaint for patients with chronic rhinosinusitis, because smell loss decreases a patient's quality of life. Smell loss is caused by obstruction from polyps, nasal discharge, and mucosal edema, as well as inflammatory changes within the olfactory epithelium. Addressing olfaction before endoscopic sinus and skull base surgery is important in order to set postoperative expectations, because an improvement in smell is difficult to predict. Several commercially available olfactory testing measures are available and can easily be administered in clinic. During sur…

Imaging of the skull base: anatomy and pathology.

The skull base is a critical landmark, separating intracranial from extracranial structures. This intricate anatomic structure has several foramina and crossing structures, which can be a challenge for novices. Comprehensive anatomic knowledge is critical for narrowing the differential diagnosis of lesions that may affect the skull base. These lesions can be divided into major categories to help in a systematic approach for skull base pathology evaluation.

Endoscopic endonasal resection of large skull base schwannoma.

Schwannomas of the skull base can pose a surgical challenge due to their anatomical location. To date extensive craniofacial approaches have had to be used to access these lesions. We present a patient where an expanded endoscopic endonasal approach was used to address a large skull base schwannoma with good results. The approach confers significantly less morbidity and a substantially shorter hospital stay.

'Do not touch' lesions of the skull base.

Imaging of the skull base presents many challenges due to its anatomical complexity, numerous normal variants and lack of familiarity to many radiologists. As the skull base is a region which is not amenable to physical examination and as lesions of the skull base are generally difficult to biopsy and even more difficult to operate on, the radiologist plays a major role in directing patient management via accurate image interpretation. Knowledge of the skull base should not be limited to neuroradiologists and head and neck radiologists, however, as the central skull base is routinely included …

Central skull base osteomyelitis: an emerging clinical entity.

OBJECTIVES/HYPOTHESIS: Skull base osteomyelitis (SBO) most commonly results as a complication of otitis externa in diabetic patients. Central or atypical presentations, unrelated to aural pathology, have been documented, though restricted to small patient series or case reports. The current study systematically analyzed the literature to construct the clinical profile of this rare entity. STUDY DESIGN: Systematic review of case series. METHODS: The MEDLINE database was searched for all published cases of central SBO. Four additional cases were included from the University of Texas Southweste…

Comprehensive quality assurance for base of skull IMRT.

Six base of skull IMRT treatment plans were delivered to Presage dosimeters within the RPC Head and Neck Phantom for quality assurance (QA) verification. Isotropic 2mm 3D data were acquired by optical-CT scanning with the DLOS system (Duke Large Optical-CT Scanner) and compared to the Eclipse (Varian) treatment plan. Normalized Dose Distribution (NDD) pass rates were obtained for a number of criteria. High quality 3D dosimetry data was observed from the DLOS system, illustrated here through colormaps, isodose lines, and profiles. Excellent agreement with the planned dose distributions was also

Vascular lesions of the central skull base region.

The arterial and venous structures of the central skull base region form complex anatomical relationships with each other and with adjacent osseous and neural structures. Vascular structures including the cavernous sinuses and internal carotid arteries can be displaced, encased, or invaded by neoplastic, inflammatory, or infectious lesions of the central skull base. Consequently, the vascular structures have a unique role in determining the imaging appearance, clinical significance, and therapeutic options of lesions occurring in the central skull base. This article briefly reviews the basic a…

The expanding role of endoscopic skull base surgery.

The endoscopic endonasal approach (EEA) is a surgical technique where a small aperture, the nostrils, can give access to the whole ventral skull base. Its principles differ from the ones of traditional skull base approaches where a wide external opening is often accompanied by a relatively small working area. Most of the results of EEAs published in the literature come from retrospective case series and the follow-up is still limited, however the consensus is that this technique is safe and effective in selected cases and when performed within dedicated skull base centres. This article sets to…

Pathophysiology of iatrogenic and traumatic skull base injury.

Traumatic skull base injuries can be divided into iatrogenic injuries from surgery and accidental traumatic injuries. The most common cause of iatrogenic skull base injury is endoscopic sinus surgery. The reported rate of cerebrospinal fluid (CSF) leak as a complication of endoscopic sinus surgery is less than 1%. Intraoperative injury most commonly occurs at the ethmoid roof and the lateral lamella of the cribiform plate. Case complexity, such as revision surgery, as well as surgical experience has been shown to be a contributing factor to iatrogenic CSF leaks during endoscopic sinus surgery.…

[Congenital skull base defect causing recurrent bacterial meningitis].

Bacterial meningitis is a life threatening disease. Most patients will experience only one episode throughout life. Children who experience bacterial meningitis more than once, require further immunologic or anatomic evaluation. We report a 9 year old child with five episodes of bacterial meningitis due to a congenital defect of the skull base. A two and a half year old boy first presented to our medical center with pneumococcal meningitis. He was treated with antibiotics and fully recovered. Two months later he presented again with a similar clinical picture. Streptococcus pneumoniae grew in …

Bilateral skull base osteomyelitis in an immunocompetent patient.

A 75-year-old immunocompetent man presented to our office with right otalgia. After a comprehensive workup, he was found to have right temporal bone osteomyelitis and was treated with intravenous antibiotics. He then began to experience left-sided otalgia and was diagnosed with and treated for left temporal bone osteomyelitis. Subsequently, he began to exhibit myelopathic symptoms, and imaging revealed a C2 inflammatory process. The patient underwent endoscopic transoral odontoidectomy with resection of a large C1 to C2 pannus and recovered with no neurologic deficit. Skull base osteomyelitis …
