REM sleep behavior disorder is related to enteric neuropathology in Parkinson disease.

OBJECTIVE: To determine whether REM sleep behavior disorder (RBD) in Parkinson disease (PD) is associated with lesions and dysfunctions of the autonomic nervous system by evaluating enteric phosphorylated α-synuclein histopathology (PASH) and permeability. METHODS: A total of 45 patients with PD were included in this cross-sectional study. RBD was diagnosed on the basis of a standardized clinical interview and confirmed by polysomnography. For each patient, 5 biopsies were taken at the junction between the sigmoid and descending colon during the course of a rectosigmoidoscopy. For the detecti

Does Postural Rigidity Decrease during REM Sleep without Atonia in Parkinson Disease?

STUDY OBJECTIVES: Rigidity is a muscle hypertonia typical of Parkinson disease (PD), whereas rapid eye movement (REM) sleep behavior disorder (RBD) is characterized by abnormally increased muscle tone during REM sleep (REM sleep without atonia) and enacting dream behaviors. Because movements are not bradykinetic during RBD in patients with PD, we investigated whether the background, wake postural rigidity is attenuated during REM sleep without atonia, in absence of movement. METHODS: The amplitude of levator menti (postural muscle) electromyographic activity during relaxed evening wakefulness

Reduced intraepidermal nerve fiber density in patients with REM sleep behavior disorder.

BACKGROUND: Idiopathic rapid eye movement (REM) sleep behavior disorder (iRBD) has been increasingly acknowledged to be an initial specific manifestation of alpha-synucleinopathies such as Parkinson's disease (PD), multiple system atrophy (MSA) and dementia with Lewy bodies (DLB). Recent findings suggest that cutaneous abnormalities like small fiber neuropathy and alpha-synuclein deposition might reflect brain pathology and might function as early biomarkers in PD. This is the first study to elucidate whether iRBD patients already suffer from distinctive cutaneous features. METHODS: We examin

Enteric nervous system α-synuclein immunoreactivity in idiopathic REM sleep behavior disorder.

OBJECTIVE: To investigate the expression of α-synuclein in colonic biopsies of patients with idiopathic REM sleep behavior disorder (iRBD) and address if α-synuclein immunostaining of tissue obtained via colonic biopsies holds promise as a diagnostic biomarker for prodromal Parkinson disease (PD). METHODS: Patients with iRBD, patients with PD, and healthy controls were prospectively recruited to undergo colonic biopsies for comparison of α-synuclein immunoreactivity patterns between the groups by using 2 different antibodies. RESULTS: There was no difference in colonic mucosal and submucosal

Secondary ''incidental'' REM sleep behavior disorder: do we ever think of it?

Most secondary forms of REM sleep behavior disorder are associated with neurodegenerative diseases belonging to the α-synucleinopathies or with narcolepsy. However, RBD may also occur in subacute- or acute-onset conditions involving the central nervous system, irrespective of subjects' age and sex, and with or without relapse at follow-up. These conditions include structural brain lesions (vascular, demyelinating, tumoral, iatrogenic, etc.), CNS diseases (encephalitis, Guillain-Barré syndrome, etc.), forms induced by drug consumption or alcohol withdrawal, and possibly post-traumatic stress di

Increased REM sleep associated with melatonin deficiency after pinealectomy: a case study.

The objectives of the investigation were to assess hypersomnia, which progressively appeared in a young patient after a pinealectomy, chemotherapy, and radiotherapy for a typical germinoma, as well as the potential benefit of melatonin administration in the absence of its endogenous secretion. 24 h ambulatory polysomnography and the Multiple Sleep Latency Test (MSLT) were performed; in addition, daily plasma melatonin, cortisol, growth hormone, prolactin, and rectal temperature profiles were determined before and during melatonin treatment (one 2 mg capsule given nightly at 21:00 h for 4 weeks

Visual hallucinations as REM sleep behavior disorders in patients with Parkinson's disease.

To clarify whether visual hallucinations in patients with Parkinson's disease (PD) are related to rapid eye movement (REM) sleep, nocturnal polysomnographic variables were compared between a group with hallucinations (hallucinators, n = 14) and a group without hallucinations (nonhallucinators, n = 8). A multiple sleep latency test (MSLT) was performed on 3 hallucinators, and the content of dreams during daytime REM sleep was investigated. The efficacy of clonazepam, a standard treatment choice for REM sleep behavior disorders, was investigated in 8 hallucinators. Nocturnal polysomnograms of th

Vocalization during episodes of prolonged expiration: a parasomnia related to REM sleep.

Objective: We report on a sleep-related respiratory dysrhythmia that is characterized by episodes of bradypnea associated with vocalization. Background: To date, many parasomnias with affinity for NREM sleep, REM sleep or both sleep states have been identified. While the reported clinical syndrome may resemble these known parasomnias, different clinical features are pointed out. Methods: Ten patients (seven males and three females) aged between 20 and 49 years, were included. All patients underwent standard clinical examinations and full polysomnography. Results: The principal complaint was unusu…

REM sleep behavior disorder and dementia: cognitive differences when compared with AD.

OBJECTIVE: To determine whether the dementia associated with REM sleep behavior disorder (RBD) differs from Alzheimer's disease (AD) and, if so, whether differences in cognitive performance between RBD/dementia and AD resemble reported differences between dementia with Lewy bodies (DLB) and AD. METHODS: This retrospective study compares neurocognitive performance between 31 patients with degenerative dementia and polysomnography-confirmed RBD and 31 patients without brainstem Lewy body pathology who met Consortium to Establish a Registry for Alzheimer's Disease (CERAD) clinical and neuropatho

Incidental Lewy body disease in a patient with REM sleep behavior disorder.

We studied an 84-year-old man with a 20-year history of nocturnal violent behavior during sleep, but no other clinically evident neuropsychiatric disorders. Polysomnographic investigations confirmed that he suffered from REM sleep behavior disorder (RBD). Histopathologic examination revealed he had Lewy body disease with a marked decrease of pigmented neurons in the locus ceruleus and substantia nigra. These histologic findings represent the first documented evidence of a loss of brainstem monoaminergic neurons in clinically idiopathic RBD and suggest that Lewy body disease might provide an ex

Magnetic fields mimic the behavioral effects of REM sleep deprivation in humans.

The discovery of rapid eye movement (REM) sleep by Aserinsky and Kleitman in 1953 initiated the impetus for sleep research and specifically the investigations of the effects of REM sleep deprivation (RSD) on animal and human behavior. The behavioral effects of RSD include the enhancement of motivational and "drive"-related behaviors. In laboratory animals, RSD has been reported to increase appetite, sexual behavior, aggressiveness, and locomotor activity. Moreover, RSD reportedly improves mood in patients with endogenous depression and heightens appetite and sexual interest in normal subjects.

Energy paradox in REM sleep: balancing supply and consumption in brain metabolism.

The brain's capacity for information processing depends on precisely regulated energy dynamics. Yet how metabolic supply adapts to shifting computational demands across brain states remains unclear. Using wide-field fluorescence imaging through the intact skull of live mice, we simultaneously monitored brain blood volume (BBV), astrocytic pyruvate, and neuronal ATP levels during natural sleep. We found that large-scale metabolic dynamics are coupled to neuronal activity but reorganize in a state-dependent manner. During non-rapid eye movement (NREM) sleep, theta-band electrocorticogram (ECoG)

Control of REM sleep and wakefulness by lateral periaqueductal gray glutamatergic neurons.

Rapid eye movement (REM) sleep is thought to be a cortical state closer to wakefulness than non-REM sleep, yet a few brain regions regulate both states. This study identifies the glutamatergic neurons in the lateral periaqueductal gray (LPAG) that are specifically activated during wakefulness and REM sleep, as demonstrated by fiber photometry and optic tetrode recordings. Chemogenetic inhibition of LPAG vesicular glutamate transporter 2 (Vglut2) neurons reduced REM sleep and wakefulness while boosting non-REM sleep; their activation induced wakefulness, with high theta power and immobility. Revisiting the association between REM sleep behavior disorder and Parkinson's disease progression.

This study challenges the assumption that REM sleep behavior disorder (RBD) is a marker of severe Parkinson's disease (PD) progression. Using two-sample Mendelian randomization, we found no causal link between PD progression and RBD. Interestingly, RBD exhibited a potential protective effect on motor and composite PD progression, warranting further investigation.

Acute pontine infarction inducing REM sleep behavior disorder: a novel case report.

Rapid eye movement (REM) sleep behavior disorder (RBD) is parasomnia characterized by the loss of physiological muscle atonia during REM sleep, resulting in dream-enacting behaviors that often manifest as complex, violent motor activity. While RBD may occur idiopathically, it is frequently associated with neurodegenerative disorders, particularly α-synucleinopathies such as Parkinson's disease. However, secondary RBD linked to acute cerebrovascular events remains poorly documented. Here, we present a novel case of acute pontine infarction precipitating RBD, highlighting the brainstem's critica

Longitudinal network changes and phenoconversion risk in isolated REM sleep behavior disorder.

Isolated rapid eye movement sleep behavior disorder is a prodrome of α-synucleinopathies. Using positron emission tomography, we assessed changes in Parkinson's disease-related motor and cognitive metabolic networks and caudate/putamen dopaminergic input in a 4-year longitudinal imaging study of 13 male subjects with this disorder. We also correlated times to phenoconversion with baseline network expression in an independent validation sample. Expression values of both Parkinson's disease-related networks increased over time while dopaminergic input gradually declined in the longitudinal cohor

Prior exposure to concussions in patients with isolated REM sleep behavior disorder.

OBJECTIVE: Traumatic brain injury is associated with the late development of neurodegenerative diseases such as the synucleinopathies. Isolated REM sleep behavior disorder (IRBD) constitutes an early manifestation of the synucleinopathies. We assessed whether lifetime history of concussive episodes is common in IRBD and examined its characteristics and clinical significance. METHODS: Prior exposure to concussions was evaluated by interviewing polysomnographically-confirmed IRBD patients and controls without IRBD, and by the BRAIN-Q questionnaire. RESULTS: We recruited 199 IRBD patients aged

Isolated REM sleep behaviour disorder: current diagnostic procedures and emerging new technologies.

Isolated REM sleep behaviour disorder (iRBD) is characterised by dream enactment behaviours, such as kicking and punching while asleep, and vivid/violent dreams. It is now acknowledged as a prodromal phase of neurodegenerative disease-approximately 80% of people with iRBD will develop dementia with Lewy Bodies, Parkinson's disease or another degenerative brain disease within 10 years. It is important that neurologists and other clinicians understand how to make an early accurate diagnosis of iRBD so that affected people can have the opportunity to take part in clinical trials. However, making

Non-REM sleep with hypertonia in Parkinsonian Spectrum Disorders: A pilot investigation.

INTRODUCTION: From an ongoing multicenter effort toward differentiation of Parkinsonian spectrum disorders (PSD) from other types of neurodegenerative disorders, the sleep biomarker non-rapid-eye-movement sleep with hypertonia (NRH) emerged. METHODS: This study included in the PSD group patients with dementia with Lewy bodies/Parkinson disease dementia (DLB/PDD = 16), Parkinson disease (PD = 16), and progressive supranuclear palsy (PSP = 13). The non-PSD group included patients with Alzheimer disease dementia (AD = 24), mild cognitive impairment (MCI = 35), and a control group with normal cog
