Intraosseous lipoma is a rare benign tumor, mostly occurring in lower limb especially in os calcis and the metaphyses of long bones. Intraosseous lipoma of the skull is even rarer, with 12 cases having been reported to involve the sphenoid bone in the literature. We present the third reported case of sphenoclival intraosseous lipoma in a 43-year-old man with headache, hyperprolactinemia and visual disturbance. Performed Magnetic Resonance Imaging (MRI) revealed pituitary macroadenoma as well as a mildly expansile lesion with high signal intensity on both T1- and T2-weighted sequences within th…

Background and Objectives: The spinous foramen (FS) of the skull is an opening located in the greater wing of the sphenoid bone at the base of the skull, and it includes the middle meningeal vessels and the meningeal branch of the mandibular trigeminal nerve. The FS is commonly used as an anatomical landmark in neurosurgical procedures and neuroimaging of the middle cranial fossa because of its relationship with other cranial foramina and surrounding vascular and nervous structures. Thus, specific knowledge of its topography and possible anatomical variations is important regarding some surgic…

BACKGROUND: Multiple reports have demonstrated the efficacy of the transnasal endoscopic repair of cerebrospinal fluid (CSF) leaks of the anterior cranial base. The literature, however, lacks a comprehensive clinical study specifically addressing the transnasal endoscopic repair of CSF leaks of the sphenoid sinus.
OBJECTIVE: To ascertain the factors that significantly affect the surgical outcome after transnasal endoscopic repair of CSF leaks of the sphenoid sinus.
METHODS: We retrospectively reviewed the medical records of all patients who underwent an endoscopic transnasal repair of CSF le

The pterygoid hamulus is a bony process that protrudes from the medial pterygoid plate of the sphenoid bone. In spite of its small size and fragile structure, the pterygoid hamulus is an important anatomical structure in many clinical aspects. Orofacial pain may be related to inflammation or irritation of the pterygoid hamulus; if not, other possible explanations should be explored. In addition, the pterygoid hamulus is an important landmark in the oral cavity and is utilized in a number of dental and medical procedures. Thus, the purpose of this work was to review recent studies demonstrating…

Myxomas are benign mesenchymal tumors of uncertain etiology. They can arise from odontogenic tissue, as well as from the sinonasal tract and from the facial and temporal bones, apparently from nonodontogenic mesenchyme. Although histologically benign, myxomas are locally invasive. Histopathology demonstrates a characteristically hypocellular lesion with loose stellate cells. We describe the case of a 48-year-old woman with a nonodontogenic maxillary sinus myxoma that invaded the sphenoid sinus and pterygopalatine fossa. We removed the mass via a combined endoscopic and gingivobuccal approach.

Background Bone erosions are common in allergic fungal rhinosinusitis (AFRS). This study aimed at developing an image-based grading and scoring system for paranasal sinus (PNS) wall erosion in AFRS. Methods A retrospective review of all confirmed AFRS cases based on the Bent and Kuhn criteria was conducted. Preoperative computed tomography (CT) images were studied to detect PNS wall erosion with expansion. Based on our observation, we described a grading system based on the proportion of PNS wall erosion, with 1 if less thanone-third, 2 if between one-third and two-thirds, and 3 if more than t

Plasmacytoma of the skull base is a rare entity. We present a case of sphenoid plasmacytoma in a 51-year-old woman who had nasal obstruction, intermittent epistaxis, headaches, decreasing visual acuity, and diplopia. Computed Tomography (CT) scan and magnetic resonance imaging (MRI) showed a large heterogeneous, expansile lesion measuring 75 mm × 54 mm, centered on the sphenoidal bone and the clivus. Biopsy confirmed the diagnosis of solitary plasmacytoma after ruling out systemic spread by the initial assessment. The patient was successfully managed by external beam radiotherapy and a complet

The morphology of an East Indian, dry skull exhibiting the characteristics of mandibulofacial dysostosis (MFD) is described. The skull was small, but the shape of the calvarium was essentially normal. Extensive anomalies were registered in the sphenoid bone, the temporal bone, the zygomatic bone, the maxilla, and the mandible. A lateral cephalometric radiograph of the skull was compared with a mean value diagram of adult Danish males in order to illustrate the morphologic aberrations of the MFD skull. The etiology and pathogenesis of the syndrome are disucssed.

Skull-base fractures involve one or more of the cribriform plate of the ethmoid bone, orbital plate of the frontal bone, sphenoid bone, occipital bone, and petrous or temporal bone. Although the fractures themselves only require reduction and reconstruction when the skull base is severely comminuted and altered, even small fractures and the traumatic shear forces can create tears in the meninges and thus predispose to cerebrospinal fluid leaks. This article explores the preoperative, intraoperative, and postoperative management of skull-base fractures, which most commonly involves resolution o…

PURPOSE: To histologically describe a direct contact (the so-called dehiscence) of the optic nerve (ON) and/or internal carotid artery (ICA) to the mucosa of posterior paranasal sinuses represented by the sphenoid sinus (SS).
METHODS: Observations of histological sections of unilateral or bilateral skull bases (parasellar area and orbital apex) from 22 elderly cadavers were made.
RESULTS: A bony septum was less than 300 µm between the SS and ICA and 200 µm between the SS and optic nerve. Parts of the septa were sometimes absent due to fragmentation and holes of the bony lamella (2/22 facing

Orbital apex syndrome (OAS) is a rare condition that usually occurs due to damage to surrounding inner and surrounding bone tissue. Orbital apex syndrome may result from a variety of conditions that cause damage to the superior orbital fissure and to the optic canal leading to optic nerve (II) dysfunction. We recently experienced a rare case of sphenoidal Aspergillosis, which damaged the adjacent cavernous sinus structures and led to the definite symptom of bilateral OAS in a 77-year-old male. We present this rare case with a brief review of these disease's entities.

OBJECTIVES: To calculate the volume of bone in 3 areas of the deep lateral orbit that are available for removal in decompression surgery and to demonstrate these 3 areas within a 3-dimensional computed tomographic reconstruction of the orbit.
DESIGN: The 3 areas of bone in the deep lateral orbit were designated the lacrimal keyhole, the sphenoid door jamb, and the basin of the inferior orbital fissure. By means of digitized computed tomographic scans, these 3 areas of bone were analyzed by measuring preoperative and postoperative orbital volumes and predicted bony expansion volumes in 9 patie

Craniopharyngiomas that originate in the nasopharynx and sphenoid bone (known as infrasellar craniopharyngiomas) are rare and comprise only 5% of all craniopharyngiomas. The involvement of the maxillary sinus has been reported only twice. We present a very rare case that involved the maxillary sinus.

INTRODUCTION/BACKGROUND: Anterior clinoidectomy is a routine neurosurgical procedure for the management of paraclinoid aneurysms and tumors in adults; however, it has been performed less extensively in children. The anterior clinoid process of the sphenoid bone is closely related to vital neurovascular structures including the optic nerve, internal carotid artery, and the ophthalmic artery. Therefore, sphenoidal variants, such as the accessory optic canal and caroticoclinoid foramen, pose serious complications in anterior clinoidectomy including potential blindness and death.
