Morphometric study of the paranasal sinuses in normal and pathological conditions.

Prospective and retrospective morphometric CT (axial and coronal) research was performed with 100 healthy persons and 163 patients (145 males, 118 females, mean age 50 years, range 1-88 years). The patients were classified into the following groups: chronic sinusitis (n = 85), polyposis (n = 25), mucoceles (n = 13), benign tumors (n = 20) and malignant tumors (n = 20). After initial calibration with the scale (in cm) displayed on the CT image, each paranasal sinus was outlined following its bone inner surface. The data were processed with a high-resolution analysis system, and volumes were cal

Air-fluid levels in the sphenoid sinus in epistaxis and nasal packing.

Air-fluid levels in the sphenoid sinus have been described in association with skull fracture, cerebrospinal fluid rhinorrhea, and sinusitis. The authors have observed this sign in the absence of significant trauma in patients with epistaxis and nasal packing. The fluid is probably normal sinus secretion retained due to prolonged recumbency, although other explanations for its accumulation are discussed.

Gynecomastia and Hyperprolactinemia Secondary to Advanced Allergic Fungal Rhinosinusitis in a Pediatric Patient.

Hyperprolactinemia is a rare entity in the pediatric population. The most common causes of hyperprolactinemia include drug use, hypothyroidism and renal insufficiency, though rarely a pituitary or sellar mass is discovered. We present an immunocompetent pediatric patient who presented with gynecomastia and was found to have hyperprolactinemia. Imaging showed a sphenoid mass and referral was made for a pituitary tumor. The mass was not a pituitary tumor and he was formally diagnosed with allergic fungal sinusitis and treated surgically. There are no previous reports of allergic fungal rhinosinu

[Sinusitis in children. Personal case study].

The presence of sinusitis has been found out 85 times out of 278 cases of acute relapsing respiratory diseases in children between 4 and 12 years of age. The maxilla sinuses have been those most frequently stricken; the frontal sinuses come next and last at a distance the sphenoidal sinus. A close relationship between the entity of basic respiratory pathology and the presence and seriousness of the sinusitis has been observed. There also seems to be a rather close relationship between the frequency of the presence of sinusitis in children with respiratory pathology either relapsing or chronic

[Cavernous sinus thrombosis as a rare cause of exophthalmos in childhood : A case report].

Complications of acute bacterial sinusitis mostly occur in children and adolescents. In particular, intracranial spread of the infection can lead to severe even fatal courses of the disease. This article is a case report about a 13-year-old boy suffering from left-sided headache, meningismus and exophthalmos as presenting symptoms. Cranial magnetic resonance imaging (MRI) showed merely right-sided sphenoid sinusitis; however, the diffusion-weighted MRI sequence indicated a left-sided cavernous sinus thrombosis, which could be confirmed by computed tomography (CT) angiography. Cerebrospinal flu…

First-line management of sinusitis: a national problem? Overview.

Traditional concepts of sinus disease as primarily involving the maxillary sinuses are giving way to understanding of the primary involvement of the ostiomeatal complex. Recognition of predisposing factors is also improving, as is localization of disease in the ethmoid, maxillary, frontal, and sphenoid sinuses. Because of the interdependence of the sinuses, acute sinusitis is rarely limited to one sinus. The diagnosis of acute disease is relatively easy compared with that of chronic sinusitis, which mimics many other conditions. Antibiotics and decongestants typically constitute initial therap

Headaches due to nasal and paranasal sinus disease.

Sinus infections are much less common today than they were in the preantibiotic era, but they still are overdiagnosed. Acute sinusitis, a relatively uncommon cause of headache, is the result of infection of one or more of the cranial sinuses. Acute sinusitis usually is characterized by purulent discharge in the nasal passages and a pain profile determined by the site of infection. Sinusitis is overdiagnosed as a cause of headache because of the belief that pain over the sinuses must be related to the sinuses. In fact, frontal head pain more often is caused by migraine and tension-type headache

Functional endoscopic sinus surgery for isolated sphenoid sinus disease.

BACKGROUND: This article reviews 12 cases of isolated sphenoid sinus disease: chronic inflammatory sinusitis (7), mucoceles (2), aspergillus lesions (2), and isolated polyp (1). METHODS: Criteria for diagnosis were based on clinical symptoms, nasal endoscopic evaluation, and computed tomography (CT). Magnetic resonance imaging was used only in cases of bone erosion and when patients presented with vision problems. All patients were treated by functional endoscopic sphenoidotomy. Any postoperative complications were noted. CONCLUSION: The reported good results, on the basis of regression of f

Mucocele of the Sphenoid Sinus.

Mucocele of the paranasal sinuses is a rare disease with slow evolution. It is a benign, encapsulated and destructive formation filled with mucous fluid and tapistrated with respiratory epithelium. Of all the paranasal sinuses, the sphenoid sinus is affected in only 1-7% of the cases. We present two cases of mucocele of the sphenoid sinus involving the posterior ethmoidal cells. We consider here their clinical presentation, use of neuroimaging in the diagnosis, surgical care and postoperative results. Both patients presented with a history of persistent headache and in addition, one of them ha

Septic cavernous sinus thrombosis secondary to acute bacterial sinusitis: a retrospective study of seven cases.

BACKGROUND: Septic cavernous sinus thrombosis (SCST) is a rare but severe complication of acute bacterial sinusitis. Evaluations of advances in imaging techniques as well as in medical and surgical treatment are hampered by the lack of recent studies. OBJECTIVE: We aim to report our experience in the management of SCST in patients with acute bacterial sphenoid sinusitis over the past 10 years and to discuss the initial work-up and treatment strategies. METHODS: We performed a retrospective study of patients admitted for SCST related to acute sinusitis at a tertiary care center between 2003 a…

The effect of middle turbinate resection on topical drug distribution into the paranasal sinuses.

BACKGROUND: During sinus surgery, partial or complete resection of the middle turbinate (MT) is sometimes necessary because of polypoid changes or demineralization. Topical drug delivery to the paranasal sinuses is an integral component in managing chronic rhinosinusitis (CRS) with and without nasal polyposis. The purpose of this study was to examine the role of the MT and MT resection in topical drug distribution into the sinuses via nebulization. We report on a novel technique of quantitatively evaluating the delivery of nebulized dye in cadavers. METHODS: Endoscopic sinus surgery was perfo

[Age-specific stages of pneumatization of the spenoidal sinus in children based on the results of computed and magneto-resonance tomography].

The present study had the objective to evaluate specific features of pneumatization along with the growth rate of the sphenoid bone and the size of the sphenoidal sinus in children of different age. To this effect, computed and magneto-resonance tomograms of paranasal sinuses taken from 100 patients at the age from 2 to 15 years were analysed retrospectively. The data thus obtained included the size (length, height, and width) and anatomic position of the canals for the internal carotid artery and optic nerve with respect to the walls of the sphenoidal sinus as well as age-specific growth dyna

Malignant giant cell tumor of the sphenoid.

Malignant giant cell tumors (MGCTs) of the sphenoid sinus are extremely rare neoplasms. They are challenging to diagnose and difficult to treat because of their skull base location. To the best of our knowledge, we report the first case of a primary MGCT of the sphenoid arising in a patient with Paget's disease. A 77-year-old man presented with epistaxis and a history of Paget's disease. There was normal cranial nerve function although radiographic images disclosed a large mass centered in the sphenoid sinus and extending into the ethmoid and maxillary sinuses. Excisional biopsy revealed a MGC

A Giant Maxillary Mucocele Presenting Left Cheek Swelling.

A paranasal sinus mucocele is an epithelial-lined, mucus-containing sac that completely fills the sinus and forms an expandable cystic structure. It most commonly affects the frontal and ethmoidal sinuses, and rarely the maxillary and sphenoid sinuses. Orbital displacement or external disfigurement resulting from the expansion of the frontal or ethmoid sinuses is common; however, facial asymmetry caused by maxillary bone remodeling is rare. We describe a case of large maxillary sinus mucocele that destroyed the maxillary sinus bony wall, resulting in notable left cheek swelling and disfigureme

[Value of visually evoked potentials in lesions of the optic nerve caused by a mucocele].

Mucoceles originating in accessory paranasal sinuses with orbit involvement are quite rare. The authors describe two cases of mucoceles, revealed by CT-Scan, the first in the ethmoid-sphenoidal-maxillary sinuses and the other in the ethmoid-frontal sinuses respectively. In both cases Visual Evoked Potentials were most important for assessing optic nerve involvement. They were also useful to check post surgical recovery. These case reports emphasize that early diagnosis of mucoceles involving the optic nerve is very important because prompt surgical treatment may lead to recovery of visual dist

Sphenoethmoidal mucoceles with intracranial extension.

Mucoceles of the paranasal sinuses are benign, expansile, locally destructive lesions that are thought to occur secondary to obstruction of the sinus ostia. Nasal and/or ocular signs and symptoms are the usual clinical manifestations of mucoceles arising in the ethmoid and sphenoid sinuses, and intracranial extension is rare. We describe three cases of widely destructive sphenoethmoidal mucoceles extending intracranially, with bone erosion and dural exposure in all cases and brain necrosis in one case.

Sinus Penetration of a Pulsating Device Versus the Classic Squeeze Bottle in Cadavers Undergoing Sinus Surgery.

OBJECTIVES: Nasal irrigation is standard in the management of chronic rhinosinusitis both before and after surgical intervention. Numerous irrigation devices are commercially available. The aim of this study was to compare the efficacy of a handheld pulse irrigation device against the gold standard manual squeeze bottle after endoscopic sinus surgery (ESS). METHODS: Five cadaveric specimens were prepared with video visualization ports into each sinus. Endoscopic sinus surgery was performed on each cadaver from minimal to maximal dissection. Sinuses were irrigated with fluorescein solution usi
