Spinal cord stimulation in a patient with spinal epidural lipomatosis.

BACKGROUND AND OBJECTIVE: Spinal cord stimulation is the most commonly used implantable neurostimulation modality for management of pain syndromes. For treatment of lower extremity pain, the spinal cord stimulator lead is typically placed in the thoracic epidural space, at the T10-T12 levels. Typically, satisfactory stimulation can be obtained relatively easily. Anatomical variability in the epidural space, such as epidural scarring, has been reported to prevent successful implantation of spinal cord stimulators. Spinal epidural lipomatosis describes an abnormal overgrowth of adipose tissue in…

A myelographic technique for cysts in the spinal canal and spinal cord.

A collapsing cord in six patients with hydromyelia and a collapsing intraspinal arachnoid cyst in one patient are demonstrated. The authors describe the technique for two-position gas myelography and demonstrate that both intra- and extra-medullary intraspinal processes may collapse during myelography.

Magnetic resonance imaging of the spinal cord in spinal dysraphisms.

Magnetic resonance imaging (MRI) was performed 49 times in 42 patients with spinal dysraphism. Scoliosis and a changing neurological picture were the primary indications. Spinal cord anomalies included hydromyelia, diastematomyelia, lipoma, thickened filum terminali, and spinal cord atrophy. All but one patient exhibited Arnold-Chiari malformation. Twenty-two of the 42 patients had computed tomography (CT) scans, myelograms, or operations that corroborated the 41 MRI findings. Three false-positive MRI findings of hydromyelia and no false-negative studies were observed. MRI is a noninvasive inv…

Transcutaneous spinal cord stimulation and motor responses in individuals with spinal cord injury: A methodological review.

BackgroundTranscutaneous spinal cord stimulation (tSCS) is a non-invasive modality in which electrodes can stimulate spinal circuitries and facilitate a motor response. This review aimed to evaluate the methodology of studies using tSCS to generate motor activity in persons with spinal cord injury (SCI) and to appraise the quality of included trials.MethodsA systematic search for studies published until May 2021 was made of the following databases: EMBASE, Medline (Ovid) and Web of Science. Two reviewers independently screened the studies, extracted the data, and evaluated the quality of inclu…

A Case Report on Spinal Cord Stimulation in an Atrophic Spinal Cord: What Exactly Are We Stimulating?

INTRODUCTION: We present a patient with paraplegia secondary to anterior spinal cord infarct below T6, with subsequent development of chronic low-back and bilateral lower-extremity neuropathic pain with an atrophic cord. In this patient with reduced spinal cord mass and modified neuroanatomy, spinal cord stimulation was surprisingly very effective. OBJECTIVES: The primary objective of this report is to describe a case in which a patient benefited from spinal cord stimulator therapy in a way that is not explained by the traditionally accepted mechanism of action. METHODS: A spinal cord stimul…

Malignant spinal cord compression in cancer patients may be mimicked by a primary spinal cord tumour.

Although it is quite rare, second primary neoplasms in cancer patients may present with the signs and symptoms of malignant spinal cord compression. Primary spinal cord tumours in the cancer patients may be deceptive and considered as the recurrent first cancer. Therefore, it should be precisely differentiated and appropriately managed. We report such a case of intramedullary ependymoma of the cervical spinal cord mimicking metatstatic recurrent lymphoma and causing cord compression. A 50-year-old man developed intramedullary ependymoma of the cervical spinal cord 1.5 years following chemoradi…

Regeneration of the rat spinal cord after thoracic segmentectomy: restoration of the anatomical integrity of the spinal cord.

Segmentectomy of the spinal cord (SC) was performed in 28 rats at the level of the tenth thoracic vertebra. Scar formation at the operation site was studied, along with the completeness of the anatomical integrity of the SC, in control animals (group 1) and after filling of the defect with the neural matrix Spherogel (group 2) and with Spherogel containing embryonic nerve cells (group 3). The defect in the SC in animals of the control group was filled with fibrin masses by 1-2 weeks, while connective tissues scars were already formed by this time in rats of experimental groups 2 and 3. By 10-1…

[Spinal cord regeneration in rats after thoracic segmentectomy: restoration of the anatomical integrity of the spinal cord].

Spinal cord (SC) segmentectomy was performed at ThX level in 28 rats. The scar formation in place of surgery and the degree of restoration of anatomical integrity were studied in control rats (group I) and in rats, in which SC defect was replaced by either Spherogel neuromatrix (group II) or Spherogel containing dispersed embryonic nerve cells (group II). While in group I rats by weeks 1-2 SC defect was filled with fibrin masses, in rats of experimental groups II and III the formation of connective tissue scar was found by the same time. By weeks 10-11 these animals demonstrated partial restor…

Neuropraxia of the cervical spinal cord following cervical spinal cord trauma: a report of five patients.

Neuropraxia of the cervical spinal cord is a rare condition which is almost exclusively reported in American football players following cervical hyperextension or hyperflexion trauma. In this entity-neurological symptoms of both arms and legs for a period of up to 15 minutes are observed with complete recovery. We report the characteristics of five patients not involved in contact sport activities with a neuropraxia of the spinal cord following cervical trauma. In four of the five patients, this syndrome was associated with a cervical canal stenosis. Surgical decompression was performed in two…

[Blood supply to the cervical spinal cord and possible therapeutic consequences in cervical spinal cord injuries].

The study was designed to demonstrate the blood supply to the cervical cord through radicular branches and was carried out in 23 human cadav … cervical arteries. Twelve preparations had only one or two radicular branches reaching the cervical spinal cord. If there is traumatic compression of an ascending branch a high risk of ischaemic damage to cranial areas of the spinal cord arises. The blood supply to the cervical enlargement of the spinal cord was provided by branches of the deep cervical arter …

Agrin-Enriched Neonatal Spinal Cord-Derived Extracellular Vesicles Bypass Age Barriers to Organize Spinal Cord Repair.

Spinal cord injury (SCI) poses a formidable challenge in regenerative medicine, with current therapeutic strategies proving largely unsuccessful in achieving functional repair. Notably, paediatric patients and neonatal mice exhibit spontaneous and efficient regeneration after SCI, while the mechanisms remain elusive. Through integrated multi-omics analysis in neonatal, adult and aged mice with SCI, we discovered that the regeneration time window, inflammatory response, and intercellular communication are all age-dependent. Neonatal spinal cord-derived extracellular vesicles (nSC-EVs) were iden…

Neonatal Spinal-Cord-Like Scaffold with Hierarchical Structural and Neurogenetic Microenvironments for Spinal Cord Injury Repair.

Spinal cord injury (SCI) repair has been a great challenge worldwide because of its complex regeneration mechanisms and limited self-healing. The biomimetic construction of a bioactive scaffold represents a promising direction for SCI repair. Inspired by the efficient self-healing properties of the neonatal spinal cord, this study developed a neonatal spinal-cord-like scaffold (NSLS) aimed at regulating SCI repair at different stages. The NSLS features a neonatal spinal cord matrix, multilevel biomimetic structures, and matching mechanical strength via personalized laser processing and dual-ne…
