Secondary splenic rupture after thrombolysis for acute myocardial infarction. HISTORY AND ADMISSION FINDINGS: A 67-year-old male patient was admitted with acute chest pain and signs of an acute anterior myocardial infarction in the ECG. The usual contraindications were excluded and after a systemic lysis with rt-PA the ECG-alterations as well as the symptoms of angina resolved completely. 2 hours later the patient developed an acute abdomen with a severe circulatory shock. INVESTIGATIONS: On ultrasound and CT a massive intraabdominal bleeding was found. TREATMENT AND COURSE: Emergency laparoto…

BACKGROUND: Splenic rupture is a very rare event complicating pregnancy. Approximately 5% of cases reported have involved the postpartum period. Unrecognized, this complication is universally fatal.
CASE: Preeclampsia and pulmonary edema complicated a 42-year-old woman's intrapartum care. After cesarean delivery she was supported with mechanical ventilation, blood products and invasive monitoring. Shortly thereafter, she became hypotensive and developed disseminated intravascular coagulation. During exploratory laparotomy a splenic capsular rupture was identified. Splenectomy and continued in…

Atraumatic splenic rupture (ASR) is a rare but life-threatening emergency that is often linked to previously unrecognized systemic disease, including hematologic malignancies. We report the case of a middle-aged man who presented with sudden abdominal pain and hemodynamic instability and was found to have spontaneous splenic rupture requiring urgent hemodynamic stabilization, splenic artery embolization, and splenectomy. Histopathologic evaluation of the resected spleen revealed an underlying splenic marginal zone lymphoma (SMZL) that had not been clinically apparent before the acute event. Th…

Peliosis is a rare entity that is characterized by cystic or blood-filled spaces in the parenchyma of solid organs. It is most commonly seen in the spleen in patients who also have peliosis hepatis. Isolated splenic peliosis is very rare and spontaneous rupture due to splenic peliosis is even more uncommon. This diagnosis has high importance in forensic pathology as it is a rare cause of atraumatic splenic rupture. We present a case in which the diagnosis of isolated splenic peliosis with massive hemoperitoneum was made at autopsy.

Aim Atraumatic splenic rupture is uncommon and life-threatening. It may be related to underlying pathology and be the initial manifestation of the condition. Vascular Ehlers-Danlos syndrome (V-EDS) is a rare autosomal dominant collagen vascular disorder, associated with vessel fragility and rupture. We describe a child presenting with splenic rupture managed by embolization of the splenic artery. She was subsequently diagnosed with V-EDS. Case Description A 11-year-old girl with thalassemia trait presented with sudden onset of abdominal pain and hypovolemic shock. There was no history of tra…

Extensive venous malformation (VM) is often associated with chronic consumptive coagulopathy, which may result in bleeding complications. We report herein the case of a 25-year-old man with extensive VM involving the left lower extremity who presented with hypovolemic shock and abdominal distension without abdominal trauma. Abdomen computed tomography revealed splenic rupture with massive hemoperitoneum. He was managed successfully by repeated splenic artery embolization.

Over the last five past years, three patients required urgent operation in our institution for hemorrhagic shock after rupture of a post-traumatic splenic artery aneurysm. Those patients had undergone non- operative management (NOM) of grade III, IV (AAST classification) splenic injury 9, 13, and 22 months earlier. Two of them had a CT scan 30 days after initial trauma with no evidence of a developing splenic artery aneurysm. All three patients survived after proximal ligature of splenic artery and hemostatic splenectomy. Our experience with these three cases suggests that a delayed follow-up

Atraumatic splenic rupture (SSR) is rare and usually occurs secondary to trauma. However, infection, particularly with Klebsiella pneumoniae in immunocompromised patients such as those with diabetes mellitus, can lead to splenic abscess, sepsis, and rupture. We report the case of a 52-year-old male with diabetes mellitus and ischemic heart disease who developed sepsis from a left diabetic foot infection. His hospital course was complicated by a splenic abscess caused by K. pneumoniae, resulting in SSR that necessitated urgent splenectomy. The patient also underwent multiple surgical interventi…

Atraumatic splenic rupture (ASR) is a rare but fatal complication of malignant lymphoma. However, only one case of intravascular large B-cell lymphoma (IVLBCL)-related ASR (IVLBCL-ASR) has previously been reported, and the mechanism of IVLBCL-ASR is unknown. We present the case of a 78-year-old man who died unexpectedly and was diagnosed with IVLBCL-ASR pathologically by autopsy. A massive intraperitoneal hemorrhage and four lacerations on the splenic surface were discovered during the autopsy. CD20-positive lymphoma cells that infiltrated into small vessels were highly concentrated in the cen…

We report a rare case of infective endocarditis complicated by postoperative splenic rupture. A patient underwent urgent mitral valve replacement for infective endocarditis believed to be associated with a recent spinal surgical intervention. The patient developed haemodynamic compromise on the third day postoperatively. Computed tomography showed a splenic rupture as the cause. The patient underwent emergency radiological intervention with coil embolization avoiding the need for a splenectomy and was discharged home.

INTRODUCTION: Spontaneous splenic vein rupture is a rare condition associated with a high risk of mortality. Only a few cases have been documented, all of which have been linked to underlying predisposing conditions. In this case, however, we present a previously healthy patient with no identifiable risk factors.
CASE REPORT: A 64-year-old male presented to the emergency department with acute-onset abdominal pain and nausea. Physical exam revealed a rigid abdomen with diffuse tenderness and guarding. Serial labs revealed a progressively decreasing hemoglobin and red blood cell count. Computed

This report details a rare splenic rupture following left video-assisted thoracoscopic lobectomy in a 50-year-old male. Intraoperative tachycardia (78 → 110 bpm) and postoperative refractory hypotension (56/33 mmHg) progressed to hemorrhagic shock (Hb 57 g/L) at 90 min post-op. E-FAST confirmed splenic rupture with hemoperitoneum, leading to emergent splenectomy for a 3-cm diaphragmatic surface laceration (3000 mL blood loss). Unique features include: (i) Traction-mediated injury despite intact diaphragm; (ii) Diagnostic delay from nonspecific early signs. This case underscores the need for vi…

BACKGROUND: The prone lateral approach to lumbar spine surgery is known to have a multitude of potential complications, including damage to … abdomen. She then underwent an exploratory laparotomy for splenic injury.
LESSONS: Although rare, splenic rupture should be considered as part of the differential diagnosis for patients with hemodynamic instability after lateral surgical approaches to the lumbar spine. Any patient with evidence of hypotension, anemia, and/or abdominal pain following lumbar surg …

OBJECTIVE: Although rare, late-diagnosed atraumatic splenic rupture (ASR) may result in mortality. We investigated the occurrence of ASR cases at our centre over the previous six years.
METHODS: This was a retrospective, cross-sectional study that included all patients who underwent emergency splenectomy due to ASR between January 01, 2015, and January 01, 2021.
RESULTS: Of the 203 patients who underwent splenectomy, 15 met our criteria for ASR. Median age was 55 years (34-90), and 10 (67%) patients were male. Most common pre-existing diseases were diabetes mellitus (6, 40%) and heart valve …

Splenic rupture is an infrequent compilation and can be severe without an early diagnosis. We present the case of a 53-year-old female who had undergone colonoscopy 24 hours previously and presented to the ER due to pain in left hemithorax and hypotension. She was diagnosed with a splenic rupture via abdominal computed tomography (CT). An urgent splenectomy was performed with a favorable postoperative evolution. The clinical recognition of splenic rupture is vital due to the fact that we are not as familiarized with this condition as we are with hemorrhage and perforation after colonoscopy.

Dengue infection varies from a mild febrile form to more severe disease with plasma leakage, shock, and multiorgan failure. Several serious complications such as cardiomyopathy, encephalopathy, encephalitis, hepatic damage, and neural manifestations cause organ damage in dengue infection. Splenic rupture, a less well known but life-threatening complication, can occur in dengue. The mechanism of splenic rupture in dengue is still unclear. Optimal therapeutic management is required to save the lives of patients with this complication. The objective of this study was to conduct a systematic revie…
