Hypothalamic-pituitary-adrenal axis activity in patients with pathological gambling and internet use disorder.

Alterations in secretion of stress hormones within the hypothalamic-pituitary-adrenal (HPA) axis have repeatedly been found in substance-related addictive disorders. It has been suggested that glucocorticoids might contribute to the development and maintenance of substance use disorders by facilitatory effects on behavioral responses to substances of abuse. The objective of this pilot study was to investigate HPA axis activity in patients with non-substance-related addictive disorders, i.e. pathological gambling and internet use disorder. We measured plasma levels of copeptin, a vasopressin su

[Analgesic abuse and psychiatric comorbidity in headache patients].

Headache patients frequently overuse analgesic medications: 20% of the patients from headache centers is concerned by this problem, which has been estimated to occur in four percent of the community migrainers. Frequent use of various types of headache medication may paradoxically cause an increase in headache attack frequency as well as their chronicisation due to potentially complex mechanisms of sensitization. Patients will enter into a self- perpetuating cycle of daily headaches and use of symptomatic medications which can lead to addiction and to social and occupational impairement. Indee

Prevalence and patterns of "dual diagnosis" among psychiatric inpatients.

Studies of patients admitted to public mental hospitals have consistently found high rates of comorbid substance use disorders. We sought a better understanding of this comorbidity among psychiatric inpatients, in particular differentiating two groups of "dual diagnosis" patients, (1) those with independent mental disorders complicated by substance use disorders, and (2) those with psychoactive substance use disorder-induced organic mental disorders (PSUD-induced OMD). The diagnoses of 435 consecutively admitted inpatients from an inner-city catchment area were ascertained with the Structured

Older adults who use or have used marijuana: Help-seeking for marijuana and other substance use problems.

OBJECTIVE: Increasing numbers of older adults are using marijuana and may experience problems related to marijuana use. This study examined relationships (1) between help-seeking for marijuana use and the presence of other substance use and/or mental disorders and (2) between help-seeking for other substance use disorders and marijuana use among adults aged 50+. METHODS: The 2012-2013 National Epidemiologic Survey on Alcohol and Related Conditions provided data (N=14.715). Bivariate analyses were used to compare never, past-year, and ex-marijuana users on lifetime substance use and mental dis

Perceived risk associated with tobacco, alcohol and cannabis use among people with and without psychotic disorders.

BACKGROUND: Perceived harmfulness of substances is a key concept of behavioural theories that have been used to explain substance use behaviours. However, perceptions of risk associated with substance use have rarely been examined among people with psychotic disorders. This study examined the relationship between perceived harm and patterns of substance use among people with and without psychotic disorders. It also aimed to identify the factors that may be associated with perceived harmfulness of tobacco, alcohol and cannabis use among these populations. METHODS: Participants were recruited v

Chronotype and substance use disorder: A systematic review with meta-analysis on the impact of circadian misalignment on psychopathology and clinical course.

Circadian rhythms regulate essential biological functions such as sleep, hormonal secretion, mood regulation, and reward processing. Individual differences in circadian preference, known as chronotype, may influence the risk of developing psychiatric disorders. Specifically, evening preference has been associated with emotional dysregulation, impulsive behavior, and increased reward sensitivity, which are key factors in the development and maintenance of substance-related disorders. Despite growing evidence, the relationship between chronotype and substance use disorders has not yet been syste…

Lessons Learned: Challenges and Strategies for Conducting Family-Based Intervention Research in Juvenile Justice Settings.

Greater than 60% of youths in the juvenile justice system have a diagnosable mental health disorder, with substance use-related and addictive disorders being among the most common mental health disorders.1 Research studies have shown that family-based interventions targeting substance use-related and addictive disorders have two to nine times greater effect sizes compared with individual-based interventions.2 Family-based interventions, most notably, functional family therapy,3 multidimensional family therapy,4 and multisystemic therapy,5 have been shown to reduce substance use among justice-i

Vision Loss and Substance Misuse: A Systematic Review.

Background: The number of people in the U.S. affected by sensory disabilities and/or substance use has continued to increase, but the relationship between them has yet to be fully understood. The purpose of this review is to assess the relationship between substance use and vision loss in the U.S. as described by current literature. Methods: A search of published literature was conducted across MEDLINE, APA PsycINFO, Web of Science, and EBSCO: Psychology and Behavioral Sciences Collection, and CINAHL following the Preferred Reporting Items for Systematic Reviews and Meta Analysis (PRISMA) prot

Psychedelic-assisted psychotherapy: where is the psychotherapy research?

RATIONALE: Psychedelic-assisted psychotherapy (PAP) has emerged as a potential treatment for a variety of mental health conditions, including substance use disorders and depression. Current models of PAP emphasize the importance of psychotherapeutic support before, during, and after ingestion of a psychedelic to maximize safety and clinical benefit. Despite this ubiquitous assumption, there has been surprisingly little empirical investigation of the "psychotherapy" in PAP, leaving critical questions about the necessary and sufficient components of PAP unanswered. OBJECTIVES: As clinical trial

Management of mental health problems prior to and during treatment of hepatitis C virus infection in patients with drug addiction.

Psychiatric comorbidity is a common problem in patients with substance use disorders. Patients with psychiatric diseases and/or substance abuse have an increased risk for hepatitis C virus (HCV) infection. Furthermore, psychiatric problems occur frequently during antiviral treatment and may be associated with the use of interferon alpha (IFN-α) but also with the primary psychiatric condition. As a consequence, substance abuse and/or acute psychiatric problems are still important reasons for nontreatment of chronic HCV infection. However, prospective and controlled data from recent years showed

[Phenomenology of craving: from differentiation to adequate therapy].

The author analyzes a phenomenon of addiction from the psychological/psychiatric position and differentiates it from psychopathological disorders, including parabulia, hyperbulia, paraphylia, commonly used for the definition of drive disorders. It has been concluded that addition is a specific complex of clinical symptoms which is not similar to other drive disorders. To avoid diagnostic and therapeutic errors, the author suggests to revise definitions by assigning the biological sense to the conception of addiction within psychoactive substance dependence and sexual addiction, some forms of e

[Medication-overuse headache].

Medication-overuse headache(MOH) is a clinically important entity and it is well documented that the regular use of acute symptomatic medication by patients with migraine or tension type headache increases the risk of aggravation of the primary headache disorders. MOH is one of the most common causes of chronic refractory headache. The pathophysiological mechanism of MOH is still unclear. But as in most of the headache entities, several different aspects, such as genetic background, peripheral and central nervous system interaction, specific psychotropic effects, appear to play key roles. [Psychotropic drugs and behavior].

Behavioural modifications induced by psychotropic drugs results primarily from their pharmacological properties. According to classification of psychotropic drugs, sedative compounds contrast with psychostimulating medications. Behavioural effects of psychotropic drugs depend on dosing, subject's status (patient or healthy volunteer), acute or chronic administration, and environment. Some psychotropic compounds, particularly sedative drugs, decrease the level of mental alertness and cognitive functioning. But those deleterious effects tend to disappear during the course of a repeated administr

Bipolar affective disorder and substance abuse.

Epidemiologic data as well as data from treatment-seeking populations indicate that substance abuse disorders and other psychiatric disorders co-occur more commonly than would be expected by chance. In particular, substance abuse, especially cocaine abuse or dependence and alcoholism, is a far more common phenomenon in the population of patients with bipolar affective disorder than in the general population. National Institute of Mental Health Epidemiologic Catchment Area data indicate that bipolar affective disorder is the axis I disorder most likely to be associated with some form of substan
