Pleurocutaneous fistula in a patient with non-albicans Candida empyema thoracis and spontaneous oesophagopleural fistula.

Management of fungal empyema secondary to an oesophagopleural fistula remains challenging, especially when primary closure of the fistula is delayed. Prolonged infection causes malnutrition and, in rare cases, pleurocutaneous fistula (PCF) formation. We report the case of a woman in the mid-40s presenting with spontaneous oesophageal rupture complicated by right-sided empyema thoracis. Chest tube thoracostomy was performed, and pleural cultures yielded Candida glabrata, for which micafungin was initiated. Accidental removal of the chest tube resulted in purulent wound discharge, prompting re-a

Risk factors for recurrent spontaneous pneumothorax: A population level analysis.

BACKGROUND: We sought to determine the rate and risk factors of recurrent spontaneous pneumothorax in a diverse population. METHODS: Cohort study using the California Public Discharge Data file (1995-2010). We identified patients with first-time spontaneous pneumothorax. The primary outcome was recurrent pneumothorax. Associations with clinical, patient, and hospital characteristics were assessed using Cox regression analysis. RESULTS: Among 14,609 patients with a first-time episode of spontaneous pneumothorax, 26.2% developed a recurrence. Risk factors included age <35 (Hazard Ratio [HR] 1.

Pneumothorax.

Pneumothorax may be classified as open or closed and as traumatic, spontaneous, or iatrogenic. The most common cause of pneumothorax is thoracic trauma. Spontaneous pneumothorax is often a result of bullous emphysema, and iatrogenic pneumothorax is an important complication of procedures involving the thoracic cavity. Most animals present with tachypnea, tachycardia, respiratory distress, and anxiety. Radiography and thoracocentesis are useful diagnostic aids. Traumatic and iatrogenic pneumothorax are commonly treated with thoracocentesis or thoracostomy tube placement. Spontaneous pneumothora

[Myotomy of the esophageal body].

Extramucosal myotomy involving the external longitudinal and internal circular layers of the musculature of the esophagus represent the surgical therapy in patients with dysphagia and regurgitation or with angina-like chest pain secondary to functional abnormalities of the musculature of the esophagel body and sphincters. Surgery has a palliative function, because cures symptoms and complication such a diverticula, but not the disease. Modern surgical techniques also prevent recurrence of symptoms and complications are minimal with better long-term results than conservative therapy. Myotomy of

[Spontaneous pneumothorax in cats: two case reports and literature review].

Spontaneous pneumothorax (SP) is a non-traumatic accumulation of air in the pleural cavity. This case report describes a cat with SP as a result of primary pulmonary adenocarcinoma. A second cat was diagnosed with primary pulmonary adenocarcinoma and asthma. A thoracostomy tube was inserted in the first cat while in the second cat a thoracostomy tube was placed and lobectomy of the right cranial and middle lung lobes was performed. Both cats died following treatment. The current literature reviewed here covers the comparative etiologies of SP as well as clinical presentation, diagnostic work-u

Management of AIDS-related pneumothorax.

BACKGROUND: Pneumothorax (PTX) occurs in 5% of patients with acquired immunodeficiency syndrome (AIDS) infected with Pneumocystis carinii pneumonia, and up to 50% of those will die during hospitalization. The treatment strategies for managing AIDS-related PTXs are often complex and ineffective at treating the PTX, and they can prolong hospitalization. METHODS: We reviewed our experience with 36 male patients with AIDS treated for 44 PTXs over a 2.5-year period to determine if a particular therapeutic approach could allow for an earlier recovery and effective treatment of the PTX. All patients

Case Report: Traumatic Tension Pneumothorax in a Pediatric Patient.

UNLABELLED: This case demonstrates the importance of quickly identifying a pediatric patient in acute respiratory distress secondary to pneumothorax and highlights the emergent management of these patients. The 16-year-old male patient presented to the emergency department (ED) with a chief complaint of right-sided rib pain and shortness of breath that began acutely after an all-terrain vehicle (ATV) accident. The patient was in respiratory distress upon presentation and had diminished right-sided breath sounds. A portable chest X-ray demonstrated pneumothorax with significant mediastinal left

[Computed tomography in the diagnosis of pulmonary barotrauma associated with the adult respiratory distress syndrome].

In the patients suffering from adult respiratory distress syndrome (ARDS), pulmonary barotrauma is a frequent and fearful complication, whose timely and accurate diagnosis is therefore needed. To this purpose, bedside chest films often exhibit some diagnostic drawbacks. Computed tomography (CT) of the chest is rarely used because of the problems concerning the transfer and the control of these critically ill patients outside the Intensive Care Unit. The chest CT findings of 84 ARDS patients were retrospectively reviewed, investigating: a) the presence of barotrauma (pulmonary bullae, pneumotho

The Perforation of the Superior Vena Cava Secondary to the Left Subclavian Dialysis Catheter.

INTRODUCTION: The perforation of the superior vena cava during the placement of dialysis catheter and consequent hemothorax is a rare serious complication. CASE REPORT: Dialysis catheter was placed in the left subclavian vein in a 69-year-old male patient with chronic renal insufficiency who was hospitalized for intracerebral hematoma. During hemodialysis a day after the procedure, the patient was noted having right-sided hemothorax, causing lethargy, dyspnea, hypotension, and bradycardia. Right tube thoracostomy was performed and 1500 cc of hemorrhagic fluid was drained. Under general anesth

Surgical salvage of acquired lung lesions in extremely premature infants.

Acquired neonatal lung lesions including pneumatoceles, cystic bronchopulmonary dysplasia, and pulmonary interstitial emphysema can cause extrinsic mediastinal compression, which may impair pulmonary and cardiac function. Acquired lung lesions are typically managed medically. Here we report a case series of three extremely premature infants with acquired lung lesions. All three patients underwent aggressive medical management and ultimately required tube thoracostomies. These interventions were unsuccessful and emergency thoracotomies were performed in each case. Two infants with acquired pneu

Should Anything Else be done Besides Prehospital Cardiopulmonary Resuscitation (CPR)? The Role of CPR and Prehospital Interventions After Traumatic Cardiac Arrest.

BACKGROUND: Cardiopulmonary resuscitation (CPR) following traumatic cardiac arrest (TCA) has been found to have limited effectiveness in the prehospital setting. While emergency medical services (EMS) initiated CPR following TCA is a common practice, variability exists due to differences in EMS perceived futility and survivability. These perceived differences have prevented the universal implementation of CPR following TCA. We hypothesized that some trauma patients may benefit from prehospital CPR following TCA. OBJECTIVES: We sought to identify factors associated with survival in TCA patient

Modified single-port non-intubated video-assisted thoracoscopic decortication in high-risk parapneumonic empyema patients.

BACKGROUND: Parapneumonic empyema patients with coronary artery disease and reduced left ventricular ejection fraction are risky to receive surgical decortication under general anesthesia. Non-intubated video-assisted thoracoscopy surgery is successfully performed to avoid complications of general anesthesia. We performed single-port non-intubated video-assisted flexible thoracoscopy surgery in an endoscopic center. In this study, the possible role of our modified surgery to treat fibrinopurulent stage of parapneumonic empyema with high operative risks is investigated. METHODS: We retrospecti

Is aggressive surgery in pleural empyema justified?

OBJECTIVE: High risk and a long hospitalization time are often quoted as negative aspects of aggressive surgery in pleural empyema. We did a retrospective analysis evaluating outcome and duration of hospitalization in patients treated according to an aggressive schedule. METHODS: Since 1989 we have treated 101 patients with pleural empyema (72 males, 29 females; mean age 50.3 years, range 11-91 years; 77 metapneumonic empyema, 24 empyema following trauma or abdominal surgery). Sixty-nine patients had had unsuccessful conservative pre-treatment (antibiotics, thorcozentses, drainage/irrigation,
