Subacute granulomatous thyroiditis is an uncommon disease that occurs most often in women in their second to fifth decades of life. This disease usually presents with thyroid tenderness, a low grade fever, and occasional dysphagia. The disease resolves spontaneously, usually without thyroid function abnormalities. We herein present the CT and MR imaging findings of two cases of subacute granulomatous thyroiditis.

OBJECTIVE: Autoimmunity plays an important role in the development of thyrotrophin (TSH) receptor antibodies and the pathogenesis of Graves' disease and Hashimoto's thyroiditis. On the other hand, subacute thyroiditis is a self-limited inflammatory disease of presumed viral aetiology. The aim of this study was to examine whether subacute thyroiditis triggers TSH receptor antibody-associated thyroid disorders.

Occurrence of various forms of thyroiditis is getting higher in the last ten years. Subacute or granulomatous or thyroiditis de Quervain has, most certainly, the greatest practical and clinical importance. We present a case of subacute thyroiditis in a female patient 50 years of age in whom Q-fever was positively diagnosed. The disease had had a benign course nevertheless the cause and it passed without any consequences.

The present study describes a case of subacute thyroiditis characterized by involvement of ocular motility that appeared many months before bioumoral findings of the disease. The authors believe that when the etiological diagnosis of an ophthalmoparesis, especially when isolated, is uncertain, one must carefully evaluate the function of the thyroid and levels of the specific antibodies, in order to start the appropriate treatment.

We describe an epidemic of self-limited (6 weeks) thyrotoxicosis which affected 12 index cases, 5 household contacts, and 6 retrospectively identified cases in July, August, and September, 1987 in the town of Winterswijk (28,011 inhabitants), The Netherlands. A small goiter was present in 9 of the 12 index patients, tender upon palpation in only 2. We report one case of subacute thyroiditis associated with acute hepatitis, which is histopathologically diagnosed. A 43-year-old woman visited our hospital with chief complaints of fever, sore throat and anterior neck pain. Thyroid gland was found to be swollen and tender. An unusual form of atypical subacute thyroiditis (SAT) occurred in 13 patients. The clinical presentation in all patients simulated systemic or malignant disease, involving fever of long duration and loss of weight without localized thyroid tenderness and without signs or symptoms of thyrotoxicosis. A study of changes in serum T4, T3, and Tg as well as of serum TSH response to TRH was done in ten patients with subacute thyroiditis, from the acute phase up to 56 mo. All patients had symptoms of thyrotoxicosis. It is difficult to study subacute thyroiditis epidemiologically since it occurs sporadically and infrequently. Information about 1,127 cases (108 males, 1,019 females, from 1967 to 1982) of subacute thyroiditis in northern Japan was obtained through a questionnaire. We present two cases of subacute thyroiditis in which increased radionuclide activity in the affected areas was observed on 201 Tl thyroid scintigrams. These were taken to evaluate cold areas on the 99m Tc-pertechnetate thyroid scintigrams. This finding suggests that 201 Tl may be a potentially useful radionuclide in visualizing inflammatory lesions.
