CBCT Use in Endodontic Diagnosis.

Endodontic diagnosis and treatment planning has taken a giant leap forward due to introduction of CBCT in dentistry. While conventional 2-D radiographs remain the most cost-effective and routine method to evaluate a patient’s dentition, their diagnostic potential is limited. The 3-D manipulation of images that CBCT offers provides better insight into diagnostic dilemmas and complicate treatment decisions. Despite the advantages of CBCT imaging, it should be used complimentary to 2-D radiography, not as a replacement. The principle of ALARA (in which patients should be exposed to radiation “as

A retrospective analysis of factors influencing the success of autotransplanted posterior teeth.

BACKGROUND: Survival and success rates of tooth transplantations even after long follow-up periods have been shown to be very high. Nevertheless, it is important to analyse factors potentially influencing these rates. The aim of this study was to assess the influence on success of potential factors. METHODS: The research was based on a retrospective analysis of clinical and radiological data from a sample of 59 subjects (75 transplanted teeth). The follow-up period varied from 0.44 to 12.28 years (mean 3.95 years). Success rates were calculated and depicted with Kaplan-Meier plots. Log-rank t

Applying extrusive orthodontic force without compromising the obturated canal space.

BACKGROUND: Complicated tooth fractures can be the unfortunate result of orofacial trauma and can offer a therapeutic challenge for the dentist. A conservative solution for gaining supragingival sound tooth structure often includes orthodontic forced eruption. Usually, this procedure is carried out by applying extrusive force after placing a provisional acrylic Richmond crown on the tooth. However, this long-lasting dental treatment may jeopardize the coronal seal of the root canal space, leading to microleakage and endodontic failure. CASE DESCRIPTION: Orthodontic forced eruption demands app

Influence of cone-beam computed tomographic scan mode for detection of horizontal root fracture.

INTRODUCTION: The purpose of the present study was (1) to test the accuracy of a small-volume cone-beam computed tomographic (CBCT) device in detecting horizontal root fractures (HRFs) in teeth with and without an intracanal metallic post (IMP) and (2) to investigate the use of 2 different acquisition protocols of a CBCT device for HRF diagnosis. METHODS: Forty endodontically treated teeth with and without an IMP were examined using PreXion 3D CBCT scanner (Terarecon, San Mateo, CA) with a 5-cm high and 5-cm diameter cylinder at 0.10-mm voxel reconstruction. Two observers analyzed the samples

Bone morphology after delayed tooth replantation - case series.

The purpose of this report was to describe the morphological changes in the alveolar bone after delayed replantation of avulsed teeth using three dimensional cone-beam computed tomography in 11 during the time period 2003-2012. The radiographic observations revealed the following: Delayed replantation results in ankylosis-related replacement root resorption; the resorption is delayed or arrested around the cervical area superior to the alveolar crest. The buccal bone is reduced in thickness but not the palatal bone. The buccal bone resorption of the alveolar crest progresses approximately to t

Using soft tissue graft to prevent mid-facial mucosal recession following immediate implant placement.

BACKGROUND: With advances in dental technology, the placement of immediate implants has progressively gained popularity. However, a common complication that surfaced was mid-facial mucosal recession, which impaired esthetic outcomes. The use of soft tissue grafts has thus been introduced to manage and prevent this undesirable result. To date, no guidelines have been put forward for clinicians to identify the indications for a soft tissue graft during immediate implant placement. Therefore, this manuscript aimed to propose a decision-making model to prevent mid-facial mucosal recession followin

Prosthodontic treatment of a patient taking nitrogen-containing bisphosphonates to preserve the integrity of the epithelial attachment: a clinical report.

Osteonecrosis of the jaw has been identified in recent years as an unfortunate potential side effect for patients taking nitrogen-containing bisphosphonates. Many authors have attempted unsuccessfully to elucidate the mechanism behind this drug-induced malady, leaving dentists with unclear guidelines on how to treat these patients. Recent literature suggests a threshold-related impairment of epithelial cell migration over bone. A conservative, non-invasive approach aimed at preserving the epithelial attachment in a patient on a long-term oral alendronate protocol is described. The advantage of

Remaining tooth structure associated with various preparation designs for the endodontically treated maxillary second premolar.

Existing literature suggests a relationship between the amount of remaining tooth structure and the fracture resistance of the restored endodontically treated tooth. This study investigated the amount of tooth structure remaining following various tooth preparations used in the restoration of the endodontically treated maxillary second premolar. Illustrations of the maxillary second premolar in buccopalatal, mesiodistal and occlusal sections were drawn to scale. Outlines of various intra- and extracoronal preparations were superim-posed on the illustrations to reveal the amount of tooth tissue

Subjective sensation and objective neural discharges recorded from clinically nonvital and intact teeth.

The aim of this study was to compare subjective sensation with objective neural discharges recorded by microneurography. We examined human teeth that did not respond to pulp vitality testing, but that responded to cavity preparation for endodontic treatment (pathophysiological). Intact teeth and endodontically obturated teeth were used as controls. Pulpal blood flow in the clinical crown and histological examination were also used. Most teeth, both in normal and pathophysiological conditions, did not respond to all pulp vitality tests. Even when teeth in the pathophysiological group showed spi

Biomechanical influence of residual lateral wall thickness and resin reinforcement on the performance of endocrowns in premolars.

BACKGROUND: Restoring endodontically treated premolars (ETPMs) with mesio-occluso-distal (MOD) cavities poses clinical challenges due to reduced coronal thickness and heightened fracture risk. Despite advances in adhesive dentistry, the biomechanical effects of residual lateral wall (RLW) thickness and composite resin reinforcement on the performance of ceramic endocrowns in ETPMs remain underdefined. This study aims to evaluate how RLW thickness and resin reinforcement influence fracture resistance, stress distribution, and failure modes in ETPMs restored with ceramic endocrowns. METHODS: Fo
