This prospective, randomized clinical trial assessed the effects of mitomycin C and cyclosporin A used as antimetabolites in trabeculectomy on the post-operative IOP and success rate. PURPOSE: To report on sixty-eight patients (98 eyes) who were trabeculectomized with the use of mitomycin C as adjunctive therapy.
METHODS: Fifty-six patients (79 eyes) were followed for at least 6 months or had IOP of >21 mm HG or any complications that were considered end point for them. Forty-six were primary congenital glaucoma; one was Sturger Weber disease; five were Axenfeld-Rieger syndrome; two were Peters anomaly; one was congenital glaucoma associated with ectopia lentis and one aniridia. Ninety-three consecutive trabeculectomies in which mitomycin C (MMC) was applied intraoperatively, either at a concentration of 0.5 mg/mL for 5 minutes (n = 48) or at a concentration of 0.4 mg/mL for 3 minutes (n = 45), were reviewed to determine whether there was a significant difference between the two groups in terms of intraocular pressure (IOP) control and the incidence of complications. Preoperative and postoperative IOPs in the two groups did not differ significantly (P = .817 and .131, respectively). The 5-minute group had a higher incidence of serous choroidal detachments (P = .001).

BACKGROUND: The "true" prevalence and clinical attributes of exfoliation glaucoma remain controversial. The authors studied these characteristics in glaucoma patients requiring trabeculectomy.
METHODS: One hundred consecutive patients undergoing trabeculectomy for open-angle glaucoma were investigated by clinical examination (biomicroscopy and gonioscopy) and classified into three categories: exfoliation glaucoma, possible exfoliation glaucoma, and primary open-angle glaucoma (POAG). A definitive diagnosis of exfoliation glaucoma was provided by pathologic examination of iris tissue.

Various mechanisms have been proposed to explain the pathogenesis of infantile glaucoma in the Sturge-Weber syndrome. We report the results of treating six patients with combined trabeculectomy and cyclocryotherapy. Successful control of the intraocular pressure occurred in all patients. Two patients required a second procedure at another site. We feel that the success of this procedure provides evidence that the glaucoma is secondary to elevated episcleral venous pressure in this condition.
