OBJECTIVE: The aim of this study was to evaluate the relationship between the degree of preoperative pain and trismus with the development of complications following the repair of isolated unilateral compound mandibular body fractures using a closed reduction technique.
SUBJECTS AND METHODS: This was a 7-year prospective study carried out at the Dental and Maxillofacial Surgery Clinic of the University of Calabar Teaching Hospital, Calabar, Nigeria. Of a total of 97 patients, 83 (85.6%) subjects (66 males, 17 females, ratio 5:1) were preoperatively evaluated for trismus and pain in a blinded …

Septic arthritis of the temporomandibular joint (TMJ) is rare, but patients with this diagnosis are at high risk for significant morbidity. We present a case of a 15-year-old man who presented with increasing trismus for 3 days. He had only minimal discomfort and swelling of the right cheek and temporal region, and pericoronitis of an impacted wisdom tooth was suspected. Under intravenous antibiotics, pain subsided, but trismus and a slight swelling remained. CT scan showed septic arthritis of the TMJ with an abscess formation penetrating into the epidural space. Immediate drainage and craniot…

Intraoral evaluation with ultrasound has been shown to be an ideal method for differentiating between peritonsillar abscess (PTA) and peritonsillar cellulitis. Unfortunately, many patients experience significant trismus and are unable to tolerate the intracavitary probe. Evaluation of the submandibular space with a high frequency linear transducer from the external aspect of the neck affords an alternative technique that has been shown to have high specificity for PTA. Unfortunately, there are no reported cases in the emergency medicine literature detailing this novel technique. We present a c…

To evaluate whether Er: YAG surgery and Nd: YAG-based low-level laser therapy (LLLT), combined with medical collagen, improve early postoperative outcomes after extraction of impacted mandibular third molars. Adults undergoing surgical removal of mesioangular or horizontal impacted mandibular third molars were randomized equally (1:1:1:1) to blank control (BCG), collagen only (COL), lasers only (LAS), or collagen plus lasers (COL + LAS). Primary endpoints were pain (VAS; spontaneous and swallowing; POD1/3/7), swelling (POD1/3/7), trismus (POD1/3/7), and early mucosal healing (POD3/7). Secondar…

INTRODUCTION: Tetanus is a rare but life-threatening disease caused by Clostridium tetani, characterized by generalized muscle rigidity, autonomic instability, and respiratory compromise.
CASE REPORT: We present a case of a 65-year-old male poultry hobbyist with type two diabetes who developed severe tetanus following a rooster-peck injury to his finger. The patient presented with progressive trismus and respiratory distress necessitating fiberoptic nasotracheal intubation and subsequent tracheostomy. Despite appropriate antimicrobial therapy, immunoglobulin administration, and sedation, the …

Myositis ossificans (MO) is a rare benign condition characterized by heterotopic bone formation within soft tissues, with masticatory muscle involvement being exceptionally uncommon. This case report describes a 45-year-old male patient who developed progressive trismus and a preauricular mass six months after right frontotemporal craniotomy for meningioma resection. Imaging revealed an ossified lesion in the temporalis muscle, consistent with post-traumatic MO. Surgical management involved zygomatic arch osteotomy and coronoidectomy, restoring intraoperative mouth opening to 23 mm. Histopatho…

Although unilateral peritonsillar abscesses (PTA) are a common complication of acute bacterial tonsillitis, bilateral PTA are quite rare. We present the case of a 14-year-old female teenager with a 1-week history of acute tonsillitis. Physical examination revealed significant trismus, symmetrically inflamed tonsils and soft palate, and a midline uvula. A contrast-enhanced CT scan of the neck demonstrated bilateral PTA. After bilateral needle aspiration and administration of antibiotics, complete disease resolution was rapidly achieved. Patients with bilateral PTA present a diagnostic dilemma b…

Tetanus remains a significant health concern, particularly in older adults with waning immunity. The diagnosis is typically clinical, based on characteristic features such as trismus, muscle rigidity, and stimulus-triggered spasms. However, diagnostic challenges arise when tetanus presents without an identifiable acute wound or with an occult portal of entry, as it may be misattributed to more common neurological conditions such as stroke or encephalitis. We report a 65-year-old Chinese male who presented with 15-day cough and sputum, followed by acute trismus and dysphagia. Initial evaluation…

BACKGROUND: Impacted lower third molar surgeries involve trauma in a highly vascularized zone with loose connective tissue leading to inflammatory sequelae including postoperative pain, swelling, trismus and generalised oral dysfunction during the post-operative phase. In minor oral surgical procedures, an all-inclusive method to protract anaesthesia and reduce the inevitable post-operative sequelae is yet to be explored substantially.
AIM: To evaluate the efficacy of dexamethasone added to local anaesthetics in extending the depth and duration of anaesthesia and decreasing the postoperative …

Tetanus is a rare but potentially fatal infectious disease caused by Clostridium tetani, characterized by muscle rigidity, trismus, and dysphagia. We report a case of tetanus mimicking post-radiotherapy dysphagia in a 68-year-old male with a history of hypopharyngeal and esophageal cancers treated with chemoradiotherapy. The patient presented with acute-onset dysphagia and speech difficulty, which were initially attributed to radiation-induced sequelae. Because dysphagia and pharyngeal discomfort are common complaints in post-chemoradiotherapy patients, and laryngoscopic findings were unchange…

Giant cell tumors (GCT) of the bone are uncommon primary bone neoplasms that occur mainly in the epiphyses of long bones. However, GCT with tumoral growth in the cranium is rarely encountered, particularly in the temporal bone. GCT demonstrates benign histological features but is locally aggressive, and surgical excision is the treatment of choice. The authors describe a 31-year-old man who presented with trismus and an extensive GCT, which originated in the petrous and squamous portions of the temporal bone and extended into the left mandibular ramus and middle ear. He was treated by en bloc …

Cephalic tetanus is defined as a combination of trismus and paralysis of one or more cranial nerves. Cranial nerves III, IV, VI, VII, and XII may be affected, but the facial nerve is most frequently implicated. A 64-year-old female visited hospital for left ptosis followed by facial palsy after a left forehead abrasion in a car accident. At nine days post injury, left ptosis developed, left facial palsy developed twelve days post injury, and at fifteen days post injury, trismus and dysphagia developed. The following day, there was progression of symptoms to generalized tetanus, such as dyspnea…

Tetanus is a neurologic disorder caused by tetanospasmin, a protein toxin elaborated by Clostridium tetani. Cephalic tetanus is a localized form of the disease causing trismus and dysfunction of cranial nerves. We report the case of a man who presented with facial trauma, complete ophthalmoplegia, exophthalmos, areactive mydriasis, and periorbital hematoma. An orbital CT revealed air bubbles in the right orbital apex. The patient was given a tetanus toxoid booster and antibiotherapy. After extraction of a wooden foreign body, the patient developed right facial nerve palsy, disorders of swallow…

Mandibular osteomyelitis in paediatric population presents as painful swelling with progressive trismus. Often the aetiology for this form of progressive osteomyelitis is inconclusive. The infective aetiology in this condition is difficult to assign as bone tissue culture is found to be seldom positive. We present a case of an 11-year-old girl, with hearing and speech impairment, who presented with report of painful right-sided mandibular swelling with progressive trismus. Clinical, radiographic and histopathological findings were suggestive of sclerotic osteomyelitis. Bone tissue culture grew…

BACKGROUND: Mandibular fractures are rare in children. Symptoms of these fractures include preauricular pain and swelling, trismus and pain when opening the mouth, and they are often preceded by a fall on the chin. If the diagnosis is missed they may have serious consequences for the growth and development of the jaw.
CASE DESCRIPTION: A 3-year-old boy was referred with a swelling in his left cheek, fever and vomiting. Two days earlier he had fallen on his chin. At physical examination he had severe preauricular pain on palpation and a trismus. The patient was admitted because of suspicion of…

Oral submucous fibrosis (OSMF) is an insidious chronic progressive disease of the oral cavity which is considered as a precancerous condition. The suggested aetiological factor is the practice of certain customs/habits of the inhabitants of South East Asia. As the disease progresses, the oral mucosa becomes stiff due to the formation of fibrotic bands in the cheek, ultimately leading to reduced mouth opening and trismus. The early form of the disease can be treated by medicinal means, however, advance form needs surgical excision of the fibrotic tissue bands. The defect created due to the exci…
