White dot syndrome in a patient with presumed ocular tuberculosis: a case report.

BACKGROUND: This manuscript describes a case of a patient with presumed ocular tuberculosis masquerading as multiple evanescent white dot syndrome. CASE PRESENTATION: A 32-year-old male patient presented with a complaint of reduced visual acuity in the left eye. Retinal fundus exam of the left eye revealed gray-whitish deep lesions predominantly nasal to the optic disc. The lesions were more clearly identifiable on fundus autofluorescence (FAF) imaging, fluorescein angiography (FA) and en face optical coherence tomography (OCT). FA also indicated retinal vasculitis and papillitis. Swept-sourc

Global Variations and Challenges With Tubercular Uveitis in the Collaborative Ocular Tuberculosis Study.

PURPOSE: The aim of this study was to examine regional variation management practices and outcomes for tubercular uveitis (TBU). METHODS: In this retrospective multinational cohort study, patients from 25 international eye care centers diagnosed with TBU with a minimum follow-up of 1 year were included. The geographic variation in treatment outcomes on survival analysis in patients with TBU were analyzed. Treatment failure is defined as a persistence or recurrence of inflammation within 6 months of completing antitubercular therapy, the inability to taper oral corticosteroids to less than 10

An unusual case of ocular tuberculosis presenting as subretinal abscess with posterior scleritis.

We report an unusual presentation of ocular tuberculosis with subretinal abscess with associated posterior scleritis. This is a first-of-its-kind report of such an association. A 26-year-old female presented to us with complaints of redness, pain, and decreased vision in her left eye since 1 week. On examination, the vision in her right eye was 6/6 and in the left eye was 1/60. Detailed ocular evaluation revealed exudative retinal detachment, subretinal abscess, and disk edema. Ocular and systemic investigations were consistent with presumed tubercular etiology and posterior scleritis. The pat

Disseminated tuberculosis with ocular tuberculoma and disseminated candidiasis in acquired immunodeficiency syndrome.

A 39 years man presented with painless progressive loss of vision and proptosis of left eye associated with constitutional symptoms. He was diagnosed to have acquired immunodeficiency syndrome (AIDS) with disseminated tuberculosis (TB) and invasive candidiasis. The proptosis was due to a retrobulbar mass. The uncommon occurrence of retrobulbar tuberculoma and invasive candidiasis is discussed and relevant literature is reviewed.

Epithelial Keratitis Secondary to Presumed Ocular Tuberculosis: A Report of Two Cases.

This report presents two rare cases of presumed epithelial keratitis secondary to ocular tuberculosis (TB), highlighting an unusual clinical presentation that may be underrecognized. Two male patients from a TB-endemic region presented with unilateral, chronic epithelial keratitis in a semicircular pattern, with mild anterior stromal infiltration and reduced corneal sensitivity. Both were unresponsive to standard topical and systemic treatments including antivirals and corticosteroids. Diagnostic evaluation included Mantoux testing and systemic screening. Mantoux tests were positive in both pa

Design and synthesis of natural antibacterial derivatives for ocular tuberculosis: a comprehensive review.

Ocular tuberculosis (TB) is an underrecognized extrapulmonary manifestation of Mycobacterium tuberculosis (M. tuberculosis) infection that can result in irreversible vision loss. Current systemic therapies, including isoniazid, rifampicin, pyrazinamide, and ethambutol, are often inadequate in achieving therapeutic intraocular concentrations and may pose ocular toxicity risks. The eye's unique anatomical and physiological barriers, including the cornea, blood-aqueous, and blood-retinal barriers, limit drug penetration, particularly to the posterior segment. This paper explores the potential of

Epidemiology and Clinical Characteristics of Ocular Tuberculosis in the United States, 1993-2019.

BACKGROUND: Data regarding ocular tuberculosis (OTB) in the United States have not been previously reported. We evaluated trends of OTB compared with other extrapulmonary TB (EPTB). METHODS: We estimated the proportion of all EPTB cases (with or without concurrent pulmonary involvement) with OTB reported to the National Tuberculosis Surveillance System during 1993-2019. We compared demographics and clinical characteristics of people with OTB and other EPTB during 2010-2019. P values were calculated by chi-square test for categorical variables and Kruskal-Wallis for continuous variables. RESU

Evaluating Truenat Assay for the Diagnosis of Ocular Tuberculosis and Detection of Drug Resistance.

BACKGROUND: Truenat MTB Plus assay was evaluated for diagnosing ocular tuberculosis (OTB) and detecting multi-drug resistant (MDR) and extremely-drug resistant (XDR) OTB. METHODS: A total of 75 vitreous fluid specimens [five confirmed OTB, 40 clinically suspected OTB and 30 controls] were subjected to Truenat MTB Plus, multiplex PCR, and Xpert Ultra. Chips of Truenat were used for detecting rifampicin, isoniazid, fluoroquinolone and bedaquiline resistance. The performance was compared against culture, composite reference standard, and gene sequencing. RESULTS: The overall sensitivity of True

Need for a holistic approach to treat TB contacts and extrapulmonary (ocular) tuberculosis.

We aim to highlight the ignorance and incidences of deliberate hiding of medical facts in society. The cause of it can be, the associated taboo with these diseases. The fear of social outcasts is the major barrier preventing diseased from accepting the diagnosis as well as treatment. Though the medical facility has improved significantly and has come up with a complete cure for these diseases, still these facilities are not able to trickle down to the lower socioeconomic group. The reason for facilities not reaching the lower socioeconomic strata is the self-made shield created by these people

The impact of ocular tuberculosis on vision after two months of intensive therapy.

Tuberculosis is an infectious disease of global importance with major economic and social burden accounting for 25% of all avoidable deaths in developing countries. Extrapulmonary involvement may occur either in association with clinically apparent pulmonary tuberculosis or in isolation. This cross-sectional descriptive study aimed to evaluate the impact of ocular tuberculosis in visual acuity at baseline and after two months of intensive anti-tuberculous therapy. A sample of 133 pulmonary tuberculosis patients, seven disseminated tuberculosis, and three pleural tuberculosis patients was evalu

Rare case of ocular tuberculosis in a diabetic patient: diagnostic and therapeutic dilemmas.

We present the case of a patient who was diagnosed by chance with macular hypopyon during a conventional interdisciplinary examination. The clinical context and the association of a systemic disease, such as uncontrolled type 1 diabetes, rendered further investigations in this patient. Due to his immunocompromised status, etiology such as ocular fungi, lymphomas, tuberculosis was taken into account. Thorough complex investigations oriented the diagnosis towards ocular tuberculosis involvement.

Tubercular Uveitis with Ocular Manifestation as the First Presentation of Tuberculosis: A Case Series.

Tuberculosis is very common disease in India. It is one of the most common causes of Granulomatous Uveitis in our Country even today. So the strongest suspicion in our mind when we are treating a case of Uveitis, should be TB. We reviewed all the cases of clinically suspected ocular tuberculosis attending the Ophthalmology OPD of Sri Siddhartha Medical College between December 2012 and December 2014 who were refractory to routine uveitis management and later on responded to anti-Tubercular treatment. History of TB contact, Ocular manifestation, Demographic Profile of the patients, Diagnostic t
