BACKGROUND: Drug-resistant tuberculous pleurisy is difficult to diagnose in children because pleural fluid is paucibacillary, culture is slow and frequently negative, and Xpert detects only rifampicin resistance.
CASE PRESENTATION: We report two children in whom thoracoscopic pleural biopsy combined with PCR-high-resolution melting (HRM) drug-resistance testing identified drug-resistant tuberculosis. Case 1, an 8-year-old girl with tuberculous pleurisy, showed persistent pleural thickening and a new mass after two months of standard first-line therapy; pleural tissue PCR detected rpoB and inh

The levels of soluble IL-2R were measured in pleural fluid from patients with tuberculosis pleurisy. There were significantly elevated soluble IL-2R values in tuberculous pleural fluid as compared with pleural fluid of nontuberculous etiology including malignant, bacterial and transudative pleural effusions. In patients with tuberculous pleurisy, the level of soluble IL-2R in pleural fluid was markedly greater than that in serum. Furthermore, a significant positive correlation was observed between soluble IL-2R levels and adenosine deaminase levels in tuberculous pleural fluid. These findings

Tuberculosis, being an airborne disease with a broad spectrum of symptoms and signs, is a rare cause of chylothorax. Mortality due to tuberculous chylothorax is 6%, and bilateral chylothorax has a higher mortality with a poor prognosis. A previously well 28-year-old male with a household contact history of tuberculosis exposure presented with a 1-month history of shortness of breath, fever, pleuritic chest pain, and constitutional symptoms. He was found to have bilateral chylothorax, generalized lymphadenopathy, hepatosplenomegaly, and miliary nodules in chest x-ray. Sputum acid-fast bacilli,

OBJECTIVE: The aim: Studying the features of the proteolytic system in patients with tuberculosis depending on the sensitivity of the pathogen.
PATIENTS AND METHODS: Materials and methods: In the course of the research we studied the level of elastase in the blood of 111 patients. The first group consisted of 66 (59.5%) people with pulmonary tuberculosis (39 were sensitive to antibacterial drugs, 27 were resistant). The second group included 13 (11.7%) patients with tuberculous pleurisy. The third group consisted of 32 (28.8%) patients with dual localization of the process (pulmonary tubercul

In order to assess the confusion attendant with current definitions of extrapulmonary tuberculosis, pleural and miliary disease, a set of 37 papers published which analyzed disease states were assessed for how the terms adhered to official definitions of the American Thoracic Society, the World Health Organization, and the Centers for Disease Control. The findings showed uncertainties in the classification of extrapulmonary disease, a frequent inconsistency in the inclusion of pleural disease within pulmonary rather than extrapulmonary disease, and the ambivalent use of the terms pleural and d…

The diagnosis of pleural tuberculosis (plTB) by the analysis of pleural effusions (PEs) with standard diagnostic tools is difficult. In routine clinical practice, the present authors evaluated the performance of a commercially available Mycobacterium tuberculosis (MTB)-specific enzyme-linked immunospot assay on peripheral blood mononuclear cells (PBMCs) and pleural effusion mononuclear cells (PEMCs) in patients with suspect plTB. The T-SPOT.TB test (Oxford Immunotec Ltd, Abingdon, UK) was performed on PBMCs and PEMCs in 20 patients with a clinical and radiological suspect of plTB and in 21 con

OBJECTIVES:: Pleural effusion is a common diagnostic and clinical problem. The differential diagnosis of pleural effusion may be difficult and may require several procedures, including invasive ones. Certain studies have investigated biochemical parameters to facilitate the diagnosis of exudative pleural effusion; however, it remains a challenging problem in clinical practice. We aimed to investigate the potential role of the neutrophil-lymphocyte ratio, which can be easily obtained by determining the cell count of the pleural fluid, in the differential diagnosis of exudative pleural effusion.

BACKGROUND: Differential diagnosis between tuberculous pleurisy (TBP) and non- tuberculosis pleural effusion represents a critically important clinical problem. In recent years, several noninvasive methods have been found for diagnosis of tuberculous pleurisy. This study aimed to evaluate the value of detection of the genome of Mycobacterium tuberculosis (MTB) by polymerase chain reaction (PCR) method for the diagnosis of tuberculous pleurisy and compare the results with those of conventional methods.
MATERIALS AND METHODS: In this cross-sectional study, we studied 62 patients (42 men and 20

Tuberculous pleuritis is a common manifestation of extrapulmonary tuberculosis and is the most common cause of pleural effusion in many countries. Conventional diagnostic tests, such as microscopic examination of the pleural fluid, biochemical tests, culture of pleural fluid, sputum or pleural tissue, and histopathological examination of pleural tissue, have known limitations. Due to these limitations, newer and more rapid diagnostic tests have been evaluated. In this review, the authors provide an overview of the performance of new diagnostic tests, including markers of specific and nonspecif

We have found that natural killer (NK) cells were very active in pleural effusions containing Mycobacterium tuberculosis. Cytotoxicity against K562 and Raji was augmented when the mononuclear cells were cocultured for 18 hr with purified protein derivative (PPD) derived from M. tuberculosis culture supernatants. In pleural effusions of cancer patients, PPD-activated mononuclear cells were less cytotoxic than their counterparts in peripheral blood. However, in the same patients, interferon and interleukin-2 production was greater in pleural effusions than in peripheral blood. On the other hand,

BACKGROUND: Approximately 20% of exudative pleural effusions remain undiagnosed even after diagnostic thoracentesis. In such situations, pleural biopsy is essential for establishing a diagnosis. Pleural biopsy can be obtained either through invasive procedures, such as medical or surgical thoracoscopy, or through semi-invasive techniques, such as closed pleural biopsy, with or without image guidance. This study compared the efficacy and complications of transthoracic ultrasound-guided versus medical thoracoscopic pleural biopsy for undiagnosed exudative pleural effusion.

We have recently reported an increased heterogeneity in the human immunodeficiency virus type 1 (HIV-1) envelope gene (env) in HIV-1-infected patients with pulmonary tuberculosis (TB) compared to patients with HIV-1 alone. This increase may be a result of dissemination of lung-derived HIV-1 isolates from sites of Mycobacterium tuberculosis infection and/or the systemic activation of the immune system in response to TB. To distinguish between these two mechanisms, blood and pleural fluid samples were obtained from HIV-1-infected patients with active pleural TB in Kampala, Uganda (CD4 cell count

Sixty-four non-immunocompromised and adult patients with proved tuberculosis were examined. Twenty-three of them had bacteriologic confirmation with sputum examination, 7 with biopsy, 8 with bronchoscopy and bronchial lavage, 3 with pleural effusion examination and 3 with specific ex-juvantibus therapy. Fifteen patients had positive Mantoux skin tests. Thirty-nine patients had primary and 25 postprimary tuberculosis. All patients underwent chest radiography and 37 underwent also CT scans of the chest. The radiographic findings in primary tuberculosis were: 33 parenchymal consolidation patterns

BACKGROUND: Medical thoracoscopy (MT) is primarily employed to obtain histopathological evidence via parietal pleural biopsy. However, parietal pleural lesions are not uniformly accessible, and evidence on visceral pleural biopsy remains limited.
OBJECTIVES: This study aimed to investigate the diagnostic sensitivity and safety of visceral pleural biopsy performed under semi-rigid MT in patients with undiagnosed exudative pleural effusions (EPEs).
DESIGN: Single-center retrospective observational study.
METHODS: A retrospective analysis was conducted on 459 patients with undiagnosed EPEs who
