Pharmacology of parturition.

Uterine contractions in labour are influenced by endogenous substances such as oestrogens, progesterone, cortisol, oxytocin, prostaglandins, relaxin, adrenergic and cholinergic secretions, cyclic nucleotides and calcium ions. Effects of progesterone and oestrogens are complimentary as well as antagonistic to each other. They regulate formation of gap junctions, influx of calcium ions, synthesis of oxytocin, adrenergic receptors and of prostaglandins and cyclic nucleotides. Cortisol shares a role in a more complex endocrine trigger but is ineffective alone in the initiation of human labour. Adr

The clinical use of intrauterine pressure catheters.

We question the need for an intrauterine pressure catheter (IUPC) to monitor uterine contractions during the induction or augmentation of labor with oxytocin, or to verify adequate uterine contractions to support the diagnosis of arrest of labor. There are appropriately conducted clinical trials demonstrating that uterine contraction frequency in spontaneous, induced and augmented labors obtained with the use of external tocography is sufficient for the clinical management of labor. The value of an IUPC is further diluted by the inherent variability of pressure recordings with its use and the

The cyclic-ADP-ribose signaling pathway in human myometrium.

Human myometrial contraction plays a fundamental role in labor. Dysfunction of uterine contraction is an important cause of failure in progression of labor. The mechanisms of control of uterine contractions are not completely understood. It appears that intracellular Ca(2+) mobilization may play an important role during uterine contraction. Several mechanisms of intracellular Ca(2+) mobilization have been described. However, in human uterus only the inositol 1,4,5-trisphosphate-induced Ca(2+) release has been extensively studied to date. In view of the identification of the presence of functio

Aster spathulifolius Maxim. Alleviates Primary Dysmenorrhea in a Mouse Model by Modulating Myometrial Contractions via NF-κB/COX-2 Pathway Inhibition.

Primary dysmenorrhea (PD) is characterized by excessive uterine contraction and ischemic vasoconstriction, primarily driven by elevated levels of prostaglandins (PGs; PGF2α) and inflammatory mediators. Nonsteroidal antiinflammatory drugs (NSAIDs) remain the standard treatment for PD; however, their associated adverse effects necessitate the use of alternative therapeutic strategies. Aster spathulifolius Maxim. is a perennial herb native to the coastal regions of Korea, that exhibits antiviral, anticancer, and antidiabetic effects. In this study, we investigated the potential of Aster spathulif

Quantitative analysis for grading uterine electromyography activities during labor.

BACKGROUND: The strength of uterine contraction is one of the decisive factors for labor progression and parturition. Clinicians usually encounter difficulties in early identification of inadequate contractions and in oxytocin treatment. Electromyography-an emerging technology for uterine contraction monitoring-can quantify the intensity of myoelectric activity of uterine contraction. Therefore, grading patients with different uterine contraction intensities by electromyography is of great significance to the clinical intensive management of uterine contraction and labor process. OBJECTIVE: T

Uterine contractility evaluated on cine magnetic resonance imaging.

The inherent contractility of the uterus has attracted concern of researchers and exhibits two known patterns of contraction. One is focal and sporadic bulging of the myometrium, and the other is rhythmic and subtle stripping movement in the subendometrial myometrium, known as uterine peristalsis. Recent development in the ultrafast magnetic resonance imaging (MRI) techniques has enabled us to observe these uterine contractions directly and noninvasively. Although the mechanisms and relationship of these two kinds of myometrial contractions and their relations to age, pharmaceutical administra

Characterization and automatic classification of preterm and term uterine records.

Predicting preterm birth is uncertain, and numerous scientists are searching for non-invasive methods to improve its predictability. Current researches are based on the analysis of ElectroHysteroGram (EHG) records, which contain information about the electrophysiological properties of the uterine muscle and uterine contractions. Since pregnancy is a long process, we decided to also characterize, for the first time, non-contraction intervals (dummy intervals) of the uterine records, i.e., EHG signals accompanied by a simultaneously recorded external tocogram measuring mechanical uterine activit

Clinically relevant concentrations of dexmedetomidine may reduce oxytocin-induced myometrium contractions in pregnant rats.

BACKGROUND: Recently, there have been some trials to use dexmedetomidine in the obstetric field but concerns regarding the drug include changes in uterine contractions after labor. We aimed to evaluate the effects of dexmedetomidine on the myometrial contractions of pregnant rats. METHODS: In a pilot study, the contraction of the myometrial strips of pregnant Sprague-Dawley rats in an organ bath with oxytocin at 1 mU/ml was assessed by adding dexmedetomidine from 10-6 to 10-2 M accumulatively every 20 min, and active tension and the number of contractions were evaluated. Then, changes in myom

Effects in vitro of progesterone and estradiol-17 beta on the contractile and electrical responses in rat myometrium.

Uterine longitudinal and circular muscles from pre- and post-term rats were studied in vitro for their contractile and electrical activities under the influence of 20-100 microM progesterone, 20 microM estradiol-17 beta, or 20 microM stilbestrol. Uterine longitudinal or circular muscle strips were irrigated by Krebs solution at 37 degrees C containing one of the hormones mentioned above, dissolved by the use of ultrasonic wave. Muscle contractions were recorded by a force displacement transducer, and electrical activities of the muscle membrane were studied using an intracellular microelectrod

Inhibition of prostaglandin-induced uterine activity by nifedipine.

The effects of the calcium antagonist, nifedipine, on uterine activity induced by prostaglandins F2 alpha (PGF2 alpha) and E2 (PGE2) were studied in women undergoing therapeutic midtrimester abortion, and in patients with a missed abortion in the 16th to 27th week of pregnancy. In the five subjects receiving intra-amniotic PGF2 alpha (25 to 40 mg) for midtrimester abortion, nifedipine (30 mg orally) decreased uterine activity from a mean of 372 to 203 Montevideo Units. The effect on the intensity of the contractions was pronounced; frequently and basal tone were little affected. In patients wi

Phase-plot analysis of the oxytocin effect on human myometrial contractility.

OBJECTIVE: Analysis of uterine contractility in vitro is usually confined to measuring a few traditional parameters of uterine contractility, such as contraction amplitude, frequency and area under the curve. In this paper, we describe parameters that provide additional information obtained from the traces of force and its first derivative. We propose an improved contractility index which is less dependent on variability between samples. STUDY DESIGN: Standard organ bath recording of myometrial contractions in the presence or absence of oxytocin on samples of human myometrium obtained from 26

Identification of first-stage labor arrest by electromyography in term nulliparous women after induction of labor.

INTRODUCTION: Worldwide induction and cesarean delivery rates have increased rapidly, with consequences for subsequent pregnancies. The majority of intrapartum cesarean deliveries are performed for failure to progress, typically in nulliparous women at term. Current uterine registration techniques fail to identify inefficient contractions leading to first-stage labor arrest. An alternative technique, uterine electromyography has been shown to identify inefficient contractions leading to first-stage arrest of labor in nulliparous women with spontaneous onset of labor at term. The objective of t

Cooperation of aquaporin 5 and the adrenergic system in the initiation of birth in rat model.

Aquaporins (AQPs) are involved in the process of implantation, regulate myometrial contractions and cervical ripening, and maintain appropriate placental functioning. The molecular mechanism of these functions is not fully understood. Our study aimed to investigate the physiological significance of AQP5 during pregnancy and to determine the cooperation between the adrenergic system and the AQP5 in uterine contraction in the late-pregnant rat uterus. After administering AQP5 siRNA intraperitoneally to Sprague-Dawley rats, the length of the gestational period was determined and the changes in ut

Measurement of head compression during labor: preliminary results.

To study the pressure to which the fetal head is exposed during the second stage of labor, an instrument was designed to measure continuously the pressure between the presenting part and the birth canal on both sides of the fetal head. Pressure parameters during and between the uterine contractions were calculated from 42 spontaneous vertex deliveries presenting as occiput anterior. During the second stage the overall average of the mean maximum head pressure during uterine contraction was 31.8 +/- 11.0 kPa, that of the mean head pressure measured 13.3 +/- 4.8 kPa during uterine contraction an

Significance of transient receptor potential vanilloid 4 and aquaporin 5 co-expression in the rat uterus at term.

AIMS: Aquaporins (AQPs) are channel proteins that facilitate the rapid passive movement of water. In our studies it was proved that the decreased AQP5 expression is followed by the increase of uterine contractility. The transient receptor potential vanilloid 4 (TRPV4) is a calcium channel, which is activated in response to osmotic changes. Our aim was to determine the possible role of AQP5 in this osmotic regulation of TRPV4, thus in pregnant uterine contraction. MAIN METHODS: We used RT-PCR and Western blot techniques for the detection of the TRPV4 expression during pregnancy in rat uterus.

Interleukin-6 does not stimulate rat myometrial contractions in an in vitro model.

PROBLEM: Interleukin-6 (IL-6) increases in culture-positive amniotic fluid in women with preterm labor. IL-6 stimulates the production of prostaglandins leading to increased uterine activity. METHODS: We tested the hypothesis that IL-6 increases myometrial activity through release of uterotonic mediators. We studied the effect of IL-6 on uterine contractions in the absence and presence of fetal membranes to determine if the effect was on myometrium alone or was mediated through fetal membranes/decidua. IL-6 in concentrations of 100, 10, 0.1 or 0 ng/ml was added to the maternal side of the dua
