There is no direct verification of pineal gland involvement in human uveitis. Specimens of pineal tissue are not available during active uveitis in human patients. Naturally occurring uveitis in horses gives us an opportunity to examine tissues during active ocular inflammation. Forty-seven cases of endogenous uveitis aged 60 and above were analyzed in the retrospective study. The result shows males are more than females (1.2:1). Among the 47 cases, 28 cases (59.57%) are anterior uveitis, 18 cases (38.30%) are panuveitis. Only 1 case (2.13%) is posterior uveitis. Immediated-type uveitis is not found. A 77-year-old woman had complicated cataract surgery with resultant uveitis. Four months after an apparently complete recovery, she was vaccinated with influenza vaccine. Within two weeks, she had a recrudescence of her inflammation accompanied by cystoid macular edema and reduced vision. We discuss the mechanisms of this reaction.

Under certain pathologic circumstances autoantigens lead to the formation of autoantibodies. In uveitis, autoimmunologic phenomena will also be discussed. We studied 149 sera of uveitis patients for various organ specific and non-organ specific autoantibodies. A patient presented with a posterior uveitis. An inferior plaque of retinal exudation was seen. Full investigation failed to establish a cause until six weeks later, when cryptococcal meningitis developed. The patient was immunocompetent. Exudation in relation to retinal vessels is unusual in idiopathic posterior uveitis, and cryptococcosis should be considered in the differential. Diagnosis is by lumbar puncture or vitreous aspiration.

The serum of 63 patients suffering from various clinical types of endogenous uveitis with unknown etiology was investigated for the presence of circulating immune complexes (CIC). None of the patients was suffering from a systemic disease usually associated with CIC. Significantly elevated CIC levels were detected in 2 women with chronic cyclitis. The studies of histocompatibility (HLA) antigens have contributed to the understanding of the pathogenesis of uveitis. Acute anterior uveitis is associated with rheumatic disorders such as ankylosing spondylitis. HLA-B 27 antigen is present in 98% of these cases. A patient with psoriasis and anterior uveitis is described. In her family the two entities of iridocyclitis and psoriasis occurred separately in two generations and appeared associated in the third generation. Tissue typing of the family revealed HL-A 27 antigen in two members and HL-A 17 in the other two members. The possible relevance of HL-A antigens to the two clinical entities is discussed.
