In the early postoperative period after selective proximal vagotomy, there is often a delayed emptying of solid food. We examined the contraction of the antrum by intraluminal manometry in duodenal-ulcer-patients prior to (n = 20), 14 days after (n = 20) and 1--2 years after SPV with pyloroplasty. The motility was tested before and after i.v. injection of 20 mg metoclopramide. Preoperatively metoclopramide effected a significant rise of contraction in the antrum. 14 days after operation the motility was significantly reduced (85,7%--90,7%). Metoclopramide stimulated again the intraluminal pres…

50 unselected cases of bleeding duodenal ulcer were treated by vagotomy and pyloroplasty with suturing of the bleeding vessels. The operative mortality was 8% of which recurrent haemorrhage accounted for 6% of the patients. The operative mortality was related to advanced age, severity of bleed and associated medical illnesses. Vagotomy-pyloroplasty with suture ligation is technically easy to perform for bleeding duodenal ulcer and has a low incidence of recurrent haemorrhage.

In the years between 1967 and 1978, Billroth I-type antrectomy and truncal vagotomy were performed in 44, and primary gastric resection in 19 cases of perforated duodenal ulcer. It was found that in the case of perforated ulcer causing extensive scarring, stenosis or penetration, antrectomy is not more complicated than pyloroplasty. In the absence of other local lesions and mainly in high-risk patients with cirrhosis, diabetes, chronic renal disease, tuberculosis, etc. excision of the perforated duodenal ulcer, pyloroplasty according to Finney and vagotomy were performed (48 operations), while…

In the five-year period 1972 to 1976 the author's preferred treatment for patients with chronic duodenal or prepyloric peptic ulcer requiring surgery was proximal gastric vagotomy. In spite of this preference, only two-thirds of such patients were so treated. Most patients with bleeding and stenosis were treated by bilateral truncal vagotomy and drainage, and a few by Pólya gastrectomy. Proximal gastric vagotomy proved to be a safe elective operation without mortality and with a proven ulcer recurrence rate so far of 6%. Compared with those who had bilateral truncal vagotomy and drainage, the …

Experiments were performed on 4 non-anaesthetized dogs with chronic gastric fistulae. Gastric tonus was studied by volume and inflow rate recording at low pressure heads. Gastric tonus was not affected by propranolol or phentolamine. It was markedly reduced by atropine, presumably by blocking excitatory cholinergic nervous activity. Guanethidine induced a marked increase of gastric tonus, presumably by inhibiting sympathetic modulating activity on intramural cholinergic ganglia. Feeding was accompanied by a marked gastric relaxation which was not blocked by any of the drugs mentioned. Vagotomy…

50 consecutive male patients with a proven duodenal ulcer disease without pyloric stenosis were electively treated with a proximal selective vagotomy. They were randomized in a group with and a group without pyloroplast. Up to now (1-3 years follow-up) no recurrences were found, and only two patients have major complaints (Visich grading 3). There are no differences between the two groups, as judged by the clinical result, the pentagastrin test, the Hollander test (2-DODG stimulation), and the gastrin analysis. Pyloroplasty therefore is not needed. Although the vagotomy which completely preser…
