Dichotomizing sensory nerve fibers innervating both the lumbar vertebral body and the area surrounding the iliac crest: a possible mechanism of referred lateral back pain from lumbar vertebral body.

STUDY DESIGN: Animal study. OBJECTIVE: To determine the existence of dichotomizing sensory nerve fibers innervating both the lumbar vertebral body and the area surrounding the iliac crest (ASIC). SUMMARY OF BACKGROUND DATA: Elderly patients with osteoporosis sometimes experience lumbar vertebral fracture and may feel diffuse nonlocalized pain in the back, the lateral portion of the trunk, and the ASIC. The pattern of sensory innervation of vertebral bodies remains unclear. DRG neurons with dichotomizing axons have been reported and are thought to be related to referred pain. The purpose of t

The relative contribution of discs and vertebral bodies to thoracic kyphosis in healthy volunteers.

INTRODUCTION: Understanding the normal anatomy of thoracic kyphosis (TK) in healthy subjects is essential for evaluating sagittal malalignment and planning the surgery accordingly. The aim of this study was to identify the proportion of thoracic kyphosis originating from disc versus vertebral body shape and to describe its variation according to age and thoracic kyphosis magnitude. METHODS: This study was a retrospective review of a prospective multicenter database of healthy volunteers aged 18 years or older. Vertebral body and disc sagittal Cobb angles were measured at each level and summed

[Vertebral body metastases : From the clarification of back pain to the treatment decision].

Vertebral body metastases represent one of the most frequent manifestations of skeletal tumor dissemination and must be considered in the differential diagnosis of unexplained back pain, particularly in patients with a known history of oncological diseases. The associated morbidity, including spinal instability, deformity, pathological fractures and severe pain, is substantial. Healthcare professionals are regularly confronted with this complex clinical scenario in both outpatient and inpatient settings. A thorough medical history, especially the identification of red flags (e.g., prior trauma

Balloon Kyphoplasty Complication: A Case of a Lodged Balloon Within the Vertebral Body.

This report describes a case involving a balloon kyphoplasty bone tamp becoming lodged inside the vertebral body and unable to be withdrawn, the first report of its kind in the literature. A board certified interventional pain management physician was performing a balloon kyphoplasty for an L3 osteoporotic vertebral compression fracture using a bipedicular approach with two bone tamps. Cannulation and cavity formation were completed without complication; however, upon removal of the balloons it was noted that one had become lodged in the vertebral body. Several attempts were made to remove the

Synovial sarcoma of the spine in the lumbar vertebral body: A rare case report.

RATIONALE: Synovial sarcoma (SS) is a soft tissue neoplasm that rarely occurs in the vertebral body and should be considered in the differential diagnosis in patients with SS and vertebral lesions. SS often presents as a painless mass in the spine, which may undergo slow enlargement, resulting in sustained symptoms of neurologic deficit and pain. Due to the difficulty in differentiating between SS from other soft tissue tumors and metastatic tumors, careful histological confirmation is required for definite diagnosis. Furthermore, due to its malignancy, the appropriate treatment procedure for

Interbody distraction and vertebral body reconstruction with polymethylmethacrylate for the treatment of pathological fractures.

OBJECTIVE Treatment of epidural spinal cord compression (ESCC) caused by tumor includes surgical decompression and stabilization followed by postoperative radiation. In the case of severe axial loading impairment, anterior column reconstruction is indicated. The authors describe the use of interbody distraction to restore vertebral body height and correct kyphotic angulation prior to reconstruction with polymethylmethacrylate (PMMA), and report the long-term durability of such reconstruction. METHODS A single institution, prospective series of patients with ESCC undergoing single-stage decompr

The extreme lateral approach for treatment of thoracic and lumbar vertebral body metastases.

In addition to radiotherapy, the utility of surgical decompression and stabilization in patients with metastatic vertebral body tumors causing cord compression, progressive deformity and/or intractable pain has been well demonstrated. Minimally invasive approaches are an attractive alternative to traditional procedures as they may reduce the degree of disruption of normal anatomy, decrease blood loss, shorten hospital stays and reduce the risk of infection or wound dehiscence. The extreme lateral approach is a procedure that provides access to the anterior spine through a small incision along

Dorsal corporectomy in a Ewing sarcoma situated in the pedicle of the L5 vertebral body.

The clinical case of a 34-year-old patient is presented who was diagnosed with Ewing sarcoma in the right pedicle of the L5 vertebral body on the basis of persisting dorsolumbar pain. Staging examinations including CT scan of the thorax and abdomen as well as bone scintigraphy were inconspicuous. The patient underwent 6 cycles of neoadjuvant chemotherapy (VIDE, similar to Euro Ewing 99). Presurgically, embolisation of the L4 and L5 vertebral bodies was performed on both sides. The tumour was removed via a dorsal approach with corporectomy of L5 and resection of the right nerve root of L5. The

Radiofrequency Heat Ablation and Vertebroplasty in the treatment of neoplastic vertebral body fractures.

BACKGROUND: Metastatic cancer is the most common malignant disease of the skeletal system. Traditionally, conventional fractionated external beam radiotherapy has been the treatment of choice. Recently, minimally invasive surgical techniques (MISS) have been added to the therapeutic armamentarium. The purpose of our study was to assess the effectiveness and safety of Radiofrequency Heat Ablation and Vertebroplasty in the treatment of neoplastic Vertebral Compressive Fractures (VCF). The aim of radiofrequency heat ablation is to destroy the tumor tissue before stabilizing the vertebra through t

[Vertebroplasty: a percutaneous procedure for the treatment of diseased vertebral bodies of various etiologies].

Percutaneous vertebroplasty is a radiologically guided invasive technique consisting of the injection of surgical cement into the diseased vertebral body. The procedure results in immediate pain relief and strengthening of the bone due to the polymerization process of the filling material hardening the vertebral body and preventing further collapse. This method is suitable for the treatment of osteoporotic vertebral fractures and of osteolytic vertebral body metastases without neurological signs, in multiple appearance as well. Authors present technical details of the procedure performed by bi

Treatment of osteoporotic late collapse of a vertebral body of thoracic and lumbar spine.

Although osteoporotic late collapse of a vertebral body is a common infirmity of old age, it has not been well studied, and no consensus regarding treatment of this condition has been reached. Forty-five patients with osteoporotic late collapse of a vertebral body were classified into six types based on the appearance on the lateral projection of a radiograph and the presence or absence of neurologic symptoms and were evaluated on imaging and clinical outcome. We concluded that the treatment of osteoporotic late collapse of a vertebral body can be individualized based on several factors such a

Anterior extension of the thoracic vertebral bodies in Scheuermann's kyphosis. An anatomic study.

Changes consistent with Scheuermann's kyphosis were noted in 103 specimens (7.4%) of a sample of 1,384 thoracic spines in the Hamann-Todd collection of human skeletons. In 94% of the affected vertebrae, a distinct anterior elongation of the vertebral centrum was present. This anterior extension was composed of mature cancellous bone and was morphologically and roentgenographically different from marginal osteophyte formation. It was not present in any vertebrae of a control group of 50 unaffected spines. Associated findings included vertebral wedging and Schmorl's nodes.

Non-neoplastic lesions of vertebral bodies: findings in magnetic resonance imaging.

This paper presents examples of non-neoplastic lesions of the spine that produce focal or diffuse signal alterations on MR images, and that may, therefore, be confused with metastases or primary neoplasms. Examples include endplate changes associated with degenerative disk disease, hemispherical spondylosclerosis, osteoporotic compression fractures, Paget's disease, focal and diffuse fatty infiltration, osteomyelitis, and changes associated with various arthritides. Distinguishing signal intensity characteristics on T1 and T2 weighted images and distinctive morphology are emphasized.

[Radical surgery and vertebral body replacement in aneurysmal bone cysts of the spine].

6 cases of aneurysmal bone cyst of the spine are presented. The clinical picture, diagnostic procedures and operative treatment will be discussed. Although aneurysmal bone cyst is a benign, non-neoplastic lesion its spreading tumor growth with destruction of important bony structures of the vertebral body, consecutive instability and cord compression presents a considerable menace to the mostly teenage-patient. Spinal computerized tomography is the diagnostic method of choice. The goal of surgical therapy of the aneurysmal bone cyst of the spine should include radical tumor excision in order t

Vertebral balloon kyphoplasty versus vertebral body stenting in non-osteoporotic vertebral compression fractures at the thoracolumbar junction: a comparative radiological study and finite element analysis (BONEXP study).

OBJECTIVE: To compare radiologically balloon kyphoplasty (BKP) and vertebral compression fracture (VCF) expansion and corroborate with a finite element (FE) analysis. The principle of BKP is to stabilize VCF by restoring vertebral body anatomy using bone expansion and cement filling. More recently, vertebral body stenting (VBS) has been developed to reduce the loss of vertebral height observed after balloon deflation. METHODS: A retrospective, monocentric and continuous study of 60 non-osteoporotic fractures of the thoracolumbar junction treated by vertebral bone expansion was carried out ove

Evolutionary Specializations of the Human Vertebral Body and Intervertebral Disc in Relation to Bipedalism.

It is widely accepted that modern humans display distinctive vertebral and intervertebral disc (IVD) morphologies that evolved to meet the biomechanical demands of habitual terrestrial bipedalism. This study synthesizes macro- and microstructural differences in the lumbar spine to clarify how human specializations compare with those of extant apes. The skeletal sample consisted of 240 humans, 20 chimpanzees, and 25 gorillas. The CT scan sample comprised 180 humans and eight chimpanzees. Histological analysis of the IVD was performed on 10 humans and four ape specimens. Vertebral bodies and dis
