Early Manifestation of Benign Paroxysmal Positional Vertigo: A Case Report.

INTRODUCTION: Benign paroxysmal positional vertigo (BPPV) is an inner ear disorder with a heterogeneous etiology, often linked to preceding infections, head injuries, or vestibular neuronitis. While it is commonly observed in the elderly, its occurrence in the pediatric population is rare. To our knowledge, there have been no reported cases of BPPV in patients younger than 5 years. CASE PRESENTATION: A 4.5-month-old female infant was admitted with episodes of paroxysmal nystagmus. Parents reported fast horizontal eye movements lasting up to 30 s, with one episode accompanied by vomiting. Comp

Vertical vestibular input to and projections from the caudal parts of the vestibular nuclei of the decerebrate cat.

1. To investigate the type of vestibular signals that neurons in the caudal parts of the vestibular nuclei transmit to the cerebellum and spinal cord, we studied their responses to natural vestibular stimulation in vertical planes in decerebrate cats with the caudal cerebellum removed. Most neurons were in the caudal half of the descending vestibular nucleus, the remainder at corresponding levels of the medial nucleus or the medial-descending border. 2. Dynamics of the responses of spontaneously firing neurons were studied with sinusoidal tilts delivered at 0.05-1 Hz near the plane of body rot

Long-Lasting Visuo-Vestibular Mismatch in Freely-Behaving Mice Reduces the Vestibulo-Ocular Reflex and Leads to Neural Changes in the Direct Vestibular Pathway.

Calibration of the vestibulo-ocular reflex (VOR) depends on the presence of visual feedback. However, the cellular mechanisms associated with VOR modifications at the level of the brainstem remain largely unknown. A new protocol was designed to expose freely behaving mice to a visuo-vestibular mismatch during a 2-week period. This protocol induced a 50% reduction of the VOR. In vivo pharmacological experiments demonstrated that the VOR reduction depends on changes located outside the flocculus/paraflocculus complex. The cellular mechanisms associated with the VOR reduction were then studied in

Synaptic mechanisms of interaction between Deiters' nucleus and the nuclei of some cranial nerves.

The effects of stimulation of the vestibular nerve, spinal trigeminal nucleus, facial and hypoglossal nuclei of the cranial nerves on the neuronal activity in the lateral vestibular nucleus of Deiters were studied in cats anaesthetized with pentobarbitone. Stimulation of these nuclei was found to produce antidromic and synaptic activation of Deiters' neurons. Descending axon collaterals of the vestibular neurons to these brainstem structures were revealed. Stimulation of the VIIIth nerve, spinal trigeminal and facial nuclei evoked mono- and polysynaptic excitatory postsynaptic potentials in De

Effects of visceral inputs on the processing of labyrinthine signals by the inferior and caudal medial vestibular nuclei: ramifications for the production of motion sickness.

Neurons located in the caudal aspect of the vestibular nucleus complex have been shown to receive visceral inputs and project to brainstem regions that participate in generating emesis, such as nucleus tractus solitarius and the "vomiting region" in the lateral tegmental field (LTF). Consequently, it has been hypothesized that neurons in the caudal vestibular nuclei participate in triggering motion sickness and that visceral inputs to the vestibular nucleus complex can affect motion sickness susceptibility. To obtain supporting evidence for this hypothesis, we determined the effects of intraga

COVID-19 reduces immune competence and precipitates superinfections.

OBJECTIVES: Whether infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) can be complicated by immune suppression is under debate, but the following case suggests decreased immune competence during and after a SARS-CoV-2 infection. CASE REPORT: The patient is a 50-year-old woman with a previous history of transient hyperthyroidism, allergy against ambrosia, and burn-out syndrome, who experienced a mild infection with SARS-CoV-2 during which she developed candida pharyngitis, which was successfully treated with miconazole. Twenty-eight days after clinical recovery from t

Selective genetic targeting of the mouse efferent vestibular nucleus identifies monosynaptic inputs and indicates function as multimodal integrator.

The vestibular system is a critical sensory modality required for coordinated movement, balance, and our ability to interact with the surrounding environment. Vestibular sensory neurons provide the nervous system with information about head rotation and acceleration. However, the nervous system can also modify the activity of sensory neurons and hair cells via the actions of the efferent vestibular system (EVS). The function of the EVS has remained unknown partly because of an inability to target efferent vestibular neurons in a selective manner to understand their synaptic inputs and function

[Vestibular compensation. Review of the literature and clinical applications].

Vestibular compensation is an excellent model for the study of plasticity of the adult central nervous system. Therefore it has been the subject of several studies in humans and animals, which will be briefly summed up by the authors. Lesions of the labyrinth or vestibular neurectomy are immediately followed of postural and oculomotor disorders, as well as by dynamic deficits of the various vestibular reflexes (vestibulo-ocular and vestibulonucal reflexes). While the former problems always recede in all species, the restoration of the dynamic properties of vestibular reflexes largely depends u

Suppressive control of optokinetic and vestibular nystagmus by the primate frontal eye field.

When the eyes are fixated on a spot, fixation neurons in the frontal eye field (FEF) show an increase in activity. Our previous study suggested that fixation neurons in the FEF contribute to the suppression of saccades and smooth pursuit eye movements to maintain active fixation. The present study examined the role of the FEF in the suppressive control of reflexive eye movements, optokinetic and vestibular nystagmus, in trained monkeys. Electrical stimulation in the FEF suppressed the quick and slow phases of optokinetic and vestibular nystagmus at an intensity lower than the threshold for eli

Regulatory Networks Driving the Specification, Differentiation, and Diversification of Neurons in the Mouse Inner Ear.

Vestibular and spiral ganglion neurons (VGNs and SGNs) developed in the inner ear, where they extend fibers to innervate the vestibular and cochlear hair cells and project centrally to the vestibular and cochlear nuclei. This review focuses on representative molecular factors that regulate key processes in the development of inner ear neurons, including their specification, differentiation, axon targeting, and functional diversification. A temporal regulatory cascade defines the initial precursors through factors such as Smarca4, Six1, Eya1, followed by Sox2. While Sox2 deletion abolishes hair

Vestibulocochlear toxicity in a pair of siblings 15 years apart secondary to aspartame: two case reports.

INTRODUCTION: Aspartame may have idiosyncratic toxic effects for some people; however, there are few case reports published in the medical literature. We present two case reports in a pair of siblings, one with a vestibular and the other with a cochlear toxicity to aspartame. The cochlear toxicity is the first case to be reported, while the vestibular toxicity is the second case to be reported. CASE PRESENTATION: A 29-year-old white female had a 20-month history of nausea and headache, progressively getting worse with time and eventually to also involve vomiting, vertigo, and ataxia. She was

The role of acute and latent virus infections in the pathogenesis of inner ear disturbances.

The possible role of herpesviral infections of the inner ear in suddenly appearing inner ear disturbances was investigated. Experimental pseudorabies virus (PRV, Herpes sui 1) infection of mice and swine was used as a model system. Infected mice represented the productive cycle of PRV infection (acute phase), whereas the latent phase of infection could be tested in swine. From the acutely infected mice the virus could be reisolated from perilymphatic fluid and various parts of the brain. Massive histopathologic alterations and signs of total cell damage to the organ of Corti and the vestibular

Role of the flocculus in the development of vestibular compensation: immunohistochemical studies with retrograde tracing and flocculectomy using Fos expression as a marker in the rat brainstem.

After unilateral labyrinthectomy in rats, Fos-like immunoreactive neurons appeared in the ipsilateral medial vestibular nucleus, contralateral prepositus hypoglossal nucleus and contralateral inferior olive beta subnucleus. and thereafter gradually disappeared in accordance with the development of vestibular compensation. This finding indicated that the activation of these nuclei is the initial event of vestibular compensation. In the present study, retrograde tracing experiments revealed that these Fos-like immunoreactive neurons project a proportion of their axons to the vestibulocerebellum

The origin of efferent labyrinthine fibres: a comparative study in vertebrates.

Parent cells of efferent acoustic and efferent vestibular fibers were determined anatomically in all classes of vertebrates by use of retrograde axonal transport of horseradish peroxidase. These neurons were found in the brainstem, in particular in the reticular formation. In the goldfish, efferent labyrinthine neurons could be demonstrated in a medial position (lateral to the fasciculus longitudinalis medialis). In the frog, efferent neurons appeared more lateral, dorsomedial to the facial motor nucleus. In reptiles and birds, efferent acoustic neurons separate from efferent vestibular neuron

Herpes simplex polyganglionitis.

Evidence suggests that many cranial nerve syndromes, such as migraine headache, acute vestibular neuronitis, globus hystericus, carotidynia, acute facial paralysis (Bell's palsy), and Meniere's disease, are caused by the neurotropic herpes simplex virus (HSV). Because transitory cranial nerve dysfunction during acute HSV infection can be asymptomatic but often occurs in conjunction with mucocutaneous vesicles, we tested five subjects with herpes labialis for cranial nerve dysfunction. Four of the subjects had hypesthesia of the trigeminal nerve (which recurred in two); four, hypesthesia of the

Projection neurons of the vestibulo-sympathetic reflex pathway.

Changes in head position and posture are detected by the vestibular system and are normally followed by rapid modifications in blood pressure. These compensatory adjustments, which allow humans to stand up without fainting, are mediated by integration of vestibular system pathways with blood pressure control centers in the ventrolateral medulla. Orthostatic hypotension can reflect altered activity of this neural circuitry. Vestibular sensory input to the vestibulo-sympathetic pathway terminates on cells in the vestibular nuclear complex, which in turn project to brainstem sites involved in the
