Acute Inner Ear Complications of Stapes Surgery: Value of Delayed Postcontrast 3D-FLAIR MRI Sequences.

OBJECTIVE: To describe and assess the usefulness of delayed postcontrast three-dimensional fluid-attenuated inversion recovery (3D-FLAIR) sequences on 3-Tesla (3 T) magnetic resonance imaging (MRI) in patients presenting with acute inner ear complications after stapes surgery. STUDY DESIGN: Case series. SETTING: French tertiary referral center. METHODS: The clinical records and imaging of patients who underwent delayed postcontrast 3D-FLAIR MRI sequences for labyrinthine complications after stapes surgery, performed between January 2019 and April 2023, were retrospectively reviewed. Quantification of endolymphatic hydrops and its correlation with Meniere's disease clinical features.

OBJECTIVE: We aimed to quantitatively evaluate the degree of endolymphatic hydrops and its correlation with the clinical features of Meniere's disease. METHODS: We retrospectively collected data from patients with Meniere's disease who underwent gadolinium-enhanced magnetic resonance imaging (MRI) at our department from January 2018 to December 2019. Mimics software was used to perform three-dimensional modelling of the labyrinth, and volume information was obtained to calculate the endolymphatic hydrops index (EHI). A correlation analysis was conducted with data from pure tone audiometry, el

The effect of noise exposure in the presence of canal fenestration on the amplitude of short-latency vestibular evoked potentials.

BACKGROUND: Exposure to high-intensity noise causes little, if any, reduction in vestibular function in normal animals as shown by short-latency vestibular evoked potentials (VsEPs). OBJECTIVE: To investigate the effect of noise exposure on VsEPs following fenestration of the horizontal semicircular canal. DESIGN AND METHODS: Psammomys obesus (fat sand rat) underwent labyrinthectomy in 1 ear, while the lateral semicircular canal in the other ear was fenestrated. Control VsEPs to linear acceleration (approximately 3g; rise time, approximately 1-2 milliseconds) were recorded immediately after

Vestibular pathophysiologic changes in experimental perilymphatic fistula.

An animal model of experimental perilymphatic fistula (EPLF) was developed in the guinea pig in order to study vestibular pathophysiology. In experimental animals, 4 microL of perilymph was suctioned from one cochlea via the round window membrane. Changes in vestibular function were as follows. 1) During the acute stage (5 hours postoperatively), spontaneous nystagmus directed toward the normal side was noted in 57.4% of the EPLF animals. This lasted less than 24 hours. 2) One week postoperatively, direction-fixed positional nystagmus toward the lesioned ear was present in 22.7% of the EPLF an

Diagnosis and management of vertigo.

Patients with vertigo may be best evaluated when the physician has the various entities that may be responsible clearly in mind. Those diseases listed in group II are more familiar to the primary care physician than those listed in group I, which belong more in the realm of neuro-otology than in general medicine and neurology. The proper use of history, physical findings, and laboratory evaluations will allow the clinician to classify the patient's problem and to initiate treatment and consider appropriate referral when indicated.

Inner ear pathology associated with Reye's syndrome.

Severe pathological changes were observed in the inner ear tissues of a 2-month-old patient who died of Reye's syndrome after 5 days of hospitalization. In the organ of Corti, the inner hair cells appeared to be more severely damaged than the outer hair cells. Various degrees of degeneration were observed in all non-sensory epithelial cells lining the cochlear duct. In most turns of the cochlear duct, Reissner's membrane was ruptured and/or collapsed onto the organ of Corti. Likewise, both sensory and non-sensory cells of the vestibular end organs were markedly degenerated. These observations

[Auditory and vestibular lesions in Behcet's disease].

Auditory and vestibular function tests were conducted in 16 patients with Behcet's disease. Vestibular lesions included spontaneous nystagmus in 5 cases, and a bilateral vestibular syndrome after provoked tests in 13 cases (7 of the deficiency type and 6 irritative in nature). Findings after auditory tests were bilateral symmetric deafness in 2 cases; one of the perception type and the other showing mixed features. Various etiopathogenic theories are discussed. The possibility of a bilateral central lesion of the VIIIth nerve was also considered, 8 patients in the series having cerebral manife

[How useful is the intersaccadic interval analysis of vestibular nystagmus for clinical diagnostic (author's transl)].

The intersaccadic interval analysis of vestibular nystagmus was proposed by Cheng et al. in 1974. This method gives a statistical and sequential description of intersaccadic intervals. Until now there were no reports concerning the clinical diagnostic relevance of this method in vestibular disturbancies. Therefore, we checked 8 normal probands and 7 patients (3 peripheral, 3 central vestibular lesions, one congenital nystagmus). Compared to common nystagmus parameters like maximum speed of slow phase or minimum frequency normal probands showed a relatively high variability, as well as the pati

Progressive cochleo-vestibular labyrinthitis.

OBJECTIVE: We report a rare case of sudden deafness and benign paroxysmal positional vertigo (BPPV) corresponding to a canalithiasis of the ipsilateral left posterior semicircular canal (PSC). CASE REPORT: The initial examination showed intact lateral semicircular canal (LSC) function. Cerebral MRI results were normal. However evoked otoacoustics emissions were absent in the left ear. The patient developed, in the course of one week, symptoms and signs of an acute unilateral vestibulopathy in the same ear. However there were no change in hearing loss after it. CONCLUSION: The association of

[Weather status and incidence of idiopathic facial nerve paralyses, vestibular disorders, Menière's attacks and sudden deafness].

The authors studied a possible relationship between the weather and Bell's palsy, acute unilateral vestibular disorder, Menière's disease and sudden deafness. These diseases definitely do occur more often at times of low pressure. This is most convincing when a stable high pressure zone is followed by a strong low, thus creating comparatively great pressure differences. Bell's palsy, sudden deafness and Menière's disease occur more often under the influence of the warm side of the low. Such atmospheric conditions could not be confirmed for acute unilateral vestibular disorder.

[Stapedovestibular otospongiosis beginning in the cochlea. Apropos of 3 cases].

Stapedovestibular ankylosis developed after a prolonged course of pure perception deafness in 3 patients, suggesting possible secondary fixation of the stapes from otospongiosis restricted to the cochlear up to that time. A precise diagnosis of pure cochlear otospongiosis is always difficult to make, its presence usually being supposed rather than confirmed in spite of clinical, audiometry and radiology data, except in those few cases where secondary ankylosis of the foot of the stapes occurs. These findings suggested the need for regular audiometric surveillance of all patients with perceptio

Blood glucose and insulin levels, thyroid function, and serology in Ménière's disease, recurrent vestibulopathy, and psychogenic vertigo.

Nineteen patients with unilateral Ménière's disease, 20 with psychogenic dizziness, and 20 with recurrent vestibulopathy (diagnostic criteria in text of paper) were found to have normal five hour glucose tolerance tests, serum thyroxine and effective thyroid indices, and serologic tests for syphilis. Hypothyroidism and hypoglycemia were absent in all groups. An unexplained finding of each diagnostic group was significant increase of fasting blood glucose and insulin levels, and elevated insulin:glucose ratios, compared to a control group. There appears to be no diagnostic indication for perfor

Early catarrhine evolution in the light of semicircular canal and vestibule shape.

The phylogenetic relationships of early catarrhines (Old World monkeys and apes) are contentious, particularly regarding pliopithecoids, which are considered a clade of Miocene stem catarrhines more derived than most of their Oligocene counterparts. However, their debated affinities and the uncertain phylogenetic relationships of the Oligocene catarrhine Saadanius leave their systematic position unresolved. Notably, Saadanius displays a mosaic morphology, coupling primitive craniodental traits with a fully ossified ectotympanic (as opposed to the annular/incompletely ossified ear tube of stem

Update on Clinical Physiology and Pathomechanisms for Vestibulo-Autonomic Interplay.

The vestibulo-autonomic interaction refers to the neural interplay between the vestibular and autonomic systems. In particular, the vestibular system plays an active role in adjusting blood distribution during movement and changes in posture, thereby complementing the baroreflex. This review summarizes recent clinical evidence highlighting the interaction between the vestibular and autonomic systems, including altered vestibular-evoked myogenic potentials in patients with postural orthostatic tachycardia syndrome, orthostatic hypotension, and neurodegenerative disorders. Additionally, the revi

Enhanced vestibular-evoked balance responses in adolescents with idiopathic scoliosis.

Scoliosis is a spinal deformity characterized by lateral deviation and rotation of the vertebrae, affecting between 0.47% and 5.2% of adolescents. A proposed explanation for adolescent idiopathic scoliosis (AIS) is asymmetrical vestibular function affecting the descending control of torso muscles. To verify this hypothesis, we applied stochastic vestibular stimulation (SVS) at frequencies of up to 25 Hz, both monaurally and binaurally, to standing participants with AIS (n = 15) and to those without AIS (n = 15). We characterized potential left-right vestibulomotor asymmetry in the correlations

Harvest of Vestibular End-Organs under Physiologic Conditions during Labyrinthectomy.

The living human inner ear is challenging to study because it is encased within dense otic capsule bone that limits access to biological tissue. Traditional temporal bone histopathology methods rely on lengthy, expensive decalcification protocols that take 9-10 months and reduce the types of tissue analysis possible due to RNA degradation. There is a critical need to develop methods to access fresh human inner ear tissue to better understand otologic diseases, such as Ménière's disease, at the cellular and molecular level. This paper describes a technique for the harvest of human vestibular en
