Mortality rates among adult critical care patients with unusual or extreme values of vital signs and other physiological parameters: a retrospective study.

BACKGROUND: We evaluated relationships of vital signs and laboratory-tested physiological parameters with in-hospital mortality, focusing on values that are unusual or extreme even in critical care settings. METHODS: We retrospectively studied Philips Healthcare-MIT eICU data (207 U.S. hospitals, 20142015), including 166,959 adult-patient critical care admissions. Analyzing most-deranged (worst) value measured in the first admission day, we investigated vital signs (body temperature, heart rate, mean arterial pressure, and respiratory rate) as well as albumin, bilirubin, blood pH via arterial…

Trends in vital signs and routine biomarkers in patients with sepsis during resuscitation in the emergency department: a prospective observational pilot study.

OBJECTIVES: Sepsis lacks a reliable and readily available measure of disease activity. Thereby, it remains unclear how to monitor response to treatment. Research on numerous (new) biomarkers associated with sepsis provided disappointing results and little is known about changes in vital signs during sepsis resuscitation. We hypothesised that trends in vital signs together with routine biomarker levels during resuscitation might provide information about the response to treatment at a very early stage of sepsis in the emergency department (ED). We therefore explore trends in vital signs and rou…

Association of Vital Sign and Laboratory Abnormalities Detected via Remote Monitoring with ICU Readmission and Mortality: An Observational Cohort Study.

BACKGROUND: Post-intensive care unit (ICU) patients remain at high risk for adverse outcomes, and early identification of instability may improve outcomes and reduce costs. This study measures the association between eight vital sign and laboratory abnormalities ("flags") on surgical wards and the odds of ICU readmission or death in surgical patients after ICU transfer. Flags captured intuitive indications for ICU admission, including tachycardia, hypotension, hypoxemia, hyperkalemia, hyperglycemia, hypoglycemia, and anemia, which may be missed with infrequent ward evaluation. METHODS: This s

Using Life-Saving Interventions to Determine Optimal Vital Sign Ranges among Adults Encountered by Emergency Medical Services.

BACKGROUND: Vital signs are an essential component of the prehospital assessment of patients encountered in an emergency response system and during mass-casualty disaster events. Limited data exist to define meaningful vital sign ranges to predict need for advanced care. STUDY OBJECTIVES: The aim of this study was to identify vital sign ranges that were maximally predictive of requiring a life-saving intervention (LSI) among adults cared for by Emergency Medical Services (EMS). METHODS: A retrospective study of adult prehospital encounters that resulted in hospital transport by an Advanced L…

Prearrest vital sign abnormalities are associated with adverse outcomes in pediatric ICU cardiac arrest: a get with the guidelines-resuscitation analysis.

AIM: We aimed to quantitatively describe vital sign abnormalities prior to pediatric IHCA and evaluate whether the severity of abnormalities was independently associated with survival. METHODS: In a retrospective cohort study using the American Heart Association's Get with The Guidelines-Resuscitation® registry, children with ≥1 min of cardiopulmonary resuscitation (CPR) in an Intensive Care Unit (ICU) from 2007 to 2022 with prearrest vital signs were included. Vital signs most proximate to CPR (10-120 min prior) were classified as abnormal (HR or RR >95th, SBP or DBP <5th percentile for age)…

Nmr-VSM: Non-Touch Motion-Robust Vital Sign Monitoring via UWB Radar Based on Deep Learning.

In recent years, biometric radar has gained increasing attention in the field of non-touch vital sign monitoring due to its high accuracy and strong ability to detect fine-grained movements. However, most current research on biometric radar can only achieve heart rate or respiration rate monitoring in static environments, which have strict monitoring requirements and single monitoring parameters. Moreover, most studies have not applied the collected data despite their significant potential for applications. In this paper, we introduce a non-touch motion-robust vital sign monitoring system via

Clinical and vital sign changes associated with late-onset sepsis in very low birth weight infants at 3 NICUs.

BACKGROUND: In premature infants, clinical changes frequently occur due to sepsis or non-infectious conditions, and distinguishing between these is challenging. Baseline risk factors, vital signs, and clinical signs guide decisions to culture and start antibiotics. We sought to compare heart rate (HR) and oxygenation (SpO2) patterns as well as baseline variables and clinical signs prompting sepsis work-ups ultimately determined to be late-onset sepsis (LOS) and sepsis ruled out (SRO). METHODS: At three NICUs, we reviewed records of very low birth weight (VLBW) infants around their first sepsi…

Patient Adherence to Scheduled Vital Sign Measurements During Home Telemonitoring: Analysis of the Intervention Arm in a Before and After Trial.

BACKGROUND: In a home telemonitoring trial, patient adherence with scheduled vital signs measurements is an important aspect that has not been thoroughly studied and for which data in the literature are limited. Levels of adherence have been reported as varying from approximately 40% to 90%, and in most cases, the adherence rate usually dropped off steadily over time. This drop is more evident in the first few weeks or months after the start. Higher adherence rates have been reported for simple types of monitoring and for shorter periods of intervention. If patients do not follow the intended …

The sixth vital sign: what reproduction tells us about overall health. Proceedings from a NICHD/CDC workshop.

STUDY QUESTION: Does the fertility status of an individual act as a biomarker for their future health? SUMMARY ANSWER: Data support an association between reproductive health and overall health for men and women. WHAT IS ALREADY KNOWN: Various chronic conditions, such as diabetes, obesity and cancer, can compromise fertility, but there are limited data for the converse situation, in which fertility status can influence or act as a marker for future health. Data reveal an association between infertility and incident cardiovascular disease and cancer in both men and women. STUDY DESIGN SIZE A

Radar-Based Monitoring: A Proof of Principle Study in a Piglet Model for a Novel Approach in Non-Contact Vital Sign Monitoring.

(1) Background: Hospitalized preterm infants often require months of vital signs monitoring in the neonatal intensive care unit. Today, wired sensors are essential for survival, but are associated with numerous disadvantages including sensor dislocations, skin trauma and hygiene risks. Non-contact vital sign monitoring would therefore represent a significant improvement in the care of hospitalized neonates. (2) Objective: This study aims to lay the foundation for non-contact radar-based monitoring of the respiratory rate, which could be used in the neonatal intensive care unit. (3) Methods: We…

From vital sign trajectories to data-driven targets: defining exploratory blood gas ranges in sepsis-associated thrombocytopenia.

BACKGROUND: The early in-intensive care unit (ICU) phase is critical for sepsis-associated thrombocytopenia (SATP) patients, yet the prognostic value of their initial physiological trajectory remains underexplored. We aimed to identify distinct subgroups based on vital sign trajectories following ICU admission and to investigate their differential outcomes and subsequent blood gas management needs. METHODS: This retrospective study utilized the MIMIC-IV database. Adults with SATP were included. Group-based multi-trajectory modeling (GBMTM) was applied to hourly vital signs (including heart ra…

Prognostic Value of Traditional and Optimised Vital Sign Thresholds for Risk of Serious Adverse Events in Continuously Monitored Hospitalised Patients.

Technological advances enable continuous vital sign monitoring with wireless biosensors in general wards. However, traditional thresholds used in routine monitoring do not effectively identify serious adverse events (SAEs). This study aimed to assess the prognostic ability of combinations of threshold severity and duration in continuous vital sign monitoring for SAEs in acute medical and surgical patients. Data on heart rate (HR), respiratory rate (RR), and peripheral oxygen saturation (SpO2) were collected from four prospective observational studies and two randomised controlled trials. The p

The Measurement of Vital Signs in Pediatric Patients by Lifelight Software in Comparison to the Standard of Care: Protocol for the VISION-Junior Observational Study.

BACKGROUND: Measuring vital signs (VS) is important in potentially unwell children, as a change in VS may indicate a more serious infection than is clinically apparent or herald clinical deterioration. However, currently available methods are not suitable for regular measurement of VS in the home or community setting, and adherence can be poor. The COVID-19 pandemic highlighted a need for the contactless measurement of VS by nonclinical personnel, reinforced by the growing use of telemedicine. The Lifelight app is being developed as a medical device for the contactless measurement of VS using …

Wearable vital-sign sensors for early detection of severe dengue and sepsis: Clinical applications and implementation in resource-limited settings.

INTRODUCTION: Severe dengue and sepsis account for tens of millions of cases and deaths annually in low-resource settings, where spot-check vital sign monitoring misses early physiological deterioration. Affordable wearable sensors that stream continuous data and feed machine learning algorithms promise earlier detection and improved outcomes. METHODS: We conducted a structured narrative literature review of human studies on PubMed, Embase, Scopus, and Google Scholar that evaluated wearable vital-sign devices for predicting severe dengue or sepsis in resource-limited areas.
