Interviewing people living with cognitive impairment: a conversation analysis of supportive interactional practices.

Effective qualitative interviews with individuals with cognitive impairment require careful conversational strategies to maintain dignity, support engagement, and enable meaningful narrative co-construction. Despite recognition of these challenges, practical guidance on interviewer talk in dementia research remains limited. This study applies conversation analysis (CA) to examine interviewer practices used in interviews with individuals with mild cognitive impairment or Alzheimer's disease. Analysis of 17 interviews identified five recurring practices: (1) scaffolding memory retrieval through

Disseminated mucocutaneous leishmaniasis in a patient with advanced HIV.

We report a 43-year-old male farm worker from El Salvador residing in California's Central Valley who presented with 4 months of progressive upper lip swelling, ulcerative lesions on the nose and left forearm, weight loss, night sweats, headaches, and hoarseness of voice. He had severe HIV-related immunosuppression (CD4 count of 81 cells/mm3). Biopsy of a forearm lesion and endobronchial tissue obtained at bronchoscopy revealed intracellular amastigotes consistent with leishmaniasis. Leishmania PCR on endobronchial tissue detected Leishmania donovani/infantum/chagasi complex, and plasma microb

Early diagnosis and critical management of wound botulism in the emergency department: a single center experience and literature review.

BACKGROUND: Clostridium botulinum remains a major threat to a select population of subcutaneous and intramuscular drug users. We conducted a retrospective study of patients who were diagnosed with wound botulism and their clinical presentations to the Emergency Department (ED). RESULTS: A total of 21 patients met the inclusion criteria and all had a confirmed history of heroin use disorder. Initial presentation to the ED included generalized weakness (n = 20, 95%), difficulty swallowing (n = 15, 71%), and speech/voice problems (n = 14, 79%). Sixteen patients (76%) also presented with visible

Laryngeal reconstruction following shrapnel injury in a British soldier: case report.

INTRODUCTION: Laryngeal injuries are uncommon but result in high mortality and morbidity rates when they do occur. We report a case of laryngeal fracture due to penetrating shrapnel injury, repaired with miniplates. CASE REPORT: A 26-year-old soldier was involved in an explosion and sustained a shrapnel wound to his right neck. After immediate airway management at the field hospital he was transferred to the UK, where he underwent a neck exploration, laryngofissure and repair of the thyroid cartilage using miniplates. An endolaryngeal stent was placed, which was removed at a second operation

Prediction of cognitive impairment through speech data analysis: A comparative evaluation of deep learning models.

BACKGROUND: The early detection of cognitive impairments, such as mild cognitive impairment (MCI) and Alzheimer's disease (AD), is essential for timely intervention and management. This study evaluates the performance of various deep-learning models in classifying speech recordings from individuals with normal cognition (NC), MCI, and AD, to identify the most effective approach for audio-based cognitive impairment diagnosis. METHODS: Speech data were obtained from the AI Hub "Cognitive Impairment Diagnosis Voice/Conversation" dataset. The study analyzed voice recordings from 320 female partic

A Study of Personal Recognition Method Based on EMG Signal.

With the increasing development of internet, the security of personal information becomes more and more important. Thus, variety of personal recognition methods have been introduced to ensure persons' information security. Traditional recognition methods such as Personal Identification Number (PIN), or Identification tag (ID) are vulnerable to hackers. Then the biometric technology, which uses the unique physiological characteristics of human body to identify user information has been proposed. But the biometrics widely used at present such as human face, fingerprint, iris, and voice can also

Eliminating Equity: A Culture-Centered Reading of the Attack on Health Equity in Project 2025's Mandate for Leadership.

Project 2025's Mandate for Leadership proposes a sweeping reorganization of the United States Department of Health and Human Services (HHS). This article reads the Mandate's HHS chapter through the culture-centered approach (CCA), joining its account of structure, culture, agency, voice, and communicative inequality to a critical discourse analysis of the chapter's mission statement, its five governing goals, and its data provisions. The chapter does more than oppose equity policies. It reorganizes the conceptual and infrastructural conditions under which health inequity can be named, measured

Schizophrenia as failure of left hemispheric dominance for the phonological component of language.

BACKGROUND: T. J. Crow suggested that the genetic variance associated with the evolution in Homo sapiens of hemispheric dominance for language carries with it the hazard of the symptoms of schizophrenia. Individuals lacking the typical left hemisphere advantage for language, in particular for phonological components, would be at increased risk of the typical symptoms such as auditory hallucinations and delusions. METHODOLOGY/PRINCIPAL FINDINGS: Twelve schizophrenic patients treated with low levels of neuroleptics and twelve matched healthy controls participated in an event-related potential e

Bench Pressing Related Traumatic Laryngeal Fracture.

Laryngeal fracture is a rare but potentially life-threatening injury requiring prompt recognition and management due to the risk of airway compromise. We present the case of a 42-year-old man who presented to a tertiary center with symptoms of dysphonia and neck pain following blunt trauma sustained when a barbell fell on him while bench pressing without assistance. Computed tomography (CT) imaging confirmed a displaced fracture of the thyroid cartilage. He was managed conservatively with corticosteroids and high dependency unit airway monitoring. He recovered well and later resumed weightlift

The Value of Public Inquiries, Ethical Accountability, and Patient Voices: Reflections on the Infected Blood Inquiry.

INTRODUCTION: This article contributes to the continuing conversation in Haemophilia about the UK Infected Blood Inquiry (IBI). Discussion within the journal to date has largely foregrounded professional and technical perspectives. AIM: This article aims to bring back into view two elements central to the Inquiry-patient voice and the roles of law and ethics-and consider what this means for how the lessons of the IBI are interpreted in clinical and public discourse. METHODS: The article engages with published reflections and commentary to consider how responsibility has been framed, how cons

Face processing and name retrieval in an anomic aphasic: names are stored separately from semantic information about familiar people.

Recent models of face recognition have proposed that the names of familiar people are accessed from a lexical memory store that is distinct from the semantic memory store that holds information about such things as a familiar person's occupation and personality. Names are nevertheless retrieved via the semantic system. If such models are correct, then it should be possible for a patient to have full access to semantic information about familiar people while being unable to name many of them. We report this pattern in an anomic aphasic patient, EST, whose inability to recall the names of famili

[Hemianopsia and agnosia].

Hemianopsia and agnosia, the coexistence of a disorder of perception and a disorder of recognition, is exemplified by two patients chosen for their clinical similarity. These two patients each had a stroke, one of the left and the other of the right posterior cerebral artery, resulting right and left hemianopsia respectively. Primary visual functions such as visual acuity and contrast sensitivity were intact. The differences were only apparent as regards the severity of the agnostic disorders. The patient with the lesion on the left failed to recognize stylized drawings, was alexic, could not

A key system response of diametric space as a paradigm for understanding the phenomenology of anorexia nervosa.

There is increasing recognition of phenomenological approaches to understanding anorexia nervosa (AN), interrogating lived experiences and voices of those diagnosed with AN. This article builds on phenomenological accounts of AN, taking a step further for a spatial phenomenology, to examine how specific central features of AN experience are structured in diametric spatial terms. Building on growing research on the phenomenology of AN, it is remarkable how consistently the phenomenology of AN clusters around structural features of diametric space, as reactive, compensatory coping mechanisms of

Artificial Intelligence in Autism Spectrum Disorder Diagnosis: A Scoping Review of Face, Voice, and Text Analysis Methods.

BACKGROUND: Autism is a complex neurodevelopmental condition affecting social interaction and behavior. Traditional diagnostic methods, relying on observational techniques and interviews conducted by trained professionals, remain the gold standard for ASD diagnosis. However, these methods can be time-consuming and may be influenced by subjective factors. Recent advancements in artificial intelligence (AI) offer promising approaches to augment existing methods, potentially enhancing efficiency and providing additional objective data through facial, vocal, and textual analysis. OBJECTIVE: The o

Conversion disorder with aphonia in 12 years old male patient: A case report.

INTRODUCTION AND IMPORTANCE: Conversion aphonia is a rare disease characterized by total loss of voice. It is more commonly reported in females than males, and its diagnosis is based on a comprehensive patient's clinical history, and assessment of vocal cords and other laryngeal structures using Fiberoptic laryngoscopy. Early diagnosis and phonation have a significant role in the treatment. CASE PRESENTATION: A 12-year-old medically free male patient with unremarkable medical history and normal physical, social, cognitive and emotional developmental milestones presented to the otorhinolaryngo

"We are people too": consumer participation and the potential transformation of therapeutic relations within drug treatment.

BACKGROUND: While there is growing recognition of the benefits of user involvement within drug treatment there is scant literature documenting the actual implementation of such initiatives. Nonetheless, the extant research is remarkably consistent in identifying poor relationships between service users and staff as a principal barrier to the successful implementation of consumer participation. Focussing on participants' accounts of change within the 'therapeutic alliance', this paper investigates a consumer participation initiative introduced within three Australian drug treatment services. Infants at high and low likelihood for autism show different EEG developmental trajectories in speech tracking and statistical learning.

Delayed onset of canonical babbling and first words is often reported in infants later diagnosed with autism spectrum disorder. Identifying the neural mechanisms underlying language acquisition in autism is therefore critical to inform early diagnosis, prognosis, and intervention strategies. In this study, we investigated two speech processing mechanisms previously identified as atypical in children and adults with autism: the neural ability to track syllables, and statistical learning, the capacity to detect speech regularities beneath surface variability. We recorded 83 longitudinal high-den
