Sleep and wake disturbances following traumatic brain injury.

Traumatic brain injury (TBI) is a major health concern in industrialised countries. Sleep and wake disturbances are among the most persistent and disabling sequelae after TBI. Yet, despite the widespread complaints of post-TBI sleep and wake disturbances, studies on their etiology, pathophysiology, and treatments remain inconclusive. This narrative review aims to summarise the current state of knowledge regarding the nature of sleep and wake disturbances following TBI, both subjective and objective, spanning all levels of severity and phases post-injury. A second goal is to outline the various

Pain and sleep-wake disturbances in adolescents with depressive disorders.

The aims of this study were to (a) assess and compare sleep disturbances (including daytime and nighttime sleep patterns) in adolescents with depressive disorders and healthy peers, (b) examine the prevalence of pain in adolescents with depressive disorders and healthy peers, and (c) examine pubertal development, pain intensity, and depressive symptom severity as predictors of sleep disturbance. One hundred six adolescents (46 depressed, 60 healthy), 12 to 18 years (M = 15.10 years; 67% female; 77% Caucasian) completed subjective measures of sleep, presleep arousal, fatigue, and pain. Particip

Sleep-Wake profiles in patients with primary biliary cirrhosis.

BACKGROUND: Impaired sleep quality and daytime sleepiness have been described in patients with primary biliary cirrhosis (PBC). However, no information is available on their sleep timing/diurnal preference. AIMS: To evaluate such variables and determine their relationship with sleep quality, fatigue, pruritus and quality of life. METHODS: Seventy-four patients with PBC (58 ± 12 years), 79 healthy volunteers (56 ± 8 years) and 60 patients with cirrhosis (58 ± 12 years) underwent formal assessment of sleep quality/timing, diurnal preference and daytime sleepiness. Patients with PBC also underw

Sleep-wake disturbances in patients with amyotrophic lateral sclerosis.

OBJECTIVE: To evaluate the frequency, severity and determinants of sleep disturbances in patients with amyotrophic lateral sclerosis (ALS). METHODS: Information about night-time complaints was collected using a standardised questionnaire, the Pittsburgh Sleep Quality Index (PSQI), and the Epworth Sleepiness Scale (ESS) in a group of 100 patients with ALS and in 100 control subjects matched for age and sex. Functional disability was assessed using the ALS Functional Rating Scale-Revised (ALSFRS-R). Sleep was studied by overnight polysomnography in 12 patients. RESULTS: Fifty-nine patients wit

Sleep-wake changes in the premotor stage of Parkinson disease.

Longitudinal studies in Parkinson disease (PD) have shown that the prevalence of sleep disorders increases with advanced disease. However, two sleep disorders, namely excessive daytime sleepiness (EDS) and REM sleep behavior disorder (RBD) have been described to antedate the development of the classical motor signs and symptoms of PD. One epidemiological study from the Honolulu-Asia Aging Study showed that aging men who reported "being sleepy most of the daily" had a threefold excess risk for developing PD after a seven-year follow-up. The origin and nature of EDS were not investigated. This s

Genetic approaches for target identification in sleep/wake systems.

Attempts to therapeutically induce sufficient amounts of sleep have occurred since the advent of the medical arts, but the health implications of failing to maintain a correct balance of sleep/wake has only recently become widely appreciated both in the medical arena and among the general public. Inappropriate amounts of sleep have been linked to cognitive impairment, immune suppression and metabolic changes, as well as to the sequelae of reduced vigilance and increased likelihood of involvement in automobile accidents or other causes of fatalities. Currently approved treatments for insomnia a

Challenging issues: sleep-wake, augmentation and quality of life.

RLS is a common disorder which has an important impact on the health and quality of life of patients, with the main consequences being sleep-related. Special attention needs to be paid to patients with depression. The diagnosis of RLS is based on medical history and clinical examination. The most important laboratory examination is measurement of serum ferritin which should be >40 microg/L. The presence of all four essential diagnostic criteria confirms the RLS diagnosis. To detect secondary forms, a clinical examination and laboratory investigations, especially measuring serum ferritin are re

Waking up properly: is there a role of thermoregulation in sleep inertia?

We assume that alertness should be highest at the end of a sleep episode: it is not. There is always sleep inertia upon awakening, which can last minutes to hours, and whose underlying physiological mechanisms are largely unknown. Previously, we had found a functional relationship between the degree of distal vasodilatation (as measured by the distal-proximal skin temperature gradient (DPG) and sleepiness (as measured by subjective ratings), promoting rapid sleep onset. This led us to hypothesize that the dissipation of sleep inertia (sleepiness) would be associated with reverse thermoregulato

Daytime waking autonomic function and vascular control in OSA.

This paper deals with the potential role of two blood pressure regulatory systems in the development of hypertension in obstructive sleep apnoea (OSA). Beside the acute and long-term sympathoexcitation documented in this disease, there seems to be an impaired endothelium-dependent vascular relaxation. In particular, nitric oxide (NO)-mediated vasodilatation was impaired in patients with OSA. Both these changes act to increase blood pressure during physiological conditions. However, they may not be determinants for hypertension development in OSA as no differences between normo- and hypertensiv

The relation of sleep-wake patterns to seasonal depressive behavior.

A sample of 1644 adults in Alaska, Chukotka, Yakutiya, Western Siberia and Turkmenistan were surveyed to examine the relationship between seasonal depressive behavior and sleep-wake patterns. Interviews included items derived from the Seasonal Pattern Assessment Questionnaire, a 20 item general depression scale, and a 40 item sleep-wake questionnaire designed to assess the underlying mechanisms which regulate the cycles of sleep and wakefulness. Results show that subjects who suffer from seasonal depressive behavior, at latitudes ranging from 38-66 degrees North, are characterized by a sleep-w…

Reliability of sleep diaries for assessment of sleep/wake patterns.

Although sleep diaries are widely used in clinical and research settings, only a few studies have compared the subjective information recorded in these diaries to objective information about sleep recorded. The goal of this study was to determine if a sleep diary could be used to obtain reliable data about home sleep/wake patterns over a 24-hour period. Fifty subjects (25 narcoleptic and 25 matched control subjects) completed a sleep diary while undergoing 24-hour ambulatory polysomnographic monitoring. The percentage agreement between the subjective data recorded in the sleep diaries and poly

The sleep-wake circuit plays a role in temporal distortions.

Normally individuals give a fairly accurate subjective estimate of short seconds-to-minutes time interval durations. However, in some circumstances such as a high fever, sudden fear, and old age, temporal distortions can occur so that a conscious individual experiences the passage of subjective time that differs substantially from the actual measured time. Behaviors associated with these temporal distortions and with time perception in general are consistent with the existence of a neural clock circuit in the brain for estimating the passage of short time intervals, but experimental studies to

Decoding propulsive wakes using biomimetic seal whiskers.

Detecting and tracking swimming animals without disturbing them is a central need in fisheries protection and aquatic biological monitoring, yet optical and acoustic methods remain limited underwater. Passive hydrodynamic sensing offers an alternative: harbour seals can follow hydrodynamic wakes over long distances, and biomimetic whiskers respond measurably to the wakes of upstream bodies. Here we quantify how well four independently varied properties of a propulsive source can be discriminated simultaneously from a single force-instrumented whisker, using towing-tank measurements of ascaled

Supporting Population Mental Health in the Wake of Mass Tragedies.

The Bondi Beach terrorist attack has caused widespread community distress resulting in complex psychosocial challenges that require urgent attention to protect population mental health. Acute reactions including shock, fear and anger are normative, while the psychological responses unfolding over time may vary from adaptive to dysfunctional. A range of mental health and psychosocial services have been mobilised to address acute needs; however, to ensure a sustained and coherent framework of care, interventions should be guided by a set of principles that accurately respond to the unique nature

The Interaction between Sleep and Development on Wake EEG Oscillations.

The amount of time previously spent awake or asleep strongly impacts the sleep electroencephalogram (EEG), especially slow waves during nonrapid-eye-movement (NREM) sleep. These effects on the sleep EEG meaningfully interact with age and to a lesser extent developmental disorders such as attention-deficit hyperactivity disorder (ADHD). We aimed to determine whether EEG oscillations during wakefulness were likewise affected by the interaction of sleep and development, using data collected from 163 participants aged 3-25 years old (62 female). We analyzed age- and sleep-dependent changes in two …

Sleep lowers waking theta frequency in the rat hippocampus.

Hippocampal theta oscillations coordinate computations underlying learning and memory. The frequency of theta varies with factors such as locomotion and anxiety, but the effect of an animal's sleep history on theta frequency remains unknown. Using long-duration CA1 recordings in rats, we found that awake theta frequency progressively decreases following sleep but remains elevated during sleep deprivation. These changes were not accounted for by movement but were predicted by the proportion of sleep in the preceding 30 min. In contrast, theta frequency remained relatively stable during rapid ey
