Application of outcomes research in occupational low back pain: the Maine Lumbar Spine Study.

Outcomes research represents an expansion of traditional clinical research to include issues of cost and quality of care in usual clinical practice, emphasizing outcomes that matter most to patients. In low back disorders, outcomes research has focused on the lack of reliable information to support much of clinical practice and has demonstrated marked variability in the treatment of these common problems. The Maine Lumbar Spine Study represents an example of an outcomes research study to investigate the treatment of patients with sciatica in usual clinical practice. Because low back symptoms a

The costs of work-related musculoskeletal disorders in automotive manufacturing.

Inadequate application of ergonomic principles to the design of workplaces and individual jobs has adverse consequences for worker health and safety, especially in terms of strains, sprains, and other musculoskeletal disorders. In addition to the human pain and suffering, other losses are externalized to workers, with adverse financial and psychosocial impacts. There are also costs to employers through workers' compensation claims, scrap, and decreased production quality, medical insurance premiums, labor turnover, and adverse impacts on labor relations, although many of these are not linked b

Predicting failure to return to work.

AIM: The research question is: is it possible to predict, at the time of workers' compensation claim lodgement, which workers will have a prolonged return to work (RTW) outcome? This paper illustrates how a traditional analytic approach to the analysis of an existing large database can be insufficient to answer the research question, and suggests an alternative data management and analysis approach. METHODS: This paper retrospectively analyses 9018 workers' compensation claims from two different workers' compensation jurisdictions in Australia (two data sets) over a 4-month period in 2007. De

Predictors of returning to work.

An investigation of predictors of returning to work in a sample of physically injured persons who are receiving workers' compensation benefits and vocational rehabilitation is presented. One hundred fourteen injured subjects (86 with back injury; 28, other injury) undergoing vocational rehabilitation and receiving workers' compensation benefits were assessed on demographic, emotional, cognitive, financial incentive, and miscellaneous variables. Predictors for returning to work were identified using stepwise logistic regression. Patients with moderate or severe depression, defined as a score gr

The role of managed care in work-related injuries.

Many opportunities exist today for orthopedic surgeons in the treatment and management of work-related injuries. Physicians wishing to enter or participate in this market must realize that the role of the physician in the treatment of the injured worker has expanded from one of being solely the care giver to being a patient manager, a problem solver, and a communicator. A number of years ago, many orthopedic surgeons missed out on managed care contracting for health maintenance organizations because they thought they could survive without it. They soon learned the challenges of sitting on the

Cow-Related Injuries in Wisconsin During 2017-2023.

INTRODUCTION: The dairy industry-a key industry for the economy-remains a potentially hazardous industry across the nation. The literature on animal-related injuries in the agriculture industry is sparse. This analysis aimed to describe cow-related injuries, the utilization of workers' compensation, and the circumstances of the injuries. METHODS: This mixed methods analysis of agricultural injuries focused on cow-related injuries in hospital discharge and workers' compensation data during 2017-2023. Cases, aged 12 or older, were identified in the hospital discharge data using ICD10-CM codes.

Return-to-work interval and surgery for carpal tunnel syndrome. Results of a prospective series of 233 patients.

AIM OF THE STUDY: To evaluate the connection between the type of patient insurance and the time taken to return to work after carpal tunnel surgery. PATIENTS AND METHODS: Two hundred and thirty-three patients in full-time work were operated on for carpal tunnel syndrome between 1 January and 30 June 1998. They were divided into three groups: independent workers (n=87), wage earners in the private sector (n=90) and civil servants (n=56). Four categories were defined: manual workers, non-manual workers, patients with social security insurance and patients with workers compensation. The average

System Failure: Work Organization and Injury Outcomes among Latino Forest Workers.

OBJECTIVES: Forestry services is a hazardous industry with high job-related injury, illness, and fatality rates. The Northwest workforce is largely Spanish-speaking, Latino, and immigrant, working in poor conditions with insufficient attention paid to safety and health. Institutional racism fundamentally shapes the structural vulnerability of Latino immigrant workers. Given this context, we sought to understand how workplace organizational factors and safety climate affect job-related injuries in this industry. METHODS: We developed 23 case studies from personal interviews after selecting fro

[Croatian and international regulations on the protection and rights of workers exposed to asbestos at work].

New regulations on the protection and rights of workers occupationally exposed to asbestos were introduced in Croatia in 2007 and 2008. They have been harmonised with the European Union (EU) and International Labour Organization (ILO) regulations, and make a step forward in safety at work, health protection, social rights, and pension schemes for Croatian workers occupationally exposed to asbestos. The 2007 Croatian regulation on the protection of workers from the risks related to exposure to asbestos at work defines and describes activities in which workers can be occupationally exposed to as

Pain and the injured worker.

Physicians who treat injured workers with painful conditions face complex challenges that require skills beyond those of a clinician. To address these challenges effectively, physicians need to understand the logic of workers' compensation systems and the interests of the various participants in the systems. They must be prepared to interface constructively between their patients and the workers' compensation carrier and attend to a multitude of administrative issues. In the present article, the authors provide an extended case history with commentary to illustrate the challenges that physicia

Repair of the rotator cuff. A comparison of results in two populations of patients.

One hundred and seven shoulders of 103 consecutive patients were evaluated after primary repair of the rotator cuff. Twenty-four patients were receiving Workers' Compensation, and the other seventy-nine were not. Preoperative and postoperative evaluation of both groups included subjective assessment of pain, function, and patient satisfaction as well as objective assessment of the strength and active range of motion of the shoulder. The shoulder-rating scale of the University of California at Los Angeles was used to determine over-all success rates. The duration of follow-up ranged from twenty

An epidemiologic study of carpal tunnel syndrome and hand-arm vibration syndrome in relation to vibration exposure.

No Minnesota workers' compensation claims have been filed for permanent partial disability benefits with a diagnosis of hand-arm vibration syndrome (HAVS). A cross-sectional study was undertaken to evaluate 519 compensation claimants following carpal tunnel surgery. Workers with significant vibration exposure and symptoms compatible with HAVS were identified within the group. The results show that some workers are not diagnosed as having HAVS even though they meet the diagnostic criteria. This suggests that HAVS should be considered in the differential diagnosis when a worker presents with neu

Differences in health status among workers who acquired COVID-19 at work compared to workers who sustained a non-COVID-19 work-related injury or illness, 18-months post-injury/infection.

OBJECTIVE: To compare the physical and mental health status of workers who experienced work-related COVID-19 infections versus workers with other types of work-related injuries, 18 months after the initial work absence. METHODS: We collected survey data from 1,000 workers with workers' compensation claims between July and October 2022, and examined differences in seven self-reported physical and mental health outcomes. Potential outcome mediation models examined factors associated with observed differences in health outcomes. RESULTS: We observed better self-rated health, physical role funct

Work-related hand injuries in Ontario: an historical perspective.

Worker's compensation legislation was enacted in Ontario almost 90 years ago. Workers injured on the job gave up their right to sue employers and received no-fault compensation from an independent, employer-funded body called the Workmen's Compensation Board. Three academic health sciences centers in Ontario that are recognized for their commitment to patient care, research, and education compose part of the Specialty Program network with the Ontario Workplace Safety and Insurance Board (WSIB). Statistical data from the WSIB database for workers with hand injuries from 1996 to 2003 show an inc

Return to work after foot and ankle injury.

A surgeon faces important issues when returning a patient to the workplace, including workers' compensation issues and the need for physician guidance. These patients deserve appropriate diagnosis and treatment protocols, and the other members of the workers compensation treatment team can help maximize the effects of careful guidance by a physician. The speediest possible return to the workplace is usually best for all concerned, the injured worker, the employer and the physician. The treatment team within the workers' compensation system assists in the facilitation and coordination of the me

Endoscopic carpal tunnel release: the guide tube technique.

This article introduces a modification of the Chow double-portal endoscopic carpal tunnel release and reviews the results of 71 endoscopic carpal tunnel decompressions performed on 50 consecutive patients over a 20-month period. The guide tube technique was developed to improve patient safety and provide good ligament visualization. In the clinical study, 33 patients not receiving workers' compensation were ready to return to work at 12 days (mean). The 17 patients receiving workers' compensation were divided into two groups: workers with carpal tunnel syndrome only and workers with carpal tun

ICF-Based Job Performance Predictors for South Korean Industrial Accident Workers: Population-Based 3-Year Longitudinal Analysis.

Since workers who have experienced industrial accidents may have a physical impairment, their workload is very likely to be less than before the industrial accident. This epidemiological study identified ICF-based predictive factors affecting the work performance of South Korean workers who were economically active after undergoing medical treatment (rehabilitation) related to injuries caused by industrial accidents by using the Panel Study of Worker's Compensation Insurance (2018-2020) as a reference. We analyzed 1383 subjects who were engaged in economic activities. The explanatory variables

It may cost an arm and a leg: workers value and occupational fatality rates in the U.S.

BACKGROUND: The present study aims to observe how societal indicators of workers' values at the state-level are related to health and safety outcomes, particularly major injuries and fatalities in the U.S. Underscoring workforce flexibility and workability over workforce stability and safety might be indicative of the worth of workers which can be associated with occupational safety and health concerns. METHODS: Linear regression analysis with a log-transformed dependent variable was adopted to examine how the state-level indicators of worker value in terms of 1) minimum wage, using data from
