A fresh look at the definition of susceptibility of Streptococcus pneumoniae to beta-lactam antibiotics.

Definitions for susceptibility or resistance of Streptococcus pneumoniae to penicillin were not developed until penicillin-resistant pneumococci appeared in South Africa in the late 1970s. The definition that was accepted (which still remains in use) and later definitions of resistance to most other beta-lactam antibiotics were derived from laboratory and clinical data relating to the treatment of meningitis, not otitis media, sinusitis, or pneumonia. An understanding of the origin of these definitions helps to resolve the apparent paradox that infections of the respiratory tract due to seemin

The glutathione metabolism of the beta-lactam producer filamentous fungus Penicillium chrysogenum.

Glutathione (gamma-L-glutamyl-L-cysteinyl-glycine; GSH) shares structural similarities with the beta-lactam biosynthetic intermediate ACV-tripeptide (delta-(L-alpha-aminoadipyl)-L-cysteinyl-D-valine). Not surprisingly, GSH has been reported to inhibit the beta-lactam biosynthetic machinery quite effectively and, hence, strategies to decrease the intracellular GSH concentrations without influencing negatively the physiological status of idiophasic mycelia would attract industrial interests. Here we present a detailed map of the GSH metabolic network of P. chrysogenum and show a promising way to

In vitro susceptibility of Chlamydia pecorum to macrolides, tetracyclines, quinolones and beta-lactam.

The in vitro susceptibility of Chlamydia pecorum to two macrolides (clarithromycin and erythromycin), two tetracyclines (doxycycline and minocycline), two quinolones (ofloxacin and ciprofloxacin) and one beta-lactam (ampicillin) was determined. The MICs were 0.004 to 0.008 microg/ml for clarithromycin, 0.008 to 0.031 microg/ml for doxycycline and minocycline, 0.063 to 0.125 microg/ml for erythromycin, 0.25 to 0.5 microg/ml for ofloxacin and 0.25 to 1.0 microg/ml for ciprofloxacin. The MIC for ampicillin was greater than 1,024 microg/ml. The results show clarithromycin and doxycycline are the t

Cross-reactivity of T cell lines and clones to beta-lactam antibiotics.

To clarify on a molecular level the specific T cell response to haptens like penicillin G, we generated T cell lines and clones from penicillin-allergic patients. Two types of beta-lactam reactivity of T cells could be delineated: one group of patients showed a rather restricted specificity, as the penicillin-elicited T cell lines generated from such donors proliferated only to the stimulating penicillin, but not to other beta-lactam antibiotics nor to cephalosporines, even if the side chain was identical. This indicates that the penicilloyl structure together with the side chain was recognize

Allergic reactions to parenteral beta-lactam antibiotics in patients with cystic fibrosis.

Certain antibiotics, particularly piperacillin, have been reported to be associated with a high incidence of allergic reactions in patients with cystic fibrosis. We initiated a study to determine the relative frequency of allergic reactions, ie, drug-induced fever and rash, to parenteral beta-lactam antibiotics in adult patients with cystic fibrosis. Charts of 111 patients were reviewed for each hospitalization to assess allergic reactions. Of 90 evaluable patients, 26 patients developed one or more allergic reactions to beta-lactam antibiotics. The number of allergic reactions per number of p

In vitro activity of cefpodoxime in comparison with other oral beta-lactam antibiotics.

The aim of our study was to re-evaluate the in vitro activity of cefpodoxime in comparison with other oral beta-lactam antibiotics against bacteria causing respiratory tract infections. The study drugs were cefpodoxime, cefaclor, cefixime, cefuroxime, cefetamet, cefprozil, and the combination of amoxicillin and clavulanic acid (= augmentin). In addition, cefotaxime as the standard agent of parenteral third generation cephalosporins was examined. The organisms tested were Staphylococcus aureus, Streptococcus pyogenes, Streptococcus agalactiae, streptococci of serogroups C and G, Streptococcus p

In-vitro evaluation of ampicillin/brobactam and comparison with other beta-lactam antibiotics.

The ability of brobactam to inhibit beta-lactamases and the in vitro activity of ampicillin combined with brobactam was compared with another beta-lactamase inhibitor, clavulanic acid, and other beta-lactam antibiotics. Both inhibitors showed good and similar activity against staphylococcal penicillinase and most broad-spectrum beta-lactamases found in the Enterobacteriaceae, whether plasmid or chromosomally mediated. Both inhibitors were less active against chromosomally mediated cephalosporinases in Enterobacteriaceae, but brobactam was 8-50 times more potent than clavulanic acid. The amount

[Mechanisms of action of beta-lactam antibiotics and mechanisms of non-enzymatic resistance].

The mode of action of beta-lactam antibiotics, and the non-enzymatic resistance mechanisms to their activity, are intimately linked to the structure and biosynthesis of the bacterial cell wall. The bacteriostatic effect of beta-lactam antibiotics is related to their various interactions and concomitant inhibition of essential enzymes (transpeptidases, carboxypeptidases) involved in the terminal stages of peptidoglycan biosynthesis. These cytoplasmic membrane-associated target enzymes bind the antibiotics covalently, and hence are known as penicillin-binding proteins (PBPs). The bactericidal ef

Beta-lactam antibiotics increase the frequency of plasmid transfer in Staphylococcus aureus.

The transfer of a plasmid specifying tetracycline resistance between different derivatives of Staphylococcus aureus by phage mediated conjugation was enhanced 100- to 1000-fold by exposure of the culture to subinhibitory concentrations of beta-lactam agents. A variety of other antibiotics, including vancomycin and teicoplanin, had no such effect. The enhanced frequency of transfer was probably due to the formation of large bacterial aggregates.

[Synergism of ciprofloxacin with beta-lactam antibiotics, gentamicin, minocycline and pipemidic acid].

Studies on the Synergism of Ciprofloxacin with beta-Lactam Antibiotics, Gentamicin, Minocycline and Pipemidic Acid. Using Escherichia coli, Klebsiella pneumoniae/Klebsiella oxytoca, Pseudomonas aeruginosa and Staphylococcus aureus strains, we examined a possible influence of pipemidic acid, minocycline, gentamicin, cefazolin, mezlocillin and ampicillin on the antibiotic activity of ciprofloxacin. We found in all bacterial species synergistic influence by pipemidic acid. When ciprofloxacin was combined with gentamicin, synergism was observed in E. coli and in K. pneumoniae/oxytoca, additive eff

Gram-positive superinfections following beta-lactam chemotherapy: the significance of the enterococcus.

The recent literature was reviewed with regard to the risks of superinfection following beta-lactam chemotherapy. The summary publications for the pseudomonas-active penicillins (azlocillin, carbenicillin, mezlocillin, piperacillin and ticarcillin), cefoperazone, cefotaxime, ceftazidime, imipenem and moxalactam show marked variations. Moxalactam was most likely to produce both gram-negative (5-38%) and enterococcal (2.2-12%) superinfections. Ceftazidime or moxalactam therapy was more often associated with anaerobic superinfections, usually by Clostridium spp., than the other beta-lactams. Comp

Susceptibility of anaerobic bacteria to Sch 34343 and other beta-lactam antibiotics.

The in-vitro activity of Sch 34343 was evaluated against 381 anaerobic clinical isolates and compared with that of imipenem, cefoxitin, latamoxef (moxalactam), cefotaxime and cefoperazone. Test isolates included strains with unknown resistance to the beta-lactam drugs, and two Bacteroides fragilis strains that could inactivate cefoxitin. Sch 34343 and imipenem demonstrated superior activity against all the anaerobic bacteria tested, with MIC90s less than or equal to 1 mg/l. Sch 34343 was generally one- to two-fold more active than imipenem. Resistance to both was demonstrated by the two Bact.

In vitro activity of temocillin (BRL 17421), a novel beta-lactam antibiotic.

The minimal inhibitory concentration of temocillin (BRL 17421) against 476 clinical isolates was determined by an agar dilution method. Temocillin was active against most of the Enterobacteriaceae, Haemophilus, and Neisseria strains tested. The compound showed low activity or was inactive against Bacteroides, Campylobacter, Acinetobacter, Pseudomonas, and Staphylococcus aureus strains. Within each species, individual strains showed a high degree of uniformity in their susceptibility to temocillin; the drug concentrations that inhibited the growth of 90% of organisms were the same or close to t

LY-127935: a novel beta-lactam antibiotic with unusual antibacterial activity.

The in vitro activity of LY-127935, a new beta-lactam antibiotic, was examined by using 370 clinical bacterial strains. In comparison with several other beta-lactam agents, LY-127935 was the most inhibitory against the Enterobacteriaceae. It was remarkably active against multi-drug-resistant strains of Enterobacter spp., Serratia spp., and Pseudomonas aeruginosa. LY-127935 had four- to eightfold greater activity than did cefoxitin against Bacteroides fragilis. Production of beta-lactamase by Enterobacteriaceae did not influence the minimal inhibitory concentration of LY-127935. However, the be

Nocardicin A, a new monocyclic beta-lactam antibiotic III. In vitro evaluation.

Nocardicin A, a new monocyclic beta-lactam antibiotic, exerts a comparatively potent antimicrobial activity against gram-negative organisms, especially Pseudomonas aeruginosa, the indole-positive and indole-negative Proteus groups (except Pr. morganii), Serratia marcescens and the Neisseria groups. The in vitro antimicrobial activity of nocardicin A against clinical isolates of Ps. aeruginosa was about twice that of carbenicillin. The mean MICs of nocardicin A for Pr. mirabilis, Pr. rettgeri and Pr. inconstans ranged from 3.13 to 12.5 microgram/ml and were 25 similar to 50 microgram/ml for Pr.

Lysis enhancement: a novel form of interaction between beta-lactam antibiotics.

A novel form of synergy has been observed to occur between cephalexin and certain other beta-lactam antibiotics. In the presence of cephalexin a reduction was found in the concentration of other beta-lactam agents needed to induce lysis of Escherichia coli; the effect was particularly marded when cephalexin was allowed to act for a short time before the addition of the second agent. The basis of this type of synergy--which is not likely to be of therapeutic value--is discussed in terms of a theroy perviously put forward, whcih suggests that penicillins and cephalosporins have two distinct site

Is the Anamnesis Enough to De-Label Patients with Reported Beta-Lactam Allergy?

Background: The decision whether to de-label patient with suspected BL hypersensitivity is based on risk stratification. The aim of this study was to prepare a characteristic of diagnostic risk groups and to create a model enabling the identification of the low-risk diagnostic group. Methods: We analyzed the medical records of patients hospitalized due to suspected hypersensitivity to BL antibiotics. Based on their medical-history data, patients were divided into three diagnostic risk groups, using the criteria proposed by Shenoy et al. Univariate and multivariate analysis models were used to

Characterization of Beta-Lactam Resistome of Escherichia coli Causing Nosocomial Infections.

Nosocomial infections caused by Escherichia coli pose significant therapeutic challenges due to the high expression of genes encoding antimicrobial drug resistance. In this study, we investigated the conformation of the beta-lactam resistome responsible for the specific pattern of resistance against beta-lactam antibiotics. A total of 218 Escherichia coli strains were isolated from in-hospital patients diagnosed with nosocomial infections, obtained from various sources such as urine (n = 49, 22.48%), vaginal discharge (n = 46, 21.10%), catheter tips (n = 14, 6.42%), blood (n = 13, 5.96%), fece
