The use of fetal and anencephalic tissue for transplantation.

Advances in transplantation have extended the life and relieved the suffering of thousands of individuals. The prospect of being able to use tissues from embryos, as well as from anencephalic newborns, offers the promise of further relief of suffering. However, these possibilities raise significant moral and public policy issues. The question arises of the extent to which those who disapprove of abortion may make use of tissues derived from abortion in order to treat serious diseases. This essay argues that, with proper safeguards, such tissue can be used without cooperating in abortion. That

[Diagnosis of fetal malformations with ultrasound--state of development].

Ultrasonography is of great importance for the prenatal diagnosis of fetal malformations and abnormalities. An early diagnosis in the second trimester is of great interest for an intrauterine or an extrauterine therapy planning (the choice of the time and mode of delivery). Defects of the neural tube including hydrocephalus, malformations of the extremities, the gastrointestinal tract, omphaloceles, the urogenital and cardiac system are described. Four cases of fetal malformations are presented: fetal myelomeningocele, hydrocephalus, bilateral hydronephrosis and lymphangioma with fetal ascites

A clinical service for prenatal diagnosis.

A centralised clinical prenatal diagnostic service has been established and the results of amniocentesis in 174 patients are discussed. Termination of pregnancy was performed in 3 patients on the basis of cytogenetic abnormalities (47,XXY, 47,XXX, 47,XY 21+): in 2 patients with anencephalic fetuses on the basis of alpha-fetoprotein and ultrasound investigations, and 1 patient with fetal Hurler's syndrome. One mother, aged 45, refused amniocentesis and gave birth to a child with Down syndrome. Amniocentesis was followed by fetal death and abortion in 3 patients, all of whom had repeat amniocent

Impact of Voluntary Folic Acid Fortification of Corn Masa Flour on RBC Folate Concentrations in the U.S. (NHANES 2011-2018).

Surveillance data have highlighted continued disparities in neural tube defects (NTDs) by race-ethnicity in the United States. Starting in 2016, the Food and Drug Administration (FDA) authorized voluntary folic acid fortification of corn masa flour to reduce the risk of neural tube defects (NTDs) among infants of Hispanic women of reproductive age. To assess the impact of voluntary corn masa fortification, cross-sectional data from the National Health and Nutrition Examination Survey (NHANES) 2011-2018 for Hispanic women of reproductive age with available red blood cell (RBC) folate concentrat

Introduction: organ transplantation -- defining the boundaries of personhood, equity and community.

The contributors to this issue address a range of moral problems associated with organ transplantation. Although each paper takes on a different challenge, they share an underlying concern with the reconfiguration of personhood and community brought about by organ transplant therapies. Taken together, these six essays provide a robust critique of the medical, social, political and economic factors that drive the practice of human organ replacement. In the first essay, Marshall, Thomasma, and Daar examine marketing strategies that have been proposed for increasing the number of organs available

Brain life and brain death--the anencephalic as an explanatory example. A contribution to transplantation.

The current debate regarding the suitability of anencephalics as organ donors is due primarily to misunderstandings. The anatomical and neurophysiological literature shows that the anencephalic lacks a cerebrum because of the failure of neuralplate fusion. However, even the incomplete function of an atrophic brain stem is currently accepted at law in most if not all countries as sufficient for brain life: which is to say, cessation of breathing is currently required in order to make the diagnosis of brain death. Because of the extensive incompleteness of the anencephalic's brain, it is not pos

Chemicals, birth defects and stillbirths in New Brunswick: associations with agricultural activity.

We describe a series of investigations that were conducted in support of the Task Force on Chemicals in the Environment and Human Reproductive Problems in New Brunswick. Geographic and temporal analyses and case-control studies, with the use of vital statistics, hospital records, the Canadian Congenital Anomalies Surveillance System and chemical databases, revealed no association between pesticides used in forestry and reproductive problems. Evidence of an association between the potential exposure to agricultural chemicals and three major anomalies combined as well as spina bifida without hyd

Ultrasound in the diagnosis of congenital anomalies.

With high-resolution ultrasound equipment, it is now possible to diagnose certain fetal anomalies in the third trimester and in some cases before the twentieth week of gestation. During a 27 month period 2,548 ultrasound scans were performed in high-risk patients. An anomaly was diagnosed in 10 of 122 second-trimester patients who were at risk for recurrent fetal defects. Fetal deformity was also found in 26 third-trimester patients. Of the 2.8% of patients found to have polyhydramnios 18% were associated with various types of anomaly. With ultrasound it was possible to examine internal fetal

Twins and neural tube defects.

The hypothesis that anencephalus stems from fetus-fetus interactions in dizygotic twin pairs is examined by comparing the epidemiological predictions of the hypothesis with available observations. The hypothesis itself was based upon the disease-discordance and sex-concordance characteristics of twin pairs affected with anencephalus, and upon the sex ratio of the disease itself. The testable predictions of the hypothesis are (a) that variations in the incidence of anencephalus should be related to variations in dizygotic twinning rates, and particularly that dizygotic twinning rates will set u
