Ileal mucosal and fecal pancreatitis associated protein levels reflect severity of salmonella infection in rats.

BACKGROUND: Microbial infections induce ileal pancreatitis-associated protein/regenerating gene III (PAP/RegIII) mRNA expression. Despite increasing interest, little is known about the PAP/RegIII protein. Therefore, ileal mucosal PAP/RegIII protein expression, localization, and fecal excretion were studied in rats upon Salmonella infection. RESULTS: Salmonella infection increased ileal mucosal PAP/RegIII protein levels in enterocytes located at the crypt-villus junction. Increased colonization and translocation of Salmonella was associated with higher ileal mucosal PAP/RegIII levels and secre

The Histoplasma capsulatum vacuolar ATPase is required for iron homeostasis, intracellular replication in macrophages and virulence in a murine model of histoplasmosis.

Histoplasma capsulatum is a dimorphic fungal pathogen that survives and replicates within macrophages (Mphi). To identify specific genes required for intracellular survival, we utilized Agrobacterium tumefaciens-mediated mutagenesis, and screened for H. capsulatum insertional mutants that were unable to survive in human Mphi. One colony was identified that had an insertion within VMA1, the catalytic subunit A of the vacuolar ATPase (V-ATPase). The vma1 mutant (vma1::HPH) grew normally on iron-replete medium, but not on iron-deficient media. On iron-deficient medium, the growth of the vma1 muta

Mitochondrial ND5 gene variation associated with encephalomyopathy and mitochondrial ATP consumption.

Mitochondrial encephalomyopathy and lactic acidosis with strokelike episodes (MELAS) is a severe young onset stroke disorder without effective treatment. We have identified a MELAS patient harboring a 13528A-->G mitochondrial DNA (mtDNA) mutation in the Complex I ND5 gene. This mutation was homoplasmic in mtDNA from patient muscle and nearly homoplasmic (99.9%) in blood. Fibroblasts from the patient exhibited decreased mitochondrial membrane potential (Deltapsim) and increased lactate production, consistent with impaired mitochondrial function. Transfer of patient mtDNA to a new nuclear backgr

Serum markers of microbial translocation and intestinal damage in assessment of gastrointestinal tract involvement in systemic sclerosis.

Gastrointestinal (GI) tract involvement affects up to 90% of Systemic sclerosis (SSc) patients. The presence of GI symptoms is assessed by the University of California, Los Angeles, and Scleroderma Clinical Trials Consortium Gastrointestinal Scale (UCLA SCTC GIT 2.0). Microbial translocation (MT) is reported in SSc patients consequently to increased intestinal permeability due to intestinal damage (ID) and dysbiosis. Aim of this study was to assess circulating levels of LBP and EndoCab IgM (markers of MT), IL-6 (marker of inflammation), I-FABP and Zonulin (markers of ID) in a cohort of SSc pat

Development of a dual-color, double fusion FISH assay to detect RPN1/EVI1 gene fusion associated with inv(3), t(3;3), and ins(3;3) in patients with myelodysplasia and acute myeloid leukemia.

Approximately 2-3% of adult patients with acute myeloid leukemia harbor a rearrangement of RPN1 (at 3q21) and EVI1 (at 3q26.2) as inv(3)(q21q26.2), t(3;3)(q21;q26.2), or ins(3;3)(q26.2;q21q26.2). The most recent World Health Organization (WHO) classification has designated AML with inv(3) or t(3;3) and associated RPN1/EVI1 fusion, as a distinct AML subgroup associated with an unfavorable prognosis. We have created a dual color, double fusion fluorescence in situ hybridization (D-FISH) assay to detect fusion of the RPN1 and EVI1 genes. A blinded investigation was performed using 30 normal bone

Characterization of a de novo balanced translocation in a patient with moderate mental retardation and dysmorphic features.

Moderate mental retardation (MR) could affect up to 3% of the general population. A proportion of these cases has a genetic origin. Genes responsible for mental retardation can be identified taking advantage of familial cases or patients carrying a chromosomal rearrangement. We have studied a female patient with mild mental retardation and dysmorphic features. Cytogenetic and molecular investigations revealed a de novo balanced translocation 46, XX, t(5;18)(q21.3;q21.32) in the patient. The karyotypes of the parents are normal. We mapped the breakpoints of the translocation on chromosomes 5 an

[Comparative study of proton pump inhibitors].

Proton pump inhibitors (PPI) have influenced dramatically the management of acid-related disorders in recent years. They all have a broadly similar mechanism of action. There are some differences however, between the PPIs in their pharmacokinetics, pharmacodynamics, clinical efficacy, and potential for drug interactions. Although the individual PPI have similar efficacy in many cases, differences between them should be considered when choosing a treatment regimen. Further clinical trials are needed to assess the clinical importance of these differences, as well as to assess whether the potenti

Saccharomyces cerevisiae fermentation products reduce bacterial endotoxin concentrations and inflammation during grain-based subacute ruminal acidosis in lactating dairy cows.

Subacute ruminal acidosis (SARA) is a metabolic disorder in dairy cows that is associated with dysbiosis of rumen and hindgut microbiomes, translocation of immunogenic compounds from the gut lumen into blood circulation, and systemic inflammatory response. In this study we hypothesized that Saccharomyces cerevisiae fermentation products (SCFP) attenuate the increases in ruminal and peripheral bacterial endotoxin concentrations and the inflammation resulting from repeated induction of SARA. Lactating Holstein dairy cows (parity 2 and 3+, n = 32) were fed diets with or without SCFP (all from Dia

Pancreatic enzymes and bile acids alone and in combination for acutely ill, severely malnourished children: a multi-country double-blind randomised placebo-controlled trial.

BACKGROUND: Translocation of bacteria across the intestinal barrier has been postulated to contribute to mortality among severely malnourished children. Pancreatic enzymes (PE) and bile acids (BA) have anti-bacterial properties in the small intestine, but severe malnutrition is associated with impaired exocrine pancreatic and hepatobiliary functions. We evaluated whether ancillary treatment with PE and BA improves survival in hospitalised, acutely ill, severely malnourished children. METHODS: We conducted a multicentre, adaptive, 2 × 2 factorial, randomised, double-blinded controlled trial of

Faecal cytokine profiling as a marker of intestinal inflammation in acutely decompensated cirrhosis.

BACKGROUND & AIMS: Gut dysbiosis and inflammation perpetuate loss of gut barrier integrity (GBI) and pathological bacterial translocation (BT) in cirrhosis, contributing to infection risk. Little is known about gut inflammation in cirrhosis and how this differs in acute decompensation (AD). We developed a novel approach to characterise intestinal immunopathology by quantifying faecal cytokines (FCs) and GBI markers. METHODS: Faeces and plasma were obtained from patients with stable cirrhosis (SC; n = 16), AD (n = 47), and healthy controls (HCs; n = 31). A panel of 15 cytokines and GBI markers…

Comparison of Immediate Oral Feeding vs Conventional Oral Feeding in Patients with Mild to Moderately Severe Acute Pancreatitis: A Randomized Controlled Trial from a Tertiary Care Center in North India.

AIM AND BACKGROUND: Acute pancreatitis (AP) is an inflammatory condition of the pancreas with varying severity, traditionally managed by delaying oral feeding to prevent enzyme stimulation. However, prolonged fasting may worsen patient outcomes by increasing catabolism and the risk of bacterial translocation. This study aims to compare the effects of immediate oral feeding (IOF) within 24 hours of hospital admission vs conventional oral feeding (COF) after 48 to 72 hours in patients with mild to moderately AP, hypothesizing that IOF will reduce length of hospital stay (LOHS) without increasing…

Polydextrose reduces the infection of Klebsiella pneumoniae in mice by downregulating the expression of TamA.

Polydextrose (PDX), as a prebiotic, is an extensively branched glucose polymer that can promote the growth of beneficial bacteria in the gut. Recent research indicates that PDX regulates intestinal function and supports immune balance, which helps to protect the gut from pathogenic bacteria. However, scarce research has been found that PDX prevents the host infection through the direct effects on the pathogen. In this study, we developed a mouse model infected with Klebsiella pneumoniae by pretreating with PDX, assessed the effect of PDX on K. pneumoniae acute infection in mice, and explored i

Stable serum peptidoglycan fragment levels do not support leaky gut in the acute phase or at one month following "mild" traumatic brain injury: A preliminary study.

The gut-brain axis is increasingly recognized as contributor to the pathophysiology of brain disorders, in part through its influence on inflammation. Impaired gut health can lead to so-called leaky gut, allowing bacterial cell wall fragments such as peptidoglycan-derived muramyl dipeptide (MDP) to translocate to the circulation. MDP can activate microglia, key mediators of neuroinflammation, via the intracellular receptor nucleotide-binding oligomerization domain-containing protein 2 (NOD2). Although neuroinflammation is a hallmark of "mild" traumatic brain injury (mTBI), the link between lea

A 12 week longitudinal study of microbial translocation and systemic inflammation in undernourished HIV-infected Zambians initiating antiretroviral therapy.

BACKGROUND: Undernourished, HIV-infected adults in sub-Saharan Africa have high levels of systemic inflammation, which is a risk factor for mortality and other adverse health outcomes. We hypothesized that microbial translocation, due to the deleterious effects of HIV and poor nutrition on intestinal defenses and mucosal integrity, contributes to heightened systemic inflammation in this population, and reductions in inflammation on antiretroviral therapy (ART) accompany reductions in translocation. METHODS: HIV-infected, Zambian adults with a body mass index <18.5 kg/m2 were recruited for a p

Profound systemic inflammatory response syndrome following non-emergent intestinal surgery in children.

PURPOSE: Systemic inflammatory response syndrome (SIRS) is an uncommon but severe complication in surgical patients. While SIRS is well known, it is poorly described in the pediatric population. The goal of this study was to describe the incidence of profound SIRS following non-emergent intestinal surgery in children and to identify potential risk factors. METHODS: A retrospective review was conducted for patients 0-19 years of age following intestinal surgery and/or lysis of adhesions from 01/01/1999-02/28/2012. Children were excluded for preoperative instability or frank bowel perforation.

Identification of new susceptibility regions for X;Y translocations in patients with testicular disorder of sex development.

Testicular disorder of sex development in the presence of a 46,XX karyotype is a rare condition. In most instances, it is caused by an X;Y translocation in the paternal gametes, causing SRY to be transferred on the X chromosome. An abnormal recombination event between homologous genes PRKX and PRKY is implicated in approximately one third of the cases. In this study, we report the characterization by fluorescence in situ hybridization of four patients with a 46,X,der(X)t(X;Y) constitution: two monozygotic adult twins, one adult male and a young boy.
