Mastoidectomy in the elderly.

OBJECTIVE: To present conditions requiring mastoid surgery in elderly patients. STUDY DESIGN: Retrospective. RESULTS: Twenty-seven patients (aged 65-90 years) who underwent mastoidectomy were enrolled. The etiologies included chronic otitis media (COM) with (n = 16) or without (n = 5) cholesteatoma, otogenic meningitis and malignant otitis externa (MOE; n = 3 each). Urgent mastoidectomy was indicated for 4 intracranial complications (3 meningitis and 1 brain abscess) and 14 extracranial complications of acute/COM or MOE (7 patients with labyrinthitis and 7 patients with facial nerve, FN, par…

A dangerous dilemma: management of infectious intracranial aneurysms complicating endocarditis.

A 41-year-old right-handed man with bicuspid aortic valve and a 3-month history of chronic fever and weight loss presented with sudden onset of severe headache. Computerised tomography of the head revealed a right basal ganglia haemorrhage. Further investigation documented Streptococcus mitis bacteraemia, a fusiform right middle cerebral artery aneurysm, and an abscess at the base of the anterior leaflet of the mitral valve. The patient subsequently died when repeat aneurysmal haemorrhage resulted in cerebral herniation and brain death while on antibiotic therapy. Infectious intracranial aneur

[Neurocysticercosis].

A 54-year-old female patient with a 10-year history of ventriculoperitoneal shunt resulting from communicating hydrocephalus of undetermined aetiology is reported. Transient gait disturbances and cerebral infarction at the age of 46 did not lead to further insights into the nature of the disease. After many years with only occasional disturbances, a distinct organic brain syndrome developed. Thorough examination led to a tentative diagnosis of neurocysticercosis; this was based on the history, liquor diagnosis and cerebral microcalcifications in CT. Despite the initiation of specific therapy,

Hereditary hemorrhagic telangiectasia with growing pulmonary arteriovenous fistulas followed for 24 years.

A 47-year-old man was admitted for follow-up of an abnormal chest x-ray. He had a history of epistaxis and a brain abscess and a family history of pulmonary arteriovenous fistulas. Physical examination showed clubbed fingers and telangiectasia of the tongue. Laboratory data revealed evidence of polycythemia and hypoxia. Contrast echocardiography and pulmonary perfusion scintigraphy were suggestive of a right-to-left shunt. From the oxygen tension and content of blood taken at cardiac catheterization, the shunt ratio was calculated to be 57.8%. Multiple bilateral pulmonary arteriovenous fistula…

[Chemotaxis defect and recurrent E. coli meningitis].

The increased susceptibility to bacterial infections due to a decreased chemotaxis of neutrophil granulocytes was investigated in a premature small for gestational age baby, who such suffered from recurrent E. coli-meningitis. Causes as anatomic lesions, brain abscess, septic granuloumatosus disease and the Chédiak-Higashi-anomaly were ruled out. At the age of 9 and 13 weeks chemotaxis function compared to age-matched and adult controls was diminished. Addition of serum from healthy adults increased directed migration without reaching normal levels. The study of the complement status revealed …

Awake Craniotomy for a Frontal Astrocytoma: A Case Report.

Awake craniotomy is a surgical procedure that has been gaining significance over the past decades. Neuronavigation is an intraoperative technology that locates tumors and monitors the brain cortex during awake craniotomy. The presence of cerebral low-grade gliomas in the frontal lobe creates a risk of affecting vital centers of the brain cortex during surgery. We present a clinical case of a 42-year-old male patient who entered the neurosurgery clinic with a clinical manifestation of headache for two months. MRI showed evidence of the recurrence of a left frontal glioma. Differential diagnoses

Perspective: Risks/adverse events for epidural spinal injections.

BACKGROUND: Despite the lack of FDA (Food and Drug Administration) approval, cervical and lumbar epidural spinal injections are frequently performed in the US to address back pain and/or painful radiculopathy. The three major types of injections include; interlaminar/translaminar (ESI), transforaminal (TFESI), or caudal injections. Notably, most studies document little to no clear short-term, and no long-term benefits/efficacy for these injections vs. various placebos. METHODS: More adverse events (AE) occurred with cervical© rather than lumbar (L) injections, and more severe AE were attribut

A Toothache That Was a Stroke: Acute Right Parietal Infarction Presenting As Isolated Left-Sided Odontogenic-Like Pain.

Pain as a presenting symptom of acute ischemic stroke is uncommon and may delay diagnosis when typical focal neurological deficits are absent. A 52-year-old man with arterial hypertension and a history of tobacco and alcohol use presented to the ED with sudden, severe left-sided toothache-like pain perceived as arising from the teeth. He had no recent dental procedure, trauma, fever, or systemic infectious symptoms. Cerebral Infectious Opportunistic Lesions in a Patient with Acute Myeloid Leukaemia: The Challenge of Diagnosis and Clinical Management.

Central nervous system (CNS) lesions, especially invasive fungal diseases (IFDs), in immunocompromised patients pose a great challenge in diagnosis and treatment. We report the case of a 48-year-old man with acute myeloid leukaemia and probable pulmonary aspergillosis, who developed hyposthenia of the left upper limb, after achieving leukaemia remission and while on voriconazole. Magnetic resonance imaging (MRI) showed oedematous CNS lesions with a haemorrhagic component in the right hemisphere with lepto-meningitis. After 2 weeks of antibiotics and amphotericin-B, brain biopsy revealed chroni

[Intracranial cystic lesions].

CLINICAL PROBLEM: Intracerebral cysts are common findings in imaging of the neurocranium and are not always clinically significant. The pathological spectrum of intracerebral cysts is, however, very broad and in addition to incidental findings includes developmental disorders, malformation tumors, primary and secondary neoplasms and infectious etiologies, such as cerebral abscess formation, cysticercosis or residuals after congenital cytomegalovirus infections. Intracerebral cystic defects may be caused by inflammatory central nervous system (CNS) diseases, such as multiple sclerosis as well a

Cryptococcus neoformans var. grubii-Induced Arthritis with Encephalitic Dissemination in a Dog and Review of Published Literature.

This article describes the clinical, pathological, and immunohistochemical findings associated with Cryptococcus neoformans var. grubii in a 4-year-old female Boxer dog from Uberlândia, Minas Gerais, Southeastern Brazil. Clinically, there was a swelling at the right metatarsal region and the hock joint with enlargement of regional lymph nodes. Radiographical evaluation revealed lysis of the tarsal bone; cytology demonstrated cryptococcal intralesional organisms at the swollen joint. Despite empirical antifungals therapeutic, the animal developed neurological cryptococcosis and died spontaneous

Leksell G frame in pediatric neurosurgery: experiences from 73 stereotactic procedures.

OBJECTIVE: Pediatric neurosurgery sets particularly high standards for indications, technique, and the risk profile of surgical procedures. These standards include precise targeting procedures due to the complex anatomy of the developing brain, especially in conditions such as epilepsy, brain tumors, hydrocephalus, and cystic lesions. The Leksell G frame, a stereotactic device designed for high-accuracy localization, has been utilized across various neurosurgical procedures in both adults and children. This study aimed to evaluate the feasibility, safety, and outcomes of stereotactic procedure
