Neurophysiological Approach by Self-Control of Your Stress-Related Autonomic Nervous System with Depression, Stress and Anxiety Patients.

BACKGROUND: Heart Rate Variability Biofeedback (HRVB) is a treatment in which patients learn self-regulation of a physiological dysregulated vagal nerve function. While the therapeutic approach of HRVB is promising for a variety of disorders, it has not yet been regularly offered in a mental health treatment setting. AIM: To provide a systematic review about the efficacy of HRV-Biofeedback in treatment of anxiety, depression, and stress related disorders. METHOD: Systematic review in PubMed and Web of Science in 2020 with terms HRV, biofeedback, Post-Traumatic Stress Disorder (PTSD), depress

Longitudinal proneuroactive and neuroactive steroid profiles in medication-free women with, without and at-risk for perinatal depression: A liquid chromatography-tandem mass spectrometry analysis.

BACKGROUND: Neuroactive steroids (NAS) are derivatives of cholesterol or steroidal precursors made in the gonads, adrenal gland, placenta and brain. We characterized longitudinal plasma proneuroactive and NAS in healthy perinatal comparison women (HPCW), women at-risk for perinatal depression (AR-PND), and women with PND with/without comorbid anxiety. We hypothesized that AR-PND women who either did or did not go on to develop PND would have elevated NAS concentrations as compared to HPCW and that NAS would be correlated to depressive and anxiety symptoms. METHODS: A prospective cohort study

Perinatal depression during armed conflict in Israel: Mental health risks and challenges, a cross-sectional study.

BACKGROUND: The recent armed conflict between Israel and Hamas (October 7,2023) has impacted various populations, including perinatal women, whose mental health can be particularly vulnerable to deterioration during periods of conflict. This study aimed to assess the prevalence of perinatal depression symptoms (PNDs) during the war and identified related risk factors for PNDs. METHODS: This cross-sectional study included 862 women who were pregnant or up to 12 months postpartum. Participants filled out an online self-report questionnaire. PNDs were measured using the Edinburgh Postnatal Depre

Variation of Behavioral Health Care by Behavioral Health Symptom Profile Among a Diverse Group of Pregnant and Parenting Mothers.

This study examines the association between behavioral health symptoms and use of behavioral health care (BHC; i.e., past year counseling and/or regular use of psychiatric medication) among a diverse group of mothers of toddlers. Data were from the Fragile Families and Child Wellbeing study (N = 4205 mothers). The association between symptom profiles (i.e., depressive, anxiety, and alcohol and drug dependence) and use of BHC was estimated with logistic regression models. Potential moderation by race/ethnicity was examined. Complex symptom profiles, older age, functional limitations, prior beha

Effects of a Prenatal Anxiety Randomized Controlled Trial Intervention on Infant Development in Pakistan.

OBJECTIVE: Given that infant development is influenced by caregiver mental health, we tested whether an intervention to reduce antenatal anxiety could affect infant development. A secondary aim was to test depressive symptoms, maternal responsiveness, and maternal infant bonding as mediators of this relationship. METHODS: Between 2020 and 2022, pregnant women participated in a randomized controlled trial of the Happy Mother-Healthy Baby (HMHB) program based on cognitive behavioral therapy. We collected data on child development from 202 intervention and 198 control participants in a public ho

Universal prevention of distress aimed at pregnant women: a systematic review and meta-analysis of psychological interventions.

BACKGROUND: There is sufficient meta-analytic evidence that antenatal interventions for women at risk (selective prevention) or for women with severe psychological symptoms (indicated prevention) are effective in reducing postpartum distress. However, women without risk or severe psychological symptoms might also experience distress. This meta-analysis focused on the effectiveness of preventive psychological interventions offered to universal populations of pregnant women on symptoms of depression, anxiety, and general stress. Paternal and infant outcomes were also included. METHOD: We includ

Intravenous dexmedetomidine use in obstetric anesthesia: a focused review.

Dexmedetomidine is an alpha-2 adrenergic receptor agonist with analgesic properties. Dexmedetomidine is currently U.S. Food and Drug Administration (FDA) approved for intravenous (IV) administration in non-pregnant patients. However, it has shown promise for various off-label indications in obstetric anesthesia. This review focuses on reported uses for IV dexmedetomidine in obstetric anesthesia. Intravenous dexmedetomidine has reported efficacy for producing light sedation, analgesia, and anxiolysis in the parturient. In addition, the use of IV dexmedetomidine during cesarean delivery has been

[Post-partum: Guidelines for clinical practice--Short text].

OBJECTIVE: To determine the post-partum management of women and their newborn whatever the mode of delivery. MATERIAL AND METHODS: The PubMed database, the Cochrane Library and the recommendations from the French and foreign obstetrical societies or colleges have been consulted. RESULTS: Because breastfeeding is associated with a decrease in neonatal morbidity (lower frequency of cardiovascular diseases, infectious, atopic or infantile obesity) (EL2) and an improvement in the cognitive development of children (EL2), exclusive and extended breastfeeding is recommended (grade B) between 4 to 6

Association of Maternal Depressive Symptoms During the Perinatal Period With Oppositional Defiant Disorder in Children and Adolescents.

IMPORTANCE: An association between perinatal maternal depression and risk of oppositional defiant disorder (ODD) in offspring has not been established. Identifying early determinants of ODD can help inform preventative intervention efforts. OBJECTIVE: To investigate the association between maternal perinatal depressive symptoms and the risk of ODD in offspring aged 7 to 15 years. DESIGN, SETTING, AND PARTICIPANTS: This population-based longitudinal birth cohort study used data from the Avon Longitudinal Study of Parents and Children (ALSPAC), in Bristol, UK. Maternal depressive symptoms and children's receipt of health care in the first 3 years of life.

BACKGROUND: Maternal depression is widely recognized to negatively influence mother-child interactions and children's behavior and development, but little is known about its relation to children's receipt of health care. OBJECTIVE: To determine if maternal depressive symptoms reported at 2 to 4 and 30 to 33 months postpartum are associated with children's receipt of acute and preventive health care services in the first 30 months. DESIGN: Cohort study of data collected prospectively as part of the National Evaluation of Healthy Steps for Young Children (HS). Data sources included medical rec

Mental Health and Maternal Mortality-When New Life Doesn't Bring Joy.

OBJECTIVE: To characterize the incidence and risk factors associated with maternal suicide during the peripartum period in an Alberta population. Our secondary objective was to characterize the incidence and risk factors associated with traumatic death in this same population. METHODS: This is a retrospective cohort study compared all-cause mortality with death by trauma (suicide, homicide, MVA, drug toxicity) using data collected by the Alberta Perinatal Health Program from 1998 to 2015. Data were summarized using descriptive statistics. The maternal mortality rate was calculated, and χ2 tes

Factors influencing maternal distress among Dutch women with a healthy pregnancy.

BACKGROUND: Maternal distress is a public health concern. Assessment of emotional wellbeing is not integrated in Dutch antenatal care. Midwives need to understand the influencing factors in order to identify women who are more vulnerable to experience maternal distress. OBJECTIVE: To examine levels of maternal distress during pregnancy and to determine the relationship between maternal distress and aetiological factors. METHODS: A cross-sectional study including 458 Dutch-speaking women with uncomplicated pregnancies during all trimesters of pregnancy. Data were collected with questionnaires

The impact of mental health disorders in pregnancy on obstetric and neonatal outcomes: A narrative review.

Perinatal psychiatry has gained increasing attention as a public health priority, given the high prevalence and burden of mental health conditions during pregnancy and the postpartum period. While research has predominantly focused on depressive disorders, growing evidence suggests that a broader range of psychiatric conditions may significantly influence obstetric and neonatal outcomes. Unlike previous works primarily centered on perinatal depression, this narrative review provides an integrated and transdiagnostic overview of perinatal psychiatric disorders - including mood, anxiety, psychot

Challenges in Cerebral Venous Thrombosis Management-Case Reports and Short Literature Review.

Cerebral venous thrombosis (CVT) is a rare type of stroke, with a complex clinical presentation that can make it a diagnostic challenge for the swift initiation of anticoagulation. When a hemorrhagic transformation is added, therapeutic management becomes even more complex. We describe a series of four cases, aged between 23 and 37 years old, with cerebral venous thrombosis. They were admitted to our clinic between 2014 and 2022. All cases presented significant challenges in either diagnostic, therapeutic or etiologic evaluation, at different stages of the disease. Late complications such as e

[Apgar status, blood gases and acid base balance of neonates after caesarean sections, using either thiopentone or ketamine for induction of anaesthesia (author's transl)].

Apgar status and acid base balance of 206 neonates, delivered by caesarean section under general anaesthesia, were investigated in order to compare the possible effects of either thiopentone- or ketamine-induction on the postpartum adaption. Several other criteria were recorded also, for instance, a possible neonatal asphyxia, the induction-delivery-interval, the maternal age, the administration of other than anaesthetic drugs etc. There were not correlations between the Apgar status and the induction-delivery interval in either groups. The number of neonates within the 3 Apgar-classes, and th

The quality of early relational health modifies the effect of early life stress on child emerging psychopathology.

BACKGROUND: Early relational health (ERH) is thought to buffer the association between early life stress (ELS) and child psychopathology, but limited work has directly tested this hypothesis. OBJECTIVE: We evaluate mother-infant emotional connection, a facet of ERH, as a buffer of combined and individual impacts of specific ELS exposures (maternal mental health and interpartner conflict) on child psychopathology. METHODS: Participants included mother-infant dyads (n=100) followed longitudinally in the COMBO cohort, a convenience sample recruited during the COVID-19 pandemic. ERH was assessed

Recommendations for Lifestyle Physical Activity and Exercise During the Perinatal Period: A Narrative Review.

BACKGROUND: The World Health Organization (WHO) has recently focused much attention on physical activity recommendations. Regular physical activity offers broad health benefits, reducing the risk of some chronic diseases and improving bone structure and muscle strength. Although the scientific literature provides numerous recommendations for physical activity during pregnancy and the postpartum period, there are no official recommendations for lifestyle-related physical activity. OBJECTIVES: This narrative review aimed to review the current knowledge on physical activity during pregnancy and

The impact of cognitive behavioral group therapy for anxiety on GAD related sleep disturbance during the perinatal period.

BACKGROUND: Generalized anxiety disorder (GAD) is highly prevalent during the perinatal (pregnancy and postpartum) period. Sleep disturbance (SD), a core symptom of GAD, and a common difficulty during the perinatal period, has been overlooked as a specific treatment target This study examined (1) the efficacy of cognitive behavioral group therapy for perinatal anxiety (CBT-PA) in reducing SD among individuals with GAD, and (2) whether SD functions as a mechanism of treatment change. METHODS: Perinatal participants (pregnant or ≤12 months postpartum) with GAD enrolled in a randomized trial (n
