PURPOSE: The mechanisms of liver fibrosis in biliary atresia (BA) after a Kasai operation deserve studying to improve the clinical outcomes. This study aimed to create a rat model simulating BA after a Kasai operation.
METHODS: We inserted a polyethylene tube (PE10) into the common hepatic duct (CHD) and ligated the common bile duct (CBD) in 30 newborn rats and injected 95% ethanol into IHD at postoperative week-one (POW-1). The PE10 was removed at POW-3. The rats were sacrificed at POW-5. The CBD cystojejunostomy was performed on another 10 rats at POW-5.
RESULTS: The IHD obliteration and C

A 61-year-old man with AIDS on chronic highly active antiretroviral treatment (HAART) presented with lethargy and jaundice and was found to have abnormal liver function tests (LFTs). Investigations including viral/autoimmune markers and imaging were unrevealing, except for positive Epstein-Barr virus. HAART was held, however, transaminases and total bilirubin continued to rise. The liver biopsy revealed classical Hodgkin's lymphoma (HL). HL presenting only with liver findings without lymphadenopathy is rare. Extreme cases can lead to fulminant liver failure. The bone marrow biopsy and dramatic

THE HEMOLYTIC ANEMIAS OF UNKNOWN CAUSE CAN BE SEPARATED INTO TWO MAIN GROUPS: (1) those produced by a defect in cell structure, which is usually hereditary, and (2) those due to a hemolysin of immune-body type. The hemolytic anemias associated with hypersensitivity to drugs and disease processes such as leukemia are less well understood and need further investigation. Splenectomy is the only effective treatment in congenital hemolytic jaundice and in acquired hemolytic anemia; the operation should be carried out promptly in most cases. Transfusion may be used in all varieties of hemolytic disea

BACKGROUND: To analyze the clinical efficacy and surgical experience of robot-assisted radical resection for choledochal cysts in infants under three months of age.
METHODS: This was a multicenter retrospective study conducted at the Department of Pediatric Surgery, Mianyang Central Hospital, and Chengdu Women's and Children's Central Hospital. A total of 31 patients diagnosed with congenital choledochal cysts under three months of age between May 2021 and May 2026 were included. The cohort comprised 9 boys and 22 girls, aged 10-86 days (median age: 52 days), with a body weight ranging from 3

For patients with high-level malignant biliary obstruction-confirmed following initial percutaneous transhepatic cholangiography drainage (PTCD) performed for preoperative bilirubin reduction and subsequently determined to be unresectable-biliary stent placement via the existing PTCD tract represents a straightforward, minimally invasive, and clinically feasible therapeutic strategy. However, the procedure may be technically challenging if the catheter tract is tortuous. This case report describes a refined interventional technique to overcome such limitations. A 56-year-old man was admitted d

BACKGROUND: Colorectal cancer remains as one of the most common cancers that are diagnosed and remains as a significant contributor to morbidity and mortality. Despite advances in techniques, improving access to diagnostic modalities and increasing awareness, it often presents at a later stage and can recur despite treatment. Recurrence can be variable and can occur years after treatment. Liver is the most common location for metastasis to occur followed by lungs. However, atypical sites of metastasis can occur although unusual and colorectal cancer can spread to the spleen, hilum of the liver

BACKGROUND: Endoscopic biliary stenting is the standard palliative approach for biliary obstruction in unresectable periampullary cancer (PAC). However, stent dysfunction and complications can impact treatment outcomes. This study aimed to evaluate the influence of surgical (laparotomy) versus endoscopic approaches on the cost-effectiveness of palliative treatment for biliary obstruction in PAC using real-world data.
METHODS: Data from consecutive patients with malignant obstructive jaundice undergoing either endoscopic biliary stenting (plastic and/or metal stents) or surgical biliary-digest

The sera of virtually all patients with primary biliary cirrhosis contained non-organ specific antibodies, probably directed against mitochondria, which give rise to cytoplasmic `M' immunofluorescence in unfixed sections from various organs in high titres. They were also found in 31% of patients with cryptogenic cirrhosis particularly those with obstructive features, and 28% of cases of active chronic (lupoid) hepatitis. In contrast only two patients out of twenty-eight with extrahepatic biliary obstruction and one patient out of twenty-five with infective hepatitis gave positive reactions and

BACKGROUND/AIMS: In all patients with pancreatic and gastric cancer we always make a laparoscopic exploration to complete the staging. Lately we have adopted the following technique for nonresectable cancers of the head of the pancreas: following endoscopic retrograde cholangiography we position a biliary stent to restore bile flow and obtain regression of jaundice, a laparoscopic-assisted gastroentero-anastomosis (GEA) is then performed as an antecolic isoperistaltic side-to-side gastrojejunostomy. Also in case of nonresectable gastric cancer we perform a laparoscopic-assisted gastrojejunosto

BACKGROUND: Diagnosis of Budd-Chiari syndrome (BCS) is often missed unless its possibility has been kept in mind. Obstruction of inferior vena cava (IVC) is reportedly the most frequent cause of BCS in Afro-Asian variety.
AIM: An attempt was made to classify BCS (in an eastern Indian population) etiopathologically.
PATIENTS AND METHODS: Thirty consecutive cases of BCS presenting over a period of five years were included. Following a thorough physical examination, necessary investigations (including coagulation profile, ultrasonography (with Doppler study) of hepatobiliary tract, hepatic vein

RATIONALE: Wernicke encephalopathy (WE) results from thiamine deficiency and frequently affects nonalcoholic surgical patients with prolonged fasting and parenteral nutrition. Perioperative WE is often misdiagnosed due to nonspecific symptoms, and its intraoperative electroencephalographic and anesthetic features remain poorly understood.
PATIENT CONCERNS: A 68-year-old male underwent hepatic surgery for malignant obstructive jaundice. Postoperative biliary anastomotic leakage and aphagia led to long-term parenteral nutrition. The patient subsequently developed confusion, somnolence, nystagmu

Unresectable locally advanced pancreatic ductal adenocarcinoma (PDAC) has a poor prognosis, particularly after distant metastasis. Irreversible electroporation (IRE) is a nonthermal ablative technique used for pancreatic tumors adjacent to major vessels, but evidence on late repeat pancreatic IRE remains limited. We report a 62-year-old man who presented with obstructive jaundice. Multidisciplinary evaluation classified the tumor as unresectable locally advanced PDAC involving the superior mesenteric vein-portal vein confluence. In January 2021, he underwent pancreatic IRE with biliary bypass,
