Clinical manifestations and proportions of severe outcomes in pediatric acute post-streptococcal glomerulonephritis: a systematic review and meta-analysis of predominantly hospital-based cohorts.

OBJECTIVE: To systematically evaluate the proportions of major clinical manifestations and severe outcomes in children with acute post-streptococcal glomerulonephritis (APSGN). METHODS: PubMed, Embase, Web of Science, the Cochrane Library, Scopus, and ClinicalTrials.gov were systematically searched for observational studies reporting clinical manifestations or outcomes of APSGN in children. Outcomes of interest included hypertension, gross hematuria, study-defined AKI/acute kidney dysfunction, nephrotic-range proteinuria, severe complications, an exploratory RPGN/crescentic-lesion composite (

Wound complications and clinical efficacy of the direct anterior approach versus the posterolateral approach in total hip arthroplasty: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: The direct anterior approach (DAA) and the posterolateral approach (PLA) are commonly used surgical techniques in total hip arthroplasty (THA). However, whether DAA is associated with different wound complication rates compared with PLA remains controversial. This meta-analysis aimed to compare wound complications and clinical outcomes between DAA and PLA in THA based on randomized controlled trials. METHODS: A comprehensive search was conducted across six major electronic databases: PubMed, Embase, Cochrane Library, Web of Science, China National Knowledge Infrastructure (CNKI),

Pathophysiology, prevention and treatment of postoperative peritoneal adhesions: a WSES narrative.

BACKGROUND: Adhesions are a common complication following surgery, and they can cause significant morbidity. However, adhesions are not easily diagnosed with imaging, and often only become apparent when they cause intestinal obstruction symptoms. The pathophysiology of adhesion formation is complex, thus despite advances in surgical techniques and postoperative care, adhesions remain a persistent problem in clinical practice. Despite evidence for efficacy of some strategies in reducing adhesion formation question remain regarding the indications and impact on clinically relevant outcomes. The

Remote blood pressure monitoring in women at risk of or with hypertensive disorders of pregnancy: A systematic review and meta-analysis.

BACKGROUND: Remote blood pressure monitoring refers to an organized framework that either allows clinicians to review home-based blood pressure readings and institute management, or provide participants with clear instructions for contacting care teams when blood pressure readings are out of prespecified targets. With widespread uptake of telemonitoring and mobile health in recent years, such models of care have been increasingly described in the literature. OBJECTIVES: This study aimed to review remote blood pressure monitoring in pregnant and postpartum women who are at high-risk for or hav

AVURT: aspirin versus placebo for the treatment of venous leg ulcers - a Phase II pilot randomised controlled trial.

BACKGROUND: Venous leg ulcers (VLUs) are the most common cause of leg ulceration, affecting 1 in 100 adults. VLUs may take many months to heal (25% fail to heal). Estimated prevalence is between 1% and 3% of the elderly population. Compression is the mainstay of treatment and few additional therapies exist to improve healing. Two previous trials have indicated that low-dose aspirin, as an adjunct to standard care, may improve healing time, but these trials were insufficiently robust. Aspirin is an inexpensive, widely used medication but its safety and efficacy in the treatment of VLUs remains

Uterine artery embolization for symptomatic uterine fibroids.

BACKGROUND: Uterine fibroids cause heavy prolonged bleeding, pain, pressure symptoms and subfertility. The traditional method of treatment has been surgery as medical therapies have not proven effective. Uterine artery embolization (UAE) has been reported to be an effective and safe alternative to treat fibroids in women not desiring future fertility. There is a significant body of evidence based on case controlled studies and case reports. This is an update of the review previously published in 2006. OBJECTIVES: To review the benefits and risks of uterine artery embolization (UAE) versus oth

Early developmental intervention programs post hospital discharge to prevent motor and cognitive impairments in preterm infants.

BACKGROUND: Infants born preterm are at increased risk of developing cognitive and motor impairments compared with infants born at term. Early developmental interventions have been used in the clinical setting with the aim of improving the overall functional outcome for these infants. However, the benefit of these programs remains unclear. OBJECTIVES: To review the effectiveness of early developmental intervention post-discharge from hospital for preterm (< 37 weeks) infants on motor or cognitive development. SEARCH STRATEGY: The Cochrane Neonatal Review group search strategy was used to ide

Antibiotic therapy for prophylaxis against infection of pancreatic necrosis in acute pancreatitis.

BACKGROUND: Acute pancreatitis is a common acute abdominal emergency which lacks specific therapy. In severe attacks, areas of the pancreas may become necrotic. The mortality risk rises to >40% if sterile necrosis becomes superinfected, usually with gut derived aerobic organisms. Experimental and clinical studies indicate a window of opportunity of 1-2 weeks, when superinfection, and thus high-risk surgical debridement, may be prevented by administering systemic antibiotics to 'sterilise' tissues adjacent to necrotic areas. There are theoretical risks of encouraging antibacterial resistance an

Robot-Assisted Simple Prostatectomy Versus Endoscopic Enucleation for Large-Volume Benign Prostatic Hyperplasia: A Systematic Review and Meta-Analysis of Perioperative Outcomes and Complications.

Introduction/Background: Large-volume benign prostatic hyperplasia (≥80 mL) requires complete anatomical removal of the adenoma. Robot-assisted or minimally invasive simple prostatectomy (RASP/MISP) and anatomic endoscopic enucleation of the prostate (AEEP) have both emerged as minimally invasive alternatives to open simple prostatectomy, but their comparative perioperative performance in very large glands remains incompletely defined. Objective: To systematically review and quantitatively synthesize the perioperative outcomes and complications of RASP/MISP versus AEEP in the surgical manageme

Comparative outcomes of robot-assisted and open radical nephrectomy with inferior vena cava thrombectomy for renal cell carcinoma: a systematic review and meta-analysis.

For patients with renal cell carcinoma (RCC) complicated by inferior vena cava (IVC) tumor thrombus, radical nephrectomy combined with thrombectomy remains a key surgical option. Open radical nephrectomy with IVC thrombectomy (O-RNTT) has long been regarded as the traditional operative strategy, whereas robot-assisted radical nephrectomy with IVC thrombectomy (R-RNTT) is now being used more frequently in high-experience centers. Direct comparative data, however, remain limited. This study aimed to compare perioperative outcomes and available short-term pathological and oncologic findings betwe

A prognostic model, including quantitative fetal fibronectin, to predict preterm labour: the QUIDS meta-analysis and prospective cohort study.

BACKGROUND: The diagnosis of preterm labour is challenging. False-positive diagnoses are common and result in unnecessary, potentially harmful treatments (e.g. tocolytics, antenatal corticosteroids and magnesium sulphate) and costly hospital admissions. Measurement of fetal fibronectin in vaginal fluid is a biochemical test that can indicate impending preterm birth. OBJECTIVES: To develop an externally validated prognostic model using quantitative fetal fibronectin concentration, in combination with clinical risk factors, for the prediction of spontaneous preterm birth and to assess its cost-

Supplemental oxygen for the treatment of prethreshold retinopathy of prematurity.

BACKGROUND: Oxygen has long been implicated in the pathogenesis of retinopathy of prematurity (ROP) and is rigorously monitored in today's neonatal intensive care units. Recent research using a feline model has shown an improvement in ROP outcome of kittens treated with supplemental oxygen. Current treatment for ROP by retinal ablation is not without complications so a non-invasive method of treatment is preferred. The possible effects of long term oxygen supplementation on chronic lung disease, length of hospital stay and growth and development are, however, unknown. OBJECTIVES: To determine

Metalloporphyrins for treatment of unconjugated hyperbilirubinemia in neonates.

BACKGROUND: Metalloporphyrins are heme analogues that inhibit heme oxygenase, the rate-limiting enzyme in the catabolism of heme to bilirubin. By preventing the formation of bilirubin, they have the potential to reduce the level of unconjugated bilirubin in neonates and thereby reduce the risk of neonatal encephalopathy and long term neurodevelopmental impairment from bilirubin toxicity to the nervous system. OBJECTIVES: 1. To determine the efficacy of metalloporphyrins in reducing bilirubin levels, reducing the need for phototherapy or exchange transfusion and reducing the incidence of bilir

Preoperative Neutrophil-to-Lymphocyte Ratio is Associated with Early Postoperative Pain and Opioid Consumption after Total Knee Arthroplasty: A Retrospective Study.

BACKGROUND: Improving quality and effectiveness of health care is one of the priorities of health policies. Hospital or physician volume represents a measurable variable with a relevant impact on effectiveness of health care. A recent Italian law, the "spending review", calls for the definition of "qualitative, structural, technological and quantitative standards of hospital care". There is a need for an accurate evaluation of the available scientific evidence in order to identify these standards, including the volume of care above or below which the public and private hospitals may be accredi

[Volume and health outcomes: an overview of systematic reviews].

BACKGROUND: Improving quality and effectiveness of health care is one of the priorities of health policies. Hospital or physician volume of activity may be a measurable variable with a relevant impact on effectiveness of health care. There are several studies and systematic reviews evaluating the association between volume and outcome of health care. The aim of this review is to identify: areas, clinical conditions or interventions (prevention, diagnostic, therapeutic, surgical or clinical) for which an association between volume and outcome has been investigated; those for which an associatio

Telehealth interventions: remote monitoring and consultations for people with chronic obstructive pulmonary disease (COPD).

BACKGROUND: Chronic obstructive pulmonary disease (COPD, including bronchitis and emphysema) is a chronic condition causing shortness of breath, cough, and exacerbations leading to poor health outcomes. Face-to-face visits with health professionals can be hindered by severity of COPD or frailty, and by people living at a distance from their healthcare provider and having limited access to services. Telehealth technologies aimed at providing health care remotely through monitoring and consultations could help to improve health outcomes of people with COPD. OBJECTIVES: To assess the effectivene

Robotic, laparoscopic, and open surgery for perihilar cholangiocarcinoma: a systematic review and direct pairwise meta-analysis.

BACKGROUND: Radical resection remains the cornerstone of curative-intent treatment for perihilar cholangiocarcinoma (pCCA). With the evolution of minimally invasive approaches, laparoscopic surgery (LS) and robot-assisted surgery (RAS) have been increasingly adopted in its surgical management. However, high-quality comparative evidence evaluating these approaches against conventional open surgery (OS) remains limited, and direct comparisons between LS and RAS are scarce. This systematic review and meta-analysis aimed to synthesize the available direct comparative evidence on RAS, LS, and OS fo

High versus standard volume enteral feeds to promote growth in preterm or low birth weight infants.

BACKGROUND: Human milk is the best enteral nutrition for preterm infants. However, human milk, given at standard recommended volumes, is not adequate to meet the protein, energy, and other nutrient requirements of preterm or low birth weight infants. One strategy that may be used to address the potential nutrient deficits is to give a higher volume of enteral feeds. High volume feeds may improve nutrient accretion and growth, and in turn may improve neurodevelopmental outcomes. However, there are concerns that high volume feeds may cause feed intolerance, necrotising enterocolitis, or complica
