Effect of prophylactic ondansetron on postoperative nausea and vomiting in patients on preoperative steroids undergoing craniotomy for supratentorial tumors.

The exact incidence of postoperative nausea and vomiting (PONV) in patients on steroids undergoing neurosurgical procedures is not known. This prospective randomized double-blind study was planned to know the efficacy of prophylactic ondansetron in the prevention of PONV in patients on steroids as compared with placebo. Seventy adult patients of either sex who had received preoperative steroids (dexamethasone) for at least 24 hours and were scheduled to undergo craniotomy for supratentorial tumors were included. Patients were randomly allocated using a randomization chart to 1 of the 2 groups

Parabolic flight reveals independent binocular control of otolith-induced eye torsion.

To examine otolith-governed ocular torsion in hyper- and hypogravity, eight subjects, including two astronauts, underwent parabolic flight while seated upright with head fixed. A mask fitted with two video cameras provided synchronized images of both eyes at a rate of 25/sec during 15 parabolas, the individual parabolas separated by a few minutes of level 1 G flight. Three main findings emerged: 1) After the first parabola, most subjects showed differential torsional offset of the two eyes in the 1 G portions between parabolas, compared to the conjugate baseline position of the eyes prior to t

The electrocardiographic response to high +Gz centrifuge training.

The electrocardiographic (ECG) responses of 59 asymptomatic, healthy flight surgeons to the acceleration profiles included in current U.S. Air Force and U.S. Navy high-G centrifuge training programs are documented. ECG dysrhythmias were frequently observed during exposure to both gradual and rapid onset training profiles. Short self-limited episodes of ventricular tachycardia occurred in 5 subjects. Advanced Lown grade ventricular ectopy occurred in 13 subjects. The type of cardiac ectopy and the frequency of occurrence for each of the training profiles is described. The results suggest that s

Modeling postural instability with Galvanic vestibular stimulation.

In this study the effect of a pseudorandom binaural bipolar Galvanic stimulus generated by a sum of nonharmonically related sine waves on postural control was functionally assessed using computerized dynamic posturography (CDP), and the results compared to vestibulopathic patient populations and astronauts exposed to microgravity. The standardized CDP test battery comprised six sensory organization tests (SOTs) that combined three visual conditions (eyes open, eyes closed, and sway-referenced vision) with two proprioceptive conditions (fixed and sway-referenced support surfaces). Subjects (12)

The effect of microgravity and space flight on the chemical senses.

The effect of space flight and microgravity on the chemical senses is reviewed. Skylab-4 and Soyuz 30-31 studies revealed changes in taste thresholds while no effect was found in a Canadian investigation (41-G) and conflicting results were obtained on another Soyuz mission. Two simulated microgravity studies found no effect on taste or smell sensitivity; while 5 other studies found an effect. Microgravity induces physiological changes including an upward shift of body fluids toward the head, which may lead to an attenuation of the olfactory component in the flavor of foods. Chemosensory change

Perception of tilt (somatogravic illusion) in response to sustained linear acceleration during space flight.

During the 1998 Neurolab mission (STS-90), four astronauts were exposed to interaural and head vertical (dorsoventral) linear accelerations of 0.5 g and 1 g during constant velocity rotation on a centrifuge, both on Earth and during orbital space flight. Subjects were oriented either left-ear-out or right-ear-out (Gy centrifugation), or lay supine along the centrifuge arm with their head off-axis (Gz centrifugation). Pre-flight centrifugation, producing linear accelerations of 0.5 g and 1 g along the Gy (interaural) axis, induced illusions of roll-tilt of 20 degrees and 34 degrees for gravito-

Neurobiological problems in long-term deep space flights.

Future missions in space may involve long-term travel beyond the magnetic field of the Earth, subjecting astronauts to radiation hazards posed by solar flares and galactic cosmic rays, altered gravitation fields and physiological stress. Thus, it is critical to determine if there will be any reversible or irreversible, detrimental neurological effects from this prolonged exposure to space. A question of particular importance focuses on the long-term effects of the space environment on the central nervous system (CNS) neuroplasticity, with the potential acute and/or delayed effects that such pe

Sympathetic nerve response to muscle during anteroposterior acceleration in humans.

This study aimed to elucidate the effects of linear acceleration on muscle sympathetic nerve activity (MSNA) in humans. Eight healthy young male volunteers were seated in a linear accelerator (sled) during the recording of their electrocardiogram, blood pressure with the Finapres, thoracic impedance and respiration curve. MSNA was recorded from the tibial nerve by microneurography. At a fixed distance of sled movements in an anteroposterior direction, eight modes of stimulation with peak accelerations at 0.05, 0.10, 0.15, and 0.20 G (gravity) in sinusoidal or step mode were applied to each sub

The intracellular histamine antagonist, N,N-diethyl-2-[4-(phenylmethyl)phenoxy] ethamine.HCL, may potentiate doxorubicin in the treatment of metastatic breast cancer: Results of a pilot study.

N,N-diethyl-2-[4-(phenylmethyl)phenoxy] ethanamine.HCl (DPPE) is a diphenylmethane analog of tamoxifen that antagonizes the intracellular binding of histamine to growth-regulatory sites, a proportion of which represents P450 enzymes, in microsomes and nuclei. We previously reported increased response rates and decreased myelotoxicity in patients with prostate and other cancers who received an intensive dose/schedule of DPPE plus single-agent chemotherapy. We now report the results of a study of DPPE combined with a standard dose/schedule of doxorubicin in twenty-three patients with metastatic

The effect of CP-99994 on the responses to provocative motion in the cat.

1. The NK1 receptor antagonist CP-99994 has been shown to prevent vomiting elicited by both peripherally and centrally acting emetogens in ferrets and dogs. These results have now been extended to another stimulus, provocative motion, and another species, the cat. 2. CP-99994 displaced [3H]-substance P from cat cortex with IC50 of 0.52 +/- 0.08 nM. Following s.c. administration, peak plasma drug levels were achieved at 30 min. The plasma drug half life was 1.4 h. 3. Subcutaneous administration of CP-99994 inhibited motion-induced vomiting in the cat with an ED50 of 144 micrograms kg-1 but did

Adaptation to telescopic spectacles: vestibulo-ocular reflex plasticity.

The vestibulo-ocular reflex (VOR) is a mechanism for the production of rapid compensatory eye movements during head movements. To investigate the adaptation of this reflex to spectacle magnifiers, the effect on the VOR of a brief period of wearing telescopic spectacles during head rotation was studied in normal subjects. VOR gain, as measured in darkness, was defined to be the ratio of compensatory slow phase eye velocity to head velocity. Initial VOR gain as measured for vertical axis sinusoidal head rotation at 0.1 Hz, amplitude 60 degrees/sec, was about 0.7. After 15 min adaptation by sinus

Sustaining humans in space.

One goal of NASA's Ames Research Center is to extend the presence of humans in space. Biomedical research, human performance, and life-support systems form the core of the Ames program. Major questions highlight how the effects of microgravity on living systems are modified by exercise, artificial gravity, autogenic feedback training, and nutrition. Bioengineering and life science studies include humans, animals, and plants. Ames investigators collaborate extensively with outside university and commercial scientists, both within the United States and internationally.
