3D-Printed Arch Supports Combined with Toe Spreaders Modulate Phase-Specific Ankle Alignment and Muscle Activity in Young Adults with Functional Flat Foot.

Background/Objectives: Functional flat foot (FF) is associated with altered lower limb biomechanics, leading to inefficient load transfer and potential overuse injuries. Customized orthoses, such as 3D-printed insoles and toe spreaders, may mitigate these deficits, but their combined biomechanical and neuromuscular effects remain unclear. The current study investigated the immediate effects of 3D-printed arch support insoles (SI) and toe spreaders (Toe) and their combination (SI+Toe) on gait pattern, center of force (COF), ankle alignment, and lower limb muscle activation in young adults with

Sinonasal cancer incidence in Lombardy, Italy, 2008-20.

BACKGROUND: Epithelial sinonasal cancers (SNC) are rare tumours with recognized associations with known/suspected occupational carcinogens (wood/leather dust, nickel/chromium compounds and formaldehyde). In Italy, a national SNC registry organized as a network of regional registries was established by law in 2008. AIMS: To describe SNC time trends, occupational exposures and geographical distribution in Lombardy, North-West Italy, based on population registry data (2008-20). METHODS: The Lombardy SNC Registry records epithelial SNCs using various sources. Interviews to collect occupational h

Early results of intramedullary nail fixation in distal tibia oblique osteotomy for the reduction of soft tissue complications.

PURPOSE: During distal tibial oblique osteotomy, external fixators can increase pin site infection risk, whereas plates can cause wound necrosis, necessitating a compromise between soft-tissue position and length. We provide the first report of the early results of intramedullary nail fixation in these osteotomies for avoiding soft tissue complications. METHODS: Ten ankles, classed as Takakura-Tanaka stages 3a to 4 and unclassified and treated via distal tibial oblique osteotomy for ankle osteoarthritis between 2017 and 2021, were included. Osteotomy was performed obliquely from the distal me

Satisfaction survey toward an innovative orthosis for children with cerebral palsy with pes planovalgus.

BACKGROUND: Pes planovalgus (PPV) is common in children with cerebral palsy and can cause pain and gait alterations over time. Initial treatment of flexible PPV includes orthotics, despite a lack of consensus on the type of orthosis. We developed an innovative ankle-foot orthosis (RAFO). RAFO is a one-piece orthosis designed to correct both the valgus and the flat of the foot. Its conception situated above the malleolar enables a moderate anti equinus effect. Its precise description and fabrication's process is detailed. OBJECTIVES: We wanted to assess user's satisfaction after several months

Anatomic Repair vs Closed Reduction of the Syndesmosis.

BACKGROUND: The goal of the study was to compare radiographic and functional outcomes between conventional closed syndesmotic reduction and screw fixation with open reduction, direct repair of the anterior inferior tibiofibular ligament (AiTFL) and screw fixation. We hypothesized that open reduction with restoration of the AiTFL would provide an improved reduction with better radiographic and functional outcomes. METHODS: Fifty consecutive patients with OTA 44-C ankle fractures were enrolled. Treatment was nonrandomized and based on surgeon preference. Patients were treated with either open r

Has the Threshold for Epiphysiodesis Versus Lengthening Changed in the Era of Magnetically Controlled Nails?

INTRODUCTION: Limb length discrepancy (LLD) is common in both the pediatric and adult population. Length inequalities can be due to a multitude of etiologies including congenital, developmental, and acquired causes. There has been little consensus on the morbidity of LLD and, as a result, the threshold necessary for treatment of LLD to prevent morbidity. Advances in magnetically controlled lengthening devices achieve greater accuracy and patient satisfaction and create an opportunity to lower the threshold for limb lengthening. DISCUSSION: Asymptomatic LLD is relatively common in both pediatr

[Fractures of the fifth metatarsal base].

Fractures of the fifth metatarsal base are among the most common fractures of the foot. They are typically caused by indirect violence during sports activities. This region is also a site where stress fractures occur frequently. Diagnosis is based on the patients medical history, clinical examination and imaging methods. The fundamental imaging method is radiography of the foot in three views. MRI is used primarily for early diagnosis of stress fractures. CT examination is beneficial mainly in complex foot injuries, in order to rule out associated fractures. Several classifications of fracture

Prevalence of acromegaly in patients with symptoms of sleep apnea.

Acromegaly is a rare disease with nonspecific symptoms with acral enlargement being almost universally present at diagnosis. The estimated prevalence is 40-125 cases/million but targeted universal screening studies have found a higher prevalence (about 10 fold). The aim of the ACROSAHS study was to investigate the prevalence of acromegaly and acromegaly comorbidities in patients with sleep apnea symptoms and acral enlargement. ACROSAHS was a Spanish prospective non-interventional epidemiological study in 13 Hospital sleep referral units. Facial and acral enlargement symptoms including: ring si

Central polydactyly of the foot: surgical management with plantar and dorsal advancement flaps.

BACKGROUND: Central polydactyly is the least common form of foot polydactyly, and the intercalary location of the duplicated ray makes the surgical exposure, excision, and closure more complex. For these reasons there is little consensus concerning the optimal technique for surgical management. METHODS: A retrospective case series of 22 patients with 27 feet with central polydactyly, treated surgically by the dorsal and plantar advancement flap technique, was performed. Change in width of the forefoot was measured from radiographs by the metatarsal gap ratio. Functional outcomes were assessed
