PURPOSE: The anterior segment findings of children with glaucoma following pediatric cataract extraction were studied to determine the mechanism of this complication.
METHODS: The clinical records of 65 children with glaucoma cared for by the author were studied. Patients with bilateral lensectomy followed by bilateral glaucoma, bilateral lensectomy followed by unilateral glaucoma, and unilateral lens surgery followed by glaucoma were identified and studied.

The early development of vision in an eye with a dislocated lens is often poor, leading to disappointing postoperative results due to amblyopia. A retrospective review of the best corrected visual acuity of 49 eyes of 27 children with ectopia lentis, who were rendered aphakic either spontaneously or by lensectomy, showed a gradual improvement. Nineteen eyes achieved an early postoperative acuity of 6/12 or better. Of the remaining 30 eyes, 21 were followed up for 1 or more years. During this period, an improvement of two or more Snellen visual acuity lines was noted in all but one of the eyes.

161 eyes with Nd:YAG laser posterior capsulotomy after in the bag IOL implantation 1-3 years before, were actively followed over a period of 24 +/- 6 months. No CME nor retinal detachment nor induced glaucoma was found. Therefore a YAG capsulotomy in eyes with completely in the bag implanted IOL seems to assure the same level of security than capsular polishing in eyes with sulcus implanted lenses.

Two hundred thirty-eight consecutive patients with pediatric cataracts were fitted with a variety of aphakic contact lenses after cataract extraction. Secondary implantation of flexible, open loop anterior chamber IOLs was performed on 20 aphakic eyes between January 1990 and May 1992. In 3 cases anterior vitrectomy was necessary. A retrospective chart review of 34 secondary or exchange posterior chamber lens implantations was performed. Overall 94% of the patients achieved a visual acuity of 20/40 or better at six to 12 months postoperative. A survey of rhegmatogenous retinal detachment during 1976 was conducted on the population of Iowa. The annual incidences (per 100,000 populations) of four types of detachment were determined: nontraumatic phakic (6.1), traumatic phakic (1.0), nontraumatic aphakic (4.9), and traumatic aphakic (0.4). The treatment of lens injury in children with irido-capsula supported intraocular lenses is described. A consecutive series of 73 children is analyzed as to the functional results and complications. Surgery and anaesthesia are discussed in detail. Guidelines are given for future straightforward management of eye injury in children, using this promising possibility of visual rehabilitation.

The intraocular lens offers an additional experimental parameter to be considered for the visual rehabilitation of children with traumatic or unilateral infantile cataracts. The disadvantages of aphakic spectacles or contact lens and amblyopia have committed many of these eyes to the status of visual cripples. In this series of 41 patients, with a maximum of 24 months follow-up, it has been concluded that the IOL may be successfully implanted.
