Augmented Reality and Gamification vs. Traditional Surgical Instrument Training on Knowledge and Skills of Medical Students: A 2×2 Factorial Randomized Study.

INTRODUCTION: Augmented reality (AR) and gamification have shown promise in medical education. This study aimed to examine their combined application in surgical instrument training for medical students. METHODS: This 2×2 factorial randomized assignment involved 164 operating room students from two universities. Participants were categorized by university and randomly allocated to four intervention groups: Group A (AR + game-based learning; n = 41), Group B (AR only; n = 41), Group C (game only; n = 41), or Group D (traditional instruction; n = 41). The 3-week intervention comprised one 30-mi

Social, environmental and policy contexts affecting the feasibility and acceptability of improving household flooring for better health in rural Kenya.

BACKGROUND: Homes with unimproved floors (earth, sand, or clay) are associated with increased risks of soil-transmitted parasites and enteric infections, leading to higher morbidity and reduced quality of life. This study explored the enablers and barriers to adopting improved flooring (sealed, washable, and durable) in three culturally diverse regions of Kenya - Narok, Bungoma, and Kwale - to mitigate disease burdens. METHODS: Between August 2021 and July 2022, we conducted 24 focus group discussions with homeowners, stratified by age, gender, and floor type. Additionally, 28 key informant i

Real-World Barriers to Molecular Biomarker Testing in Neuro-Oncology Clinical Trials in Resource-Limited Settings: A Scoping Review.

OBJECTIVES: Molecular biomarkers are crucial for the diagnosis, prognosis, and enrollment in biomarker-driven neuro-oncology clinical trials, but their inequitable availability in low- and middle-income countries (LMICs) hinders uptake of the 2021 WHO Classification of CNS tumours. This scoping review reports an analysis of existing evidence concerning challenges to molecular biomarker testing within the context of neuro-oncology clinical trial recruitment in resource-limited settings. METHOD: An Arksey and O'Malley-based scoping review was conducted from May to July 2026 across PubMed, Scopu

Enablers and barriers to implementing a digital logistics system: A qualitative evaluation of OpenLMIS for Nigeria's vaccine supply chain.

Logistics management information systems (LMIS) strengthen health commodity distribution and decision-making. Nigeria introduced OpenLMIS, an electronic LMIS, in August 2021 to track COVID-19 vaccines, and a routine immunisation (RI) module was deployed nationwide in 2023. This evaluation examined enablers and barriers to effective implementation and sustained use. A qualitative design combined desk reviews of LMIS documents, 42 key informants interviews, and 16 structured observational assessments with Cold Chain Officers. Data collection occurred in three states, the Federal Capital Territor

APEW-Fed: adaptive privacy-aware ensemble weighting for federated anomaly detection in enterprise resource planning systems.

Enterprise Resource Planning (ERP) systems are high-value targets for fraud owing to the sensitive financial and operational data they process. Anomaly detection in such systems faces a fundamental tension: centralising data for accurate detection conflicts with privacy regulations such as the European Union's General Data Protection Regulation (GDPR) and the United States' Sarbanes-Oxley Act (SOX). We present APEW-Fed, a federated ensemble framework that resolves this tension through three provably complementary mechanisms. First, Adaptive Privacy-Aware Ensemble Weighting (APEW) frames model

Assessing rheumatoid arthritis prevalence from 1990 to 2040: viewing global inequities through the lens of sociodemographic development.

OBJECTIVES: Rheumatoid arthritis (RA) imposes a substantial global health burden, contributing significantly to chronic disability and escalating healthcare costs. Accurate, long-term prevalence forecasts are essential to inform strategic planning for rheumatology services and health resource allocation. METHODS: Hence, we developed and calibrated a sex-specific illness-death model (IDM) at global, regional, and national scales using GBD data (1990-2021). To uncover underlying epidemiological patterns, we augmented our projections with a frontier analysis that evaluates the efficiency of RA b

Effectiveness of shortwave diathermy in patients with chronic low back pain: A study protocol for a randomised, single-blinded, multicentre clinical trial.

BACKGROUND: Chronic low back pain (CLBP) is a leading cause of disability worldwide and a major public health concern in low- and middle-income countries such as Bangladesh. Although international guidelines classify shortwave diathermy as a low-value treatment, it remains widely used in physiotherapy practice. Evidence on the effectiveness of continuous shortwave diathermy (CSWD) for CLBP is limited and inconsistent. This study aims to determine whether CSWD, when combined with standard physiotherapy, provides additional benefits compared with sham CSWD plus physiotherapy. METHODS: This mult

Intensive combined balneotherapy and aquatic exercise for knee osteoarthritis: short-term clinical and functional outcomes.

BACKGROUND: Knee osteoarthritis (OA) is a major cause of global disability, necessitating cost-effective interventions. While balneotherapy and acquatic exercise (AE) are established conservative treatments, evidence regarding their combined efficacy remains limited. This study evaluated the clinical impact of a combined program that include balneotherapy and AE in natural mineral waters - compared to balneotherapy alone in patients with mild-to-moderate knee OA in Health Resorts. Primary outcomes included pain intensity, joint range of motion (AROM/PROM), and functional indices [Western Ontar

Global Paid and Unpaid Productivity Losses Due to Gastrointestinal Cancer Mortality.

OBJECTIVES: Gastrointestinal (GI) cancers contribute substantially to premature mortality, yet their full economic burden, including unpaid work, remains underquantified. We aimed to estimate global productivity losses attributable to GI cancer mortality, considering paid and unpaid work. METHODS: Years of productive life lost (YPLL) were calculated using data from GLOBOCAN for GI cancer among working-age individuals (15-64 years) across 185 countries. Paid productivity losses were estimated using the human capital approach, while unpaid work was valued via the opportunity cost method. Countr

Optimizing short-term antibiotic treatment in patients with acute cholangitis: study protocol for an open-label randomized controlled trial (the BOLT-P3 trial).

BACKGROUND: Acute cholangitis (AC) frequently presents as a community-acquired infection and is associated with a high prevalence of antibiotic use among infectious diseases. The Tokyo Guidelines 2018 (TG18) recommend 4-7 days of antibiotic administration after biliary drainage. However, this recommendation lacks strong evidence of its effectiveness and is primarily based on heterogeneous clinical findings and expert opinions. Recent retrospective studies have advocated a shorter 1- to 3-day antibiotic course as effective for AC treatment, prompting the need to reassess the treatment duration
