Detection of a persistent foramen ovale using echocardiography.

Right-to-left shunts can result in decompression illness in divers and lead to a number of other conditions. Transthoracic echocardiography with intravenous injection of bubble contrast, when performed according to a well-tested protocol by trained personnel, enables the safe, simple, rapid and inexpensive detection of right-to-left shunts, the assessment of the size of the shunts and the differentiation of atrial shunts from pulmonary shunts. This article summarises the author's views on the techniques available and his preferred protocol for transthoracic echocardiography.

Safety of transport and hyperbaric oxygen treatment in critically-ill patients from Padua hospitals into a centrally-located, stand-alone hyperbaric facility.

INTRODUCTION: Some patients admitted to the intensive care unit (ICU) might require repetitive hyperbaric oxygen treatment (HBOT) while receiving critical care. In such cases, the presence of a hyperbaric chamber located inside or near an ICU is preferable; however, this set-up is not always possible. In Padua, the "Associazione Tecnici IPerbarici" hyperbaric centre is a stand-alone facility outside of a hospital. Despite this, selected ICU patients receive HBOT at this facility. METHODS: We retrospectively reviewed the medical records from 2003 to 2013 of 75 consecutive, critically-ill patie

Staffing and training issues in critical care hyperbaric medicine.

The integrated chain of treatment of the most severe clinical cases that require hyperbaric oxygen therapy (HBOT) assumes that intensive care is continued while inside the hyperbaric chamber. Such an approach needs to take into account all the risks associated with transportation of the critically ill patient from the ICU to the chamber and back, changing of ventilator circuits and intravascular lines, using different medical devices in a hyperbaric environment, advanced invasive physiological monitoring as well as medical procedures (infusions, drainage, etc) during long or frequently repeate

Correction and response to: Grommets in HBOT patients: GA vs. LA, unanswered questions.

We thank Gibbs and Commons for their interest in our paper. There is a key difference between the datasets for Commons et al and our study. Our data set, has grouped five years of data according to the calendar year. This is different from Commons et al's study population recruited between 01 June 2009 and 31 May 2010. We feel this may explain the difference of one case between the two papers in 2010. Our data collection used the standard clinic and operating theatre databases, and we were advised that there was no searchable clinical code for grommet procedures undertaken with local anaestheti

Commentary: Hyperbaric oxygen treatment for wounds - evidence and the Sword of Damocles.

Increased access to any treatment sensibly follows the clinical and cost benefit being established. For many treatments this requires multiple, high-quality clinical trials and supporting cost analysis. Cost analysis may be applied to a single treatment or used to compare two or more treatments. Clinical efficacy and cost benefit are best scrutinised and validated by publication in the peer-reviewed literature. True peer review is most effectively achieved 'after publication' by the wider scientific community, i.e., the journal readers. However, initially an editor, usually advised by referees

Retrospective review of grommet procedures under general versus local anaesthesia among patients undergoing hyperbaric oxygen therapy.

INTRODUCTION: One significant side effect of hyperbaric oxygen treatment (HBOT) is middle ear barotrauma (MEBT) may require tympanostomy tube (grommet) insertion by the Ear, Nose and Throat service. Where timely HBOT is needed, routine insertion of grommets under local anaesthesia (LA) is becoming common. AIMS: To investigate the differences between patients receiving HBOT and concurrent grommets under LA versus general anesthesia (GA) at The Townsville Hospital (TTH). METHODS: A retrospective chart analysis of patients receiving HBOT between 2008 and 2012 and requiring grommets was undertak

Hyperbaric oxygen treatment for unilateral testicular torsion in a child.

Torsion of the testis is a urological emergency most commonly occurring in adolescent boys. Hyperbaric oxygen treatment (HBOT) has been shown to alleviate reperfusion injury in experimental ischaemia of the testis. We report a 13-year-old boy who had prolonged right testicular ischaemia. Despite surgery, the colour of the testis remained poor. He underwent a post-operative course of 10 HBOT over 8 days, with restoration of blood flow on colour Doppler and reduction of oedema. At four-month followup, the testis appeared normal on ultrasonography. To the best of our knowledge, this is the first

Unestablished indications for hyperbaric oxygen therapy.

Unestablished indications are conditions in which systematic clinical use of hyperbaric oxygen treatment (HBOT) is not supported by adequate proof of benefit. HBOT is vulnerable to use in many such conditions for various reasons, perhaps the most important being that a placebo or participation effect may create an impression of efficacy. The systematic use of HBOT in unestablished indications raises ethical concerns about provision of misleading information, giving false hope, and taking payment for therapy of doubtful benefit. Any practice perceived as unethical or unscientific has the potent

Monitoring cardiac output during hyperbaric oxygen treatment of haemodynamically unstable patients.

INTRODUCTION: Patients suffering from necrotizing fasciitis (NF) are often haemodynamically unstable and require extended monitoring of cardiovascular parameters; yet this is limited during hyperbaric oxygen treatment (HBOT). We aimed to evaluate the use and safety of transoesophageal Doppler (TED) monitoring of cardiac output (CO) under hyperbaric conditions in haemodynamically unstable patients diagnosed with NF and sepsis or septic shock. METHODS: Cardiac output was measured prior to, during and after HBOT with the use of TED in seven consecutive patients diagnosed with NF and sepsis or se

Hyperbaric oxygen in the treatment of perichondritis of the pinna.

Perichondritis is a condition characterized by pain, swelling and purulent discharge from the external ear, which may progress to a deformity of the pinna. The presence of co-morbidities such as diabetes mellitus may aggravate the situation. The main aetiological agent is Pseudomonas aeruginosa and treatment consists of antibiotics combined with surgical drainage of the ear. We present the case of a diabetic patient with recurrent perichondritis of the pinna treated with hyperbaric oxygen therapy, with successful healing. Hyperbaric oxygen therapy has proved beneficial as adjunctive therapy of

Irukandji syndrome: a widely misunderstood and poorly researched tropical marine envenoming.

Irukandji syndrome is a poorly defined set of symptoms that occur after envenoming by certain species of jellyfish, primarily cubozoans or 'box jellyfish'. Envenomed victims can show symptoms ranging from headaches, severe pain, nausea and vomiting to pulmonary oedema, cardiac failure and severe hypertension resulting in death. Historically, this syndrome appears to have been misdiagnosed and reported cases are undoubtedly a significant underestimation of the prevalence of this syndrome. The variation in symptoms has resulted in a myriad of treatments though none has been established as defini

Hyperbaric oxygen therapy for vaso-occlusive crises in nine patients with sickle-cell disease.

INTRODUCTION: Vaso-occlusive crisis (VOC) is the most frequent complication of sickle-cell disease and is associated with significant acute bone pain. OBJECTIVE: To evaluate the feasibility and efficacy of hyperbaric oxygen therapy (HBOT) for severe VOC. METHODS: We report our retrospective experience with HBOT in VOC in nine patients and 15 HBOT sessions. RESULTS: All nine patients had received appropriate conventional treatment prior to HBOT. Pain scores using a Visual Analog Scale (0 to 10) determined whether HBOT was effective or not in improving symptoms. While no change in pain score

A cost-analysis case study of radiation cystitis treatment including hyperbaric oxygen therapy.

AIM: To undertake an economic analysis of the direct costs of treating radiation cystitis from a purchaser perspective, comparing conservative, non-operative and surgical interventions with hyperbaric oxygen treatment (HBOT). METHODS: A male in his 60s with prostatic carcinoma consented to this study. Full details of treatment costs in AUD were obtained (AUD 1.0, approx. EUR 0.6). A detailed patient diary accurately cross-referenced the consultations, investigations, admissions and treatment. Costs were recorded on a spreadsheet, dated and grouped under eight major headings related to treatme

Retinal artery occlusion: visual outcome after treatment with hyperbaric oxygen.

METHODS: We describe a case series of 11 patients with retinal artery occlusion treated with hyperbaric oxygen therapy (HBOT) at Intermountain Dixie Regional Medical Center between 2005 and 2009. We then combined data from our case series with data from two other case series to report on a combined total of 51 patients. RESULTS: Eight of our 11 patients achieved improved visual acuity. Analysis of the combined case series showed that 74 % of patients treated with HBOT had improvement in visual acuity (P < 0.0001) with 53 % improving two lines or more on a modified Snellen value. The combined

Malignant otitis externa: experience with hyperbaric oxygen therapy.

INTRODUCTION: The treatment of malignant otitis externa (MOE) with hyperbaric oxygen therapy (HBOT) remains controversial. The rarity of MOE, combined with poor access to hyperbaric facilities, explains the paucity of existing data. METHODS: We retrospectively reviewed all patients with a diagnosis of MOE referred to the Prince of Wales Hospital hyperbaric unit over a period of six years, and report one of the largest case series to date. RESULTS: From August 2001 to October 2007, 17 patients with MOE were referred, of whom 15 (88%) completed therapy, one did not tolerate HBOT and one was wi

Adjunctive therapy for decompression illness: a review and update.

Therapeutic interventions may augment the outcome or provide temporizing support pending recompression. Effective measures include first aid (surface) oxygen therapy, fluid resuscitation, non-steroidal anti-inflammatory drugs and avoidance of fever. Lignocaine may also be effective, particularly for cerebral arterial gas embolism (CAGE). For critically ill patients with CAGE, animal studies do not support the use of hyperventilation. There is strong experimental evidence in animals for the efficacy of intravenous perfluorocarbon. When lower limb paralysis occurs, low molecular weight heparin i

NHMRC guidelines for clinical practice for ASD and PTSD.

Dear Editor, Recently I described the case of a scuba instructor suffering from acute stress disorder (ASD), a type of post-traumatic stress disorder (PTSD), following the death of one of her students. The treatment described was a combination of eye movement desensitization and reprocessing (EMDR) and cognitive-behavioural therapy (CBT) exposure based exercises. As it happens, in August the Australian Centre for Post Traumatic Mental Health published Australian clinical practice guidelines for ASD and PTSD. These have been endorsed by the National Health and Medical Research Council (NHMRC).

Heat shock increases survival in rats exposed to hyperbaric pressure.

It has been shown that a single bout of exercise performed 20 hours prior to hyperbaric exposure reduces bubble formation and increases survival in rats. Heat shock proteins (HSPs) are stress proteins expressed in cells that are exposed to different stressors. HSPs are known to protect cells, by binding to proteins and stabilizing them. As it is known that a single bout of exercise induces HSPs, and that HSPs exert their protective effects 20-24 hours after the stimulus for induction, we hypothesized that HSPs might be one mechanism behind the observed exercise-induced protection. We hypothesi

Outcomes of ST-Elevation Myocardial Infarction in Patients With Opioid Dependence: A National Inpatient Sample Study.

ST-segment elevation myocardial infarction (STEMI) is a critical condition characterized by the sudden obstruction of one or more coronary arteries, resulting in diminished blood flow to the heart muscle. This acute ischemic event demands swift and precise intervention to minimize myocardial damage and preserve cardiac function. Opioids, a class of potent analgesic medications, are frequently utilized in the management of STEMI-related chest pain. Despite their efficacy in alleviating discomfort, their use in this context warrants careful consideration due to potential adverse effects and inte

Understanding the Needs and Lived Experiences of Patients With Graft-Versus-Host Disease: Real-World European Public Social Media Listening Study.

BACKGROUND: Graft-versus-host disease (GVHD) is the major cause of short- and long-term morbidity and mortality after allogeneic hematopoietic stem cell transplantation. Treatment options beyond corticosteroid therapy remain limited, and prolonged treatment often leads to impaired quality of life (QoL). A better understanding of the needs and experiences of patients with GVHD is required to improve patient care. OBJECTIVE: The aim of this study is to explore different social media (SM) channels for gathering and analyzing the needs and experiences of patients and other stakeholders across 14
