High proximal migration rate of a partially covered "big cup" duodenal stent in patients with malignant gastric outlet obstruction.

Endoscopic placement of self-expandable metal stents (SEMS) has emerged as a palliative treatment for patients with malignant gastric outlet obstruction (GOO). Recently, a new partially covered "big cup" SEMS has been developed to prevent both stent migration and tissue ingrowth. The aim of the study was to evaluate safety and efficacy of this SEMS in a cohort of patients with incurable malignant GOO. The study was terminated prematurely due to three proximal stent migrations in six patients. Migrations occurred at 2, 4, and 29 days, respectively, and necessitated endoscopic removal and placem

Percutaneous insertion of double-J ureteral stent in children with ureteral obstruction: our experiences.

OBJECTIVE: Ureteral obstruction (UO) is usually treated by surgical or endoscopic approaches. We investigated whether percutaneous anterograde treatment with insertion of double-J ureteral stent (DJ) is a feasible alternative technique for the management of UO in selected cases, where traditional approaches are not possible or too risky. PATIENTS AND METHODS: The DJ was percutaneously inserted into 10 children (mean age 9 years) who suffered from UO. Three children had already been treated surgically for complex urotract congenital anomalies; six children had restenosis/reocclusion or stenosi

Cage migration after transforaminal lumbar interbody fusion and factors related to it.

OBJECTIVE: To review and analyze cage migration and related risk factors in patients who have undergone transforaminal lumbar interbody fusion (TLIF). METHODS: A retrospective study was conducted to review the complications of cage migration in 512 patients who had undergone a TLIF procedure from January 2010 to June 2011 in five spinal centers. In all, 263 men and 249 women with a mean age of 54.7 years were included. All patients were followed up at 3, 6 and 12 months after the procedure. The clinical outcomes were evaluated by visual analogue scores, the Oswestry disability index, plain ra

Late-acquired stent malapposition after sirolimus-eluting stent implantation following acute coronary syndrome: angiographic, IVUS, OCT and coronary angioscopic observation.

Drug-eluting stents (DES) have been shown to significantly reduce the incidence of restenosis and target lesion revascularization in a wide variety of clinical situations. DES have also been shown to significantly reduce neointimal hyperplasia as compared to bare-metal stents. However, the antiproliferative properties of DES also delay vascular healing and have been associated with stent malapposition, hypersensitivity reactions and late stent thrombosis. Stent thrombosis could result in myocardial infarction or death. We describe here a case report of late stent malapposition following siroli

Transmediastinal migration of Kirschner wire.

The authors describe a case of transmediastinal migration, and lodging into the right lung, of a Kirschner wire used to fix a fracture of the left clavicle. The medical literature reports cases of intrathoracic migration of Kirschner wire; however, a transmediastinal migration has not been previously described. A small right thoracotomy was performed and the wire was removed without complications. We discuss aspects of this uncommon finding and alert physicians for the potential risks of the migration of metallic pins used to fixate boney structures within the chest.

Endoscopic resolution of pseudocyst infection and necrosis as a complication of endoscopic pseudocyst drainage. A case report.

CONTEXT: Endoscopic therapy has recently gained importance as an option for the treatment of pancreatic necrosis. We report a case of transgastric endoscopic resolution of pseudocyst infection and necrosis as a complication of endoscopic pseudocyst drainage. CASE REPORT: The patient underwent several sessions of endoscopic removal of debris and necrotic material, intercalated with daily lavage and aspiration through a nasocystic tube. CONCLUSIONS: At the present time, fourteen months after pancreatic pseudocyst secondary infection, the patient remains asymptomatic with no radiographic signs

Lower limb ischemia after migration of a coronary artery stent into the femoral artery.

A coronary stent may be lost in the peripheral or visceral arterial system with an incidence ranging from 0.9 to 8.4%, however, a limb or organ ischemia after stent migration is very uncommon. We report the case of an 83-year-old man who underwent coronary artery stenting at our Hospital's Cardiological Department. During this difficult procedure, due to the critical stenosis of the circumflex artery, the stent was accidentally lost and found at the level of the insertion of the right common femoral on the external iliac artery. After several attempts to rescue the stent through an omolateral

Surgical management of a paraclinoid aneurysm containing a displaced neuroform microstent: technical case report.

OBJECTIVE: We report the first case of clip obliteration of a cerebral aneurysm containing a displaced microstent. CLINICAL PRESENTATION: A 63-year-old woman presented with a 6-month history of headaches. She had no other neurological symptoms, and her examination was normal. A computed tomographic scan showed no evidence of hemorrhage. Angiography confirmed the presence of a large left paraclinoid aneurysm. INTERVENTION: The patient initially underwent attempted endovascular repair of the aneurysm. A Neuroform microstent (Boston Scientific, Natick, MA), which was placed across the aneurysm

Trimming a metallic biliary stent using an argon plasma coagulator.

BACKGROUND: Distal migration is one of the common complications after insertion of a covered metallic stent. Stent repositioning or removal is not always possible in every patient. Therefore, trimming using an argon plasma coagulator (APC) may be a good alternative method to solve this problem. METHODS: Metallic stent trimming by APC was performed in 2 patients with biliary Wallstent migration and in another patient with esophageal Ultraflex stent migration. The power setting was 60-100 watts with an argon flow of 0.8 l/min. OBSERVATIONS: The procedure was successfully performed and all dist

Long-term follow-up in patients with lost coronary stents during interventional procedures.

The aim of this retrospective study was to determine the incidence of stent loss during interventional procedures and to identify the associated immediate and long-term consequences of such loss. We evaluated 36 cases in which the stents were prematurely displaced from the stent delivery device. Five stents were completely removed from the vascular system successfully. Five stents could not be retrieved from the coronary circulation and led to significant adverse events in 3 patients, in whom the lost stents were not excluded by the placement of another stent. In 26 patients, peripheral emboli

Intraarticular migration of a broken biodegradable interference screw after anterior cruciate ligament reconstruction.

Poly-L: -lactic acid biodegradable screws have been used effectively for graft fixation in anterior cruciate ligament (ACL) reconstruction. The overall complication rate associated with the use of this implant is low, although some authors reported complications, such as osteolysis and aseptic effusion of the knee joint. We report a case of a 29-year-old female patient with a failure of a biodegradable interference screw at 22 months after ACL reconstruction using bone-patellar tendon-bone graft. In this illustrated case, the screw broke and migrated into the knee joint. In addition, we perfor

Efficacy and safety of endoscopic retrograde cholangiopancreatography in patients with Billroth II anastomosis.

BACKGROUND/AIMS: Endoscopic retrograde cholangiopancreatography (ERCP) procedure is quite difficult to perform in patients with Billroth II anastomosis because of anatomical alterations. The aim of this study is to evaluate retrospectively the results of ERCP applications done in patients with Billroth II operation. METHODOLOGY: Out of the 1632 patients who underwent ERCP between 1992 and 2002, 27 (1.65%) had Billroth II operation. The records of these 27 patients were reviewed. Details noted included indications for ERCP, therapeutic interventions, causes of failure and complications. RESUL

Reoperations after failure of stent grafting for type B aortic dissection: report of two cases.

We describe our successful management of two patients who suffered complications after stent grafting for Type B aortic dissections. One patient was found to have stent-graft migration, which we treated with repeat aortic stent grafting, and the other patient had a proximal endoleak and total occlusion of the stent graft, which we treated with open surgical repair. We discuss the measures used to assist us in deciding on the most appropriate surgery, as well as the treatment alternatives.

Cerebral aneurysm perforations complicating therapy with Guglielmi detachable coils: a meta-analysis.

BACKGROUND AND PURPOSE: The risk of intraprocedural aneurysm perforation in patients with previously ruptured aneurysms tends to be higher than that of patients with previously unruptured aneurysms, but a statistically significant difference has not been shown. Our purpose was to define the rates of occurrence and of morbidity and mortality associated with aneurysmal perforation associated with coil embolization. METHODS: A meta-analysis of the results from 17 published retrospective reports of aneurysm perforations complicating therapy with Guglielmi detachable coils (GDCs) was performed. Ra

Extrusion of porous polyethylene orbital implant in recurrent retinoblastoma.

High-density porous polyethylene allografts are used as orbital implants in reconstruction of anophthalmic sockets. The use of this material for integrated buried implants is rather new, and studies on removed implants from humans are limited. We studied the case of a 6-year-old boy with an extruded porous polyethylene implant due to orbital recurrence of retinoblastoma. The clinical and morphological features are reported. Histopathology revealed fibrovascular downgrowth in the outer two thirds of the sphere. Scanning electron microscopy, however, documented further downgrowth of fibrous tiss
