Tonsillar herniation and cervical syringomyelia in association with posterior fossa tumors in children: a case-based update.

BACKGROUND: Posterior fossa tumors most commonly occur in children. They frequently present with raised intracranial pressure and may have tonsillar herniation. The symptomatology is predominantly directed towards the tumor. The occurrence of syringomyelia in such cases is most unusual. Symptomatic syringomyelia is rare. CASE REPORT: We describe a case of a pilocytic astrocytoma of the cerebellum in a 13-year-old girl who presented with clinical features of progressively worsening raised intracranial pressure and secondary tonsillar herniation and cervical syringomyelia. Magnetic resonance (M

When a Root Is the Cause of Infection.

BACKGROUND: Sinorhizobium meliloti is a phytobacterium found in the root nodules of plants, where it is involved in fixing nitrogen for delivery to the roots in exchange for a photosynthate carbon source. There have been no reported cases of S. meliloti infection in humans. We conducted a retrospective review of clinical records and diagnostic tests. CASE DESCRIPTION: An 81-year-old woman who presented to the emergency department with a 1-day history of progressive decline in her level of consciousness following a head injury and deep scalp laceration. Her medical history was significant for

EXPERIMENTAL HYDROCEPHALUS.

Internal hydrocephalus can be produced experimentally by injecting a foreign substance into the ventricles. In these experiments, aleuronat, a granular, insoluble material, has caused an acute inflammatory reaction, characterized in the first week by an exudate consisting largely of polynuclear leucocytes. Later the picture is one of a chronic process; polynuclear cells are replaced by lymphoid and large mononuclear cells and there is proliferation of the connective tissue in the choroid plexus. Proliferation of the ependyma occurs in the first week but becomes more advanced in the second and

Alzheimer Disease Cerebrospinal Fluid Biomarkers in a Tertiary Neurology Practice.

OBJECTIVE: To evaluate the performance of Alzheimer disease (AD) cerebrospinal fluid (CSF) biomarkers in a tertiary neurology clinic setting with high frequency of non-AD cases, including normal pressure hydrocephalus (NPH). METHODS: There were 534 patients who underwent AD CSF biomarkers (Roche Elecsys Aβ42, p-Tau181, total-Tau) from April 1, 2020, through April 23, 2021. A behavioral neurologist blinded to CSF results assigned a clinical diagnosis retrospectively on the basis of consensus criteria, and a neuroradiologist blinded to the diagnosis and CSF studies graded brain magnetic resonan

The brain in older persons with and without dementia: findings on MR, PET, and SPECT images.

Neuroimaging plays an increasingly important role in the evaluation of dementias of older adults. Although no unequivocal diagnostic test other than biopsy is currently available for Alzheimer's disease and some other dementias during life, neuroimaging techniques are an integral part of the examination and follow-up of patients with dementia. This review describes findings in the brains of patients with normal aging and in elderly patients with dementia, as shown by MR imaging and MR spectroscopy, positron emission tomography (PET), and single-photon emission computed tomography (SPECT) and c

Transvaginal Doppler assessment of fetal intracranial venous flow.

OBJECTIVE: To investigate physiologic blood-flow-velocity waveform patterns of the fetal cerebral venous system during normal pregnancies by transvaginal Doppler studies and to evaluate cases with abnormal venous-flow patterns. METHODS: Internal cerebral veins and the three dural sinuses, those of the superior sagittal sinus, vein of Galen, and straight sinus, were examined in normal cephalic-presenting fetuses of 20-40 weeks' gestation. For analysis, the venous index was defined as maximum minus minimum velocity divided by maximum velocity. Different cases with intracranial abnormalities wer

Detection of third and sixth cranial nerve palsies with a novel method for eye tracking while watching a short film clip.

OBJECT: Automated eye movement tracking may provide clues to nervous system function at many levels. Spatial calibration of the eye tracking device requires the subject to have relatively intact ocular motility that implies function of cranial nerves (CNs) III (oculomotor), IV (trochlear), and VI (abducent) and their associated nuclei, along with the multiple regions of the brain imparting cognition and volition. The authors have developed a technique for eye tracking that uses temporal rather than spatial calibration, enabling detection of impaired ability to move the pupil relative to normal

Rare Complications of Lumboperitoneal Shunts: Abdominal Cerebrospinal Fluid Pseudocysts in Adults.

Abdominal cerebrospinal fluid (CSF) pseudocysts are rare complications of shunt surgery, predominantly reported in pediatric patients undergoing a ventriculoperitoneal (VP) shunt. In contrast, their occurrence following lumboperitoneal (LP) shunt is exceptionally uncommon. We report the case of a 76-year-old woman who presented with recurrent symptoms, including gait disturbance and cognitive decline, approximately three years after undergoing LP shunt placement for idiopathic normal pressure hydrocephalus. Diagnostic imaging, including shuntography and abdominal computed tomography (CT), reve

Pure Hereditary Spastic Paraplegia in a Patient With a Novel Heterozygous KIDINS220 Gene Mutation.

This case presents a somewhat unique and different phenotype of hereditary spastic paraplegia from previously reported kinase D-interacting substrate of 220 kDa (KIDINS220) gene mutation-related disease. We report a unique putative causative heterozygous mutation in KIDINS220 in a pure hereditary spastic paraplegia (HSP) patient expanding the HSP group further. We also deliberate on how our case was different from prior KIDINS220-related pathologies including spastic paraplegia, intellectual disability, nystagmus, and obesity (SINO) syndrome, and the observation of KIDINS220 and aquaporin-4 (A

Feasibility of Nonintubated Anesthesia for Lumboperitoneal Shunt Implantation.

Lumboperitoneal shunt (LPS) implantation is a cerebrospinal fluid diversion therapy for the communicating type of normal-pressure hydrocephalus (NPH); NPH mainly affects older adults. However, endotracheal intubation for mechanical ventilation with muscle relaxant increases perioperative and postoperative risks for this population. Based on knowledge from nonintubated thoracoscopic surgery, which has been widely performed in recent years, we describe a novel application of nonintubated anesthesia for LPS implantation in five patients. Anesthesia without muscle relaxants, with a laryngeal mask

Whole Brain Parcellation with Pathology: Validation on Ventriculomegaly Patients.

Numerous brain disorders are associated with ventriculomegaly; normal pressure hydrocephalus (NPH) is one example. NPH presents with dementia-like symptoms and is often misdiagnosed as Alzheimer's due to its chronic nature and nonspecific presenting symptoms. However, unlike other forms of dementia NPH can be treated surgically with an over 80% success rate on appropriately selected patients. Accurate assessment of the ventricles, in particular its sub-compartments, is required to diagnose the condition. Existing segmentation algorithms fail to accurately identify the ventricles in patients wi

Necrotizing fasciitis caused by a primary appendicocutaneous fistula.

We report a case of necrotizing fasciitis in the loin of a 76-year old man with several coexisting or past health issues, including diabetes mellitus, hypertension, alcohol-related liver cirrhosis, gastrectomy for gastric cancer, subarachnoid hemorrhage, normal pressure hydrocephalus, and cerebral infarction. Incision of the necrotizing fasciitis was successful, but it revealed an appendicocutaneous fistula; thus, we performed appendectomy and fistulectomy. We think that the necrotizing fasciitis was caused by appendicitis perforation involving the retroperitoneum, inducing the formation of an

Foramen magnum tumors. Analysis of 57 cases of benign extramedullary tumors.

This study involved 57 patients with benign extramedullary tumors of the foramen magnum (19 neurinomas, 37 meningiomas, and one teratoma), who were operated on between 1957 and 1976. The 37 meningiomas represented 3.2% of 1139 meningiomas of the neuraxis. The initial neurological examinations of about half of these patients were unremarkable. The clinical presentation of tumors of the foramen magnum frequently mimics multiple sclerosis, cervical spondylosis, intramedullary tumor, syrinx, carpal tunnel syndrome, and even normal-pressure hydrocephalus. All operations were performed through a pos

Evaluation of the elderly patient with an abnormal gait.

Distinguishing between the normal gait of the elderly and pathologic gaits is often difficult. Pathologic gaits with neurologic causes include frontal gait, spastic hemiparetic gait, parkinsonian gait, cerebellar ataxic gait, and sensory ataxic gait. Pathologic gaits with combined neurologic and musculoskeletal causes include myelopathic gait, stooped gait of lumbar spinal stenosis, and steppage gait. Pathologic gaits with musculoskeletal causes include antalgic gait, coxalgic gait, Trendelenburg gait, knee hyperextension gait, and other gaits caused by inadequate joint mobility. A working kno

Evaluation of fetal and uterine hemodynamics during maternal cardiopulmonary bypass.

BACKGROUND: About 4% of pregnant women have concomitant cardiac disease and a few of these do not respond to medical therapy and require surgical correction during pregnancy. We report a unique case describing fetal hemodynamics and uterine blood flow before, during, and after maternal cardiopulmonary bypass, and compare them to normal reference values in the second trimester. CASE: A woman with severe aortic regurgitation underwent aortic valve replacement at 19 weeks' gestation. A nonpulsatile cardiac pump was used for cardiopulmonary bypass, maintaining the mean arterial pressure at 77-90

The Valsalva maneuver and Alzheimer's disease: is there a link?

Recent research findings provide evidence for Alzheimer's disease-related changes in brain diseases, such as normal pressure hydrocephalus and traumatic brain injury, and in glaucoma at the level of the retinal ganglion cells. This is a group of diseases that affect central nervous system tissue and are characterized by elevation of intracranial or intraocular pressure and/or local shear stress and strain. This strengthens the possibility that Alzheimer-type changes in these diseases may result at least in part from exposure of central nervous system tissue to elevated mechanical load. As acti

Degenerative pathologies on cortical biopsy, dopaminergic depletion, and shunt efficacy in iNPH.

INTRODUCTION: Coexisting degenerative pathologies may influence the efficacy of ventriculoperitoneal (VP) shunting in idiopathic normal pressure hydrocephalus (iNPH). METHODS: We evaluated 58 iNPH patients who underwent VP shunting with cortical biopsy assessing pathologies including amyloid beta (Aβ). Concordance between biopsy-identified Alzheimer's pathology and amyloid positron emission tomography (PET) and the influence of magnetic resonance imaging-based iNPH indices, dopamine transporter (DAT) imaging, and biopsy pathologies on postoperative outcomes were examined. Analysis of intracranial volume ratios by means of cerebrospinal fluid deployment indicators.

INTRODUCTION: Imaging studies make it possible not only to visualize the general structure of the brain but also to take precise measurements of brain tissue volume and the size of individual lobes and their structure. The aim of this study was to determine the ratio of cerebrospinal fluid (CSF) volume contained in the intracranial ventricular system to the brain tissue volume and the ratio of CSF volume in the subarachnoid space and basal cisterns to the brain tissue volume. MATERIAL AND METHODS: The evaluation of volumetric measurements of computed tomographic (CT) images was undertaken on
