Diagnosis and surgical management of extracranial PICA aneurysms presenting through subarachnoid haemorrhage: Case report and review of the literature.

OBJECTIVE AND IMPORTANCE: We present a very unusual case in which a proximal posterior inferior cerebellar artery (PICA) aneurysm was located extracranially. We reviewed the PICA origin anatomy and pathology of aneurysms at this site. CLINICAL PRESENTATION: A Caucasian woman, 52 years of age, presented with a lesion at the craniocervical junction. She complained only of headache off and on without other symptoms. Her physical examination was unremarkable. A CT angiography clearly visualized an approximately 9-mm wide aneurysm of the right PICA which lay intradurally in the spinal canal at the

Subclavian embolization associated to carotid-subclavian bypass and stent-graft repair of acute type-B dissection.

Intentional closure of the left subclavian artery (LSA) during an endovascular procedure can be complicated by retrograde filling of the excluded aorta, increasing the risk of aneurysm expansion and sudden rupture. Retrograde coil embolization of the LSA, as alternative to open subclavian ligature, is a safe and effective method of rapid false lumen sealing in patients requiring coverage of the LSA and carotid-subclavian bypass, even in the setting of acute aortic syndromes.

Urgent stenting for patients with acute stroke due to atherosclerotic occlusive lesions of the cervical internal carotid artery.

Acute symptomatic occlusion of the cervical internal carotid artery (ICA) can be treated by intravenous administration of tissue plasminogen activator, percutaneous transluminal angioplasty, and carotid endarterectomy. Carotid artery stenting (CAS) is now indicated for cervical ICA stenosis, but the safety and the efficacy of urgent CAS have not been established. We retrospectively reviewed 10 patients treated by urgent CAS for atherosclerotic occlusive lesions of cervical ICA with acute stroke. Five patients had complete occlusions and five had near total occlusions. Five of the 10 patients h

Procedural safety and potential vascular complication of endovascular recanalization for chronic cervical internal carotid artery occlusion.

BACKGROUND: Patients with chronic cervical internal carotid artery occlusion (ICAO) and cerebral ischemia may benefit from revascularization. The feasibility of endovascular recanalization for chronic ICAO has been reported recently, but its safety is still unproven. We report the follow-up results of 54 chronic ICAO patients who underwent endovascular recanalization, focusing on potential vascular complications and corresponding management. METHODS AND RESULTS: Endovascular recanalization for chronic ICAO was attempted in 54 consecutive patients (48 men; 69.2 + or - 9.8 years old) with eithe

Diffuse leptomeningeal enhancement, "ivy sign," in magnetic resonance images of moyamoya disease in childhood: case report.

We report three cases of children with moyamoya disease who revealed prominent leptomeningeal enhancement on postcontrast magnetic resonance (MR) images. Two of the children underwent bilateral extracranial-intracranial bypass operations because of ischemic symptoms. During the operations, a profuse pial arterial network was observed, which was considered to be the origin of the enhancement. The ischemic episodes disappeared after the operations, and the follow-up MR images showed marked reduction of the leptomeningeal enhancement. One of the three patients had an extracranial-intracranial byp

The cerebral venous system.

The authors discuss the gross and microscopic anatomy and the physiology of the cerebral venous system. Cerebral veins under pathological circumstances (hypercapnia, arterial hypertension, and increased intracranial pressure), pharmacological observations, the venous blood-brain barrier, and traumatic involvement are reviewed. Neoplastic involvement and radiological aspects are included. Surgical reconstruction of venous sinuses (including the Donaghy technique), tumor removal, sinus thrombectomy, and extraanatomical bypass of the transverse sinus are discussed.

Femoral vs. radial approach for primary percutaneous intervention in cardiogenic shock: a subanalysis from the ECLS-SHOCK trial.

AIMS: Cardiogenic shock (CS) is a life-threatening complication of acute coronary syndromes. Early revascularization with treating the culprit lesion improves survival. Nevertheless, the impact of access site (femoral vs. radial) on outcomes in infarct-related CS also in conjunction with extracorporeal life support (ECLS) remains unclear. METHODS AND RESULTS: This subanalysis of the ECLS-SHOCK trial included patients with infarct-related CS treated with or without ECLS, divided into femoral and radial access groups. The primary endpoint was 30-day mortality. Secondary endpoints included renal

A prospective randomized open-label trial of sirolimus versus paclitaxel-coated balloons for the side branch of de novo coronary bifurcation lesion.

BACKGROUND: Drug-coated balloons (DCBs) are commonly used in small coronary arteries. A recent trial reported similar outcomes between paclitaxel-coated balloons (PCBs) and sirolimus-coated balloons (SCBs) in side branch (SB) stenosis after stenting main branch (MB). This trial investigated whether SCBs and PCBs produce similar effects in de novo bifurcation lesions. METHODS: In this prospective, multicenter, randomized, non-inferiority clinical trial, patients with de novo non-left main coronary bifurcation lesions undergoing provisional stent of SB were randomly assigned in a 1:1 ratio to r

Arterial duplex for diagnosis of peripheral arterial emboli.

BACKGROUND: Whether duplex ultrasound (DUS) imaging alone can be used to successfully plan revascularization for peripheral arterial embolism (PAE) is unknown. This study evaluated the utility of DUS imaging alone for the diagnosis and treatment of PAE. METHODS: Patients with cardiogenic PAE to the lower or upper extremities during a 20-year period were retrospectively evaluated. Patients with visceral or cerebral PAE were excluded. Diagnosis by DUS imaging alone was compared with contrast angiography (CA) or computed tomography angiography (CTA). Patient demographics, use of intraoperative C

A Novel Hyperspectral Imaging System for Intraoperative Prediction of Cerebral Hyperperfusion Syndrome after Superficial Temporal Artery-Middle Cerebral Artery Anastomosis in Patients with Moyamoya Disease.

BACKGROUND: Postoperative cerebral hyperperfusion syndrome (CHS) may occur after superficial temporal artery (STA)-middle cerebral artery (MCA) bypass for moyamoya disease (MMD). Predicting postoperative CHS is challenging; however, we previously reported the feasibility of using a hyperspectral camera (HSC) for monitoring intraoperative changes in brain surface hemodynamics during STA-MCA bypass. OBJECTIVE: To investigate the utility of HSC to predict postoperative CHS during STA-MCA bypass for patients with MMD. METHODS: Hyperspectral images of the cerebral cortex of 29 patients with MMD w

Cerebral ischemia due to compression of the brain by ossified and hypertrophied muscle used for encephalomyosynangiosis in childhood moyamoya disease.

BACKGROUND: Encephalomyosynangiosis is one of the indirect method in which ischemic brain surface is covered by temporal muscle for the treatment of moyamoya disease. CASE DESCRIPTIONS: A 14-year-old girl who had been treated with bilateral STA-MCA anastomosis and EMS in 1999 was admitted on January 5, 2005. She showed transient incomplete palsy on the left side of the face and the ipsilateral upper extremity. On the day of admission, MRIs/MRA and 3-dimensional regional CBF measurement using stable xenon and CT scanning were conducted after performance of plain CT scanning. The MRI and CT stu

Moyamoya Across the Lifespan: Current Neurologic Care and Future Directions.

Moyamoya arteriopathy is an important cause of stroke across the lifespan, with high rates of incident and recurrent stroke in affected individuals. Although it affects adults and children globally, moyamoya is more prevalent in East Asian countries, particularly Japan and Korea. The R4810K variant of the RNF213 gene, most common in Asian populations, is associated with severe, early onset, multisystem vasculopathy. Neuroimaging is critical for moyamoya diagnosis and treatment planning, with conventional imaging, catheter angiography, perfusion imaging, and cerebrovascular reactivity assessmen

Preventive Effect of Bypass Surgery on Rebleeding in Patients with Hemorrhagic Twiglike Middle Cerebral Artery.

BACKGROUND: Aplastic or twiglike middle cerebral artery (Ap/T-MCA) is a rare cerebrovascular anomaly correlated to stroke. The most common characteristic of the disease is intracerebral hemorrhage. However, treatment measures that can prevent rebleeding have not yet been established. OBJECTIVE: We sought to validate whether extracranial-intracranial (EC-IC) bypass is effective in preventing recurrent intracerebral hemorrhage in patients with Ap/T-MCA. METHODS: Ap/T-MCA was defined on the basis of the following criteria: 1) M1 occlusion with plexiform vessels, 2) unilateral MCA involvement wi
