Task-Related Dynamic Division of Labor Between Anterior Temporal and Lateral Occipital Cortices in Representing Object Size.

UNLABELLED: Object size is represented by functionally distinct sectors along the ventral visual pathway. The early visual cortex encodes objects' sensory-retinal size. Subsequently, the occipitotemporal cortex computes objects' canonical size based on statistical regularities of visual features. Although the neurocomputation of size has been studied in a "bottom-up" sensory-driven framework, little is known about how perceptual size information is transformed into conceptual knowledge and how this computation is modulated by "top-down" goal-driven signals. Using continuous theta burst stimula

[Insight and psychiatric dangerousness: A review of the literature].

INTRODUCTION: Violence committed by individuals with severe mental illness has become an increasing focus of concern among clinicians, policy makers, and the general public, often as the result of tragic events. Research has shown in the past two decades an increased risk of violence among patients with mental disorder. Nevertheless, of those suffering from mental illness, perpetrators of other directed violence form a minority subgroup. The means by which there is this association between mental illness and violence has remained controversial. Factors such as positive psychotic symptoms, medi

Quantitative classification of primary progressive aphasia at early and mild impairment stages.

The characteristics of early and mild disease in primary progressive aphasia are poorly understood. This report is based on 25 patients with aphasia quotients >85%, 13 of whom were within 2 years of symptom onset. Word-finding and spelling deficits were the most frequent initial signs. Diagnostic imaging was frequently negative and initial consultations seldom reached a correct diagnosis. Functionality was preserved, so that the patients fit current criteria for single-domain mild cognitive impairment. One goal was to determine whether recently published classification guidelines could be impl

Next-step strategies for panic disorder refractory to initial pharmacotherapy: a 3-phase randomized clinical trial.

BACKGROUND: More data are needed to guide next-step interventions for panic disorder refractory to initial intervention. METHOD: This 24-week randomized clinical trial (RCT) enrolled 46 patients with DSM-IV-defined panic disorder from November 2000 to April 2005 and consisted of 3 phases. Patients who failed to meet remission criteria were eligible for randomization in the next treatment phase. Phase 1 was a 6-week lead-in with open-label sertraline flexibly dosed to 100 mg (or escitalopram equivalent) to prospectively define treatment refractoriness (lack of remission). Phase 2 was a 6-week

A randomized controlled trial of fall prevention strategies in old peoples' homes.

BACKGROUND: Falls are a major cause of morbidity in old age. A small number of fall prevention trials in cognitively intact community-dwelling older people have been effective. This study set out to examine the preventability of falls in older people living in institutional care. OBJECTIVE: To evaluate the effectiveness of falls risk factor assessment/modification and seated balance exercise training in reducing falls among elderly people living in residential care. METHODS: 133 residents with a mean age of 84+/- (SD) 6.8 years were allocated at random by home to receive either a 6-month fal

[Investigation of Molecular Differences in Plasmodium spp. Isolates Obtained from Malaria Patients].

Malaria continues to pose a serious threat to more than half of the world’s population each year. The Plasmodium parasites that cause disease are not limited to humans alone; they have a wide range of infections that extend to reptiles, birds, mammals and other vertebrates. Plasmodium species have extraordinary genetic flexibility that allows them to adapt to changes in the environment, giving them the potential to rapidly develop resistance to therapeutics such as antimalarials and to change host specificity. Parasites multiply dynamically in the erythrocytes of vertebrate hosts and during th

Palliative care needs among residential aged care facility residents referred to an Australian hospital-in-the-home service: a descriptive retrospective cohort study.

BACKGROUND: Although the majority of those living in residential aged care facilities (RACFs) prefer to remain in their facility if their health deteriorates, many do not end up achieving this. Hospital-in-the-home (HITH) services have been increasingly used to support RACFs by providing acute, palliative, and supportive care to residents on-site. Understanding the reasons for RACFs engaging HITH services can help identify unmet needs and guide improvements in community-based end-of-life care. This study aimed to characterise the palliative care needs of RACF residents admitted to an Australia

Zuranolone - synthetic neurosteroid in treatment of mental disorders: narrative review.

With each passing year, the number of people suffering from mental disorders grows at a disturbing speed. Neuroactive steroids are a new promising group of drugs with the potential for use in many diseases like postpartum depression, postnatal psychosis, major depression, insomnia, bipolar disorder, and Parkinson's tremor, due to their ability to modulate the activity of GABAA receptor. Neurosteroids are progesterone metabolites that are synthesized from cholesterol or steroid hormones in various brain regions. They regulate neuronal development, regeneration, and neurotransmission. They are i

Colocalized White Matter Plasticity and Increased Cerebral Blood Flow Mediate the Beneficial Effect of Cardiovascular Exercise on Long-Term Motor Learning.

Cardiovascular exercise (CE) is a promising intervention strategy to facilitate cognition and motor learning in healthy and diseased populations of all ages. CE elevates humoral parameters, such as growth factors, and stimulates brain changes potentially relevant for learning and behavioral adaptations. However, the causal relationship between CE-induced brain changes and human's ability to learn remains unclear. We tested the hypothesis that CE elicits a positive effect on learning via alterations in brain structure (morphological changes of gray and white matter) and function (functional con

Clozapine and Psychosocial Function in Schizophrenia: A Systematic Review and Meta-Analysis.

BACKGROUND: Clozapine has unique efficacy for symptoms in treatment-resistant schizophrenia; however, symptomatic remission is not necessary nor sufficient for functional improvement. No study has pooled the effect of clozapine on psychosocial function across clinical trials. OBJECTIVE: We conducted a systematic review and meta-analysis to compare the effects of clozapine with other antipsychotics on psychosocial function, and described the predictors of functional outcome. METHODS: We searched MEDLINE/PubMed, PsychINFO, EMBASE, CINAHL, Scopus, Web of Science, Cochrane Central Register of co

Effects of physical exercise on physical function in older adults in residential care: a systematic review and network meta-analysis of randomised controlled trials.

BACKGROUND: Physical exercise is effective at attenuating ageing-related physical decline in general, but evidence of its benefits for older adults in residential care, who often have functional dependency, multimorbidity, and polypharmacy, is inconclusive. We aimed to establish the effects of exercise interventions on the physical function of this population. METHODS: For this systematic review and network meta-analysis, we searched PubMed, Web of Science, Cochrane Library, Rehabilitation & Sports Medicine Source, and SPORTDiscus to identify randomised controlled trials assessing the effects

Psychodynamic accessibility: a testable framework for supported agency in social psychiatry and psychiatric rehabilitation.

Recovery-oriented social psychiatry and psychiatric rehabilitation increasingly emphasize rights, participation, community-based care, supported decision-making, supported employment, and prevention-oriented early intervention. Yet formal access to services, information, rehabilitation, legal-administrative procedures, or participation formats does not by itself ensure that people can use these resources when shame, mistrust, affective overload, fatigue, cognitive narrowing, or structural vulnerability are already present. This Hypothesis and Theory article introduces psychodynamic accessibili

Advanced Drug Delivery Strategies in Geriatric Patients with Polypharmacy: Integrating Pharmacokinetics, Personalized Medicine, and Emerging Technologies.

Background/Objectives: The rapid growth of the global aging population, projected to reach 2.1 billion older adults by 2050, presents major challenges for pharmacotherapy and drug delivery. Age-related physiological changes affecting pharmacokinetics and pharmacodynamics, widespread polypharmacy, and functional impairments such as dysphagia, cognitive decline, and sensory or motor limitations reduce the effectiveness and safety of conventional "one-size-fits-all" medication approaches. This review aimed to evaluate the major barriers to effective drug delivery in older adults and to assess eme

Short-term, circuit-based heat acclimation enhances intermittent work capacity, reaction time, and neuromuscular function under heat stress in active males.

Exertional heat stress impairs cardiovascular, thermoregulatory, cognitive, and neuromuscular function, compromising health and performance. Although effective, traditional active heat acclimation (HA) relies on prolonged, low- to moderate-intensity exercise heat exposures and may therefore lack context-specificity. We investigated whether short-duration, high-intensity, circuit-based HA elicits favorable performance and thermophysiological adaptations. Eighteen participants completed either five circuit training sessions in hot (HEAT: 35°C; n = 9) or thermoneutral conditions (CON: 20°C; n = 9
