The effect of blood transfusion protocol on retinopathy of prematurity: A prospective, randomized study.

OBJECTIVE: Controversy exists regarding the potential influence of anemia and blood transfusions on the rate of retinopathy of prematurity (ROP) in premature infants. A prospective, randomized, masked trial was performed to determine the influence of red blood cell transfusion protocol on ROP incidence and severity in a population of high-risk infants. METHODS: A total of 50 infants with birth weights <1251 g were divided randomly into two groups beginning on day of life 29. Group 1 (n = 24) received red cell transfusions during the 6-week study period, only if certain symptom-based guideline

Neonatal outcome of very premature infants from multiple and singleton gestations.

OBJECTIVES: Our purpose was to determine whether, in the era of surfactant treatment, very premature neonates from multiple gestations have outcomes similar to those of singletons. STUDY DESIGN: We collected data on 572 infants (369 singletons, 203 multiple gestation) born and cared for at a single institution from July 1, 1992, through Dec, 31, 1994, of gestational ages 24 to 32 weeks. We compared singleton infants with infants from multiple gestations within gestational age categories 24 to 26 weeks, 27 to 29 weeks, and 30 to 32 weeks. RESULTS: Infants of multiple gestations were more like

Fetal cerebral Doppler studies as a predictor of perinatal outcome and subsequent neurologic handicap.

OBJECTIVE: To study the use of middle cerebral arterial Doppler findings in a group of high-risk fetuses as a predictor of adverse perinatal outcome, including subsequent neurologic handicap. METHODS: A group of very high-risk fetuses was recruited over a 2-year period for study. Weekly fetal biometries and Doppler studies of the umbilical artery and middle cerebral arteries were carried out until delivery. Main outcome indices analyzed included birth weight ratio (ratio of observed birth weight to mean birth weight for gestation), days of ventilator requirement, neonatal intracranial hemorrh

Amniotic fluid interleukin-6: a sensitive test for antenatal diagnosis of acute inflammatory lesions of preterm placenta and prediction of perinatal morbidity.

OBJECTIVE: Our purpose was to determine whether amniotic fluid concentrations of interleukin-6 are of value in the antenatal diagnosis of acute inflammatory lesions (histologic chorioamnionitis) of preterm placenta and in the prediction of perinatal morbidity and mortality. STUDY DESIGN: The relation among placental histologic findings, perinatal outcome, and amniotic fluid interleukin-6 concentrations was examined in 50 consecutive patients who delivered preterm neonates within 72 hours after amniocentesis. Interleukin-6 was determined by enzyme-linked immunosorbent assays. Receiver-operator

Enterotropic coronavirus (mouse hepatitis virus) in mice: influence of host age and strain on infection and disease.

The course of enterotropic mouse hepatitis virus (MHV) infection was examined in genetically susceptible (BALB) and resistant (SJL) mice of different ages at inoculation (1, 3, and 12 weeks) and at sequential intervals (1, 2, 3, 5, 10, 20, and 30 days) after oral inoculation with the Y strain of MHV (MHV-Y). Virus was quantified in stomach, upper and lower segments of small intestine, cecum, upper and lower segments of colon, Peyer's patches, mesenteric lymph node, and feces, and tissues were examined microscopically. An infant mouse bioassay was used to quantify virus in all tissues of 3-week

The Vermont-Oxford Trials Network: very low birth weight outcomes for 1990. Investigators of the Vermont-Oxford Trials Network Database Project.

This report describes the Vermont-Oxford Trials Network, a voluntary collaborative research network, and summarizes the outcomes and medical interventions for very low birth weight infants at participating centers in 1990. The Vermont-Oxford Trials Network included 36 centers in 1990 (11% university hospitals, 44% university affiliates, 44% nonaffiliated) with a total of 2961 infants weighing 501 to 1500 g (median 73 infants, range 5 to 172). Eighty percent of the infants were inborn and 65% were white. The overall network frequencies for selected interventions and outcomes were as follows: pr

Pulmonary sequestration: 17-year experience at UCLA.

Pulmonary sequestration is a complex anomaly involving the pulmonary parenchyma and its vascularity. From 1975 to 1992, 10 cases have been treated at the UCLA Medical Center. The ages of the seven females and three males were bimodal, the median age of the seven children was 29 days (range 1 day-6 years); it was 32 years (range 28-39) for the three adults. One child was delivered by cesarean section for fetal distress and another was born at 29 weeks gestation. Symptoms included: recurrent pneumonia (5), respiratory distress (5), hemoptysis (2), stridor (1), and pleuritis (1). Chest radiograph

Total parenteral nutrition-associated cholestasis: clinical and histopathologic correlation.

Cholestatic jaundice is the major complication of total parenteral nutrition (TPN) in infants and children. The pathogenesis of this syndrome is poorly understood. The aims of this study were: (1) to define the histologic liver injury in relation to the clinical course of infants on TPN and (2) to determine whether enteral feeding will reverse or halt these changes. We identified 31 infants treated for severe gastrointestinal disease for whom liver histology was available from 1987 to 1991. Clinical records and liver biopsy (23) or autopsy specimens (13) were reviewed. Five patients had biopsi

Comprehensive fetal assessment with three ultrasonographic characteristics.

OBJECTIVES: When three ultrasonographic characteristics--umbilical artery Doppler recording, growth (abdominal circumference), and biophysical profile score--are used in combination in assessment of fetuses at risk of chronic asphyxia: (1) What are the order and time scale for the development of abnormality with each characteristic? (2) What is the short-term outcome associated with abnormalities of the three characteristics? (3) Should we amend our fetal assessment protocol? Short-bowel syndrome in children. Quality of life in an era of improved survival.

A number of disorders in childhood can result in short-bowel syndrome (small bowel length, less than 100 cm). Improved care has increased survival in patients with short-bowel syndrome, but the quality-of-life factors associated with such improved survival have not been examined, to our knowledge. Sixteen consecutive pediatric patients with short-bowel syndrome (bowel length range, 22 to 98 cm) were followed up for 2 to 10 years. The original diagnoses were as follows: necrotizing enterocolitis (n = 6), multiple intestinal atresias (n = 4), extensive aganglionosis (n = 2), meconium peritonitis

Successful closure of intestinal fistulae in an infant using the somatostatin analogue SMS 201-995.

Enterocutaneous and other gastrointestinal fistulae create tough challenges with respect to wound care, nutrition, and strategy for closure. SMS 201-995 is a long-acting somatostatin analogue that has been effective in the treatment of adult enteric fistulae. The use of SMS 201-995 as an adjunct in the successful closure of intestinal fistulae in an infant is reported. This newborn infant developed a high-output enterocutaneous fistula and multiple enteroenteric fistulae after an intestinal resection for necrotizing enterocolitis. Copious fistulae output led to extensive wound breakdown, dehyd

Pneumatosis intestinalis in children beyond the neonatal period.

Pneumatosis intestinalis (PI) is a well-recognized manifestation of necrotizing enterocolitis (NEC) in the newborn--a condition that often requires surgical intervention for infarcted bowel. However, little information is available concerning PI in older children or its management. Sixteen older infants and children (greater than 2 months) had x-ray findings of PI (intramural air). There were eight girls and eight boys ranging in age from 2 months to 8 years. Associated conditions included short bowel syndrome (SBS) (8), congenital heart disease (2), iron ingestion (1), nesidioblastosis (1), h

Characterization of intestinal collateral blood flow in the developing piglet.

Interest in the pathogenesis of neonatal necrotizing enterocolitis has prompted study of the intestinal circulation in developing animals. It is conceivable that poorly developed collateral channels may predispose the neonatal intestine to ischemic insults. We therefore characterized intestinal collateral blood flow in anesthetized and ventilated 1-day and 1-month-old piglets. Intestinal blood flow was measured with radioactive microspheres (15 micron diameter) before and after either 1) total occlusion of the superior mesenteric artery (SMA) or 2) occlusion of a distal (jejunoileal) branch of

Neonatal polycythemia: frequency of clinical manifestations and other associated findings.

We report the results of a two-part study examining the frequency of symptoms and other findings associated with neonatal polycythemia. In the first part of the investigation, we evaluated the occurrence and features of the disorder in a cohort of 3,768 infants born at our institution that had been screened for the disorder during a 4-year period. Fifty-five infants (1.46%) had neonatal polycythemia. Of these infants, 85% had features associated with the disorder. Frequent signs and symptoms included "feeding problems" (21.8%), plethora (20.0%), lethargy (14.5%), cyanosis (14.5%), respiratory

Water balance in very low-birth-weight infants: relationship to water and sodium intake and effect on outcome.

The clinical effects of fluid therapy designed to maintain different degrees of negative water balance during the first five days of life were determined prospectively in 88 very low-birth-weight infants. Infants with birth weights of 750 to 1,500 gm were matched for birth weight in 250 gm increments. RDS or no RDS, asphyxiated or not, and inborn or outborn. Each infant was randomized to either Group 1--fluids managed to allow 1 to 2% loss of BW per day to a maximum loss of 8 to 10%, or Group 2--fluids managed to allow 3 to 5% loss of BW per day to a maximum loss of 13 to 15%. Optimizing Enteral Nutrition During and After Therapeutic Hypothermia: An Evidence-Based Practice Initiative.

BACKGROUND: Feeding practices during therapeutic hypothermia (TH) vary widely across neonatal intensive care units, and many centers delay enteral nutrition because of concerns about gastrointestinal intolerance and necrotizing enterocolitis. Current evidence suggests that early, clinically guided enteral feeding may be safe and beneficial for neonates with hypoxic-ischemic encephalopathy. An evidence-based feeding guideline was implemented to promote enteral nutrition during and after TH. PURPOSE: To evaluate whether initiating minimal enteral feeding in clinically stable neonates undergoing

Neonatal and 18-24-Month Growth and Neurodevelopmental Outcomes in Very Low Birth Weight Infants With Severe Congenital Heart Disease: A Korean Neonatal Network Study.

BACKGROUND AND OBJECTIVES: Severe congenital heart disease (CHD) in very low birth weight (VLBW) infants poses substantial risks, yet contemporary national data remain limited. This study evaluated the prevalence, lesion spectrum, and neonatal and 18-24-month outcomes of VLBW infants with severe CHD in South Korea. METHODS: We conducted a nationwide cohort study using the Korean Neonatal Network registry (2013-2021). Severe CHD was defined as a structural defect requiring surgical or catheter-based intervention within the first year of life and was categorized by physiological lesion type. Ne

Quantitative assessment of neonatal health using dried blood spot metabolite profiles and deep learning.

Neonatal prematurity leads to considerable morbidity and mortality, partly because of acquired conditions such as bronchopulmonary dysplasia (BPD), intraventricular hemorrhage (IVH), necrotizing enterocolitis (NEC), and retinopathy of prematurity (ROP). Standard gestational age and birthweight-based classifications of prematurity inadequately capture the variation in newborns' health outcomes, creating an urgent need to develop risk stratification tools for vulnerable newborn infants to initiate the most appropriate care pathways as early as possible. We hypothesized that the metabolic profile

Current Management of Pneumatosis Intestinalis in Children: A Review.

IMPORTANCE: Pneumatosis intestinalis (PI), defined by the presence of gas within the bowel wall, ranges in severity from a benign, incidental finding to a life-threatening condition. The true incidence in children is unclear, and standardized treatment protocols are lacking, resulting in variable management and potential for both over- and undertreatment. This review summarizes current knowledge on pediatric PI, including pathophysiology, clinical presentation, diagnostics, treatment strategies, and risk stratification. OBSERVATIONS: Development of PI is multifactorial, involving mechanical,
