Long-term outcome of Hurler syndrome following bone marrow transplantation.

Previous reports suggested a therapeutic response of lysosomal storage diseases such as Hurler syndrome following bone marrow transplantation. However, a clearer understanding of outcome has awaited long-term follow-up. We evaluated prospectively 11 consecutive patients with Hurler syndrome receiving marrow from an HLA-identical sib donor between September 1983-October 1988. Follow-up evaluations included assessment of donor engraftment by restriction fragment polymorphism analysis, determination of leukocyte alpha-L-iduronidase level, measurement of lumbar cerebrospinal fluid (CSF) pressure,

Creating of "fascial sheath" around subcutaneous lumboperitoneal shunt catheters largely prevents postoperative subcutaneous shunt catheter migration.

BACKGROUND: Spinal catheter migration into the subcutaneous tissue is common after lumboperitoneal shunt (LPS) placement. This study proposed a new method (i.e., wrapping fascia around the catheter like a sheath) to prevent LPS spinal catheter migration. METHODS: After a LPS spinal catheter was inserted under routine fluoroscopic guidance, and the paravertebral muscle fascia was closed, the fascia was sutured to wrap the catheter like a sheath using intermittent sutures. RESULTS: Before the introduction of this technique, the rate of LPS spinal catheter subcutaneous migration was 4.6%. In th

[A Case of Rectus Sheath Hematoma Presenting Immediately after a Lumboperitoneal Shunt and Requiring Emergency Surgery:Importance of Abdominal Anatomy Knowledge].

As the incidence of idiopathic normal-pressure hydrocephalus (iNPH) rises in an aging society, the number of cerebrospinal fluid (CSF) shunts performed increases every year. The morbidity of iNPH in patients>65 years of age has been reported as 1.4%-2.9% in Japan. CSF shunts are rarely associated with mortality and are generally safe to perform, but subcutaneous hematomas and intestinal injuries are the major potential complications of the abdominal surgery for CSF shunts. In this report, we describe an uncommon case of rectus sheath hematoma (RSH) that occurred immediately after a lumboperito

[Charles Miller Fisher: the grandmaster of neurological observation].

Charles Miller Fisher is widely regarded as the father of modern stroke neurology. He discovered almost all pathomechanisms of cerebral infarction, including embolism from atrial fibrillation, carotid artery disease, and lacunar infarcts and their syndromes, by the most meticulous clinico-pathological observations. Moreover, his work provided the basis for treatments such as anticoagulation, antiplatelet therapy, and carotid endarterectomy. He also contributed greatly to several topics of General Neurology; for example, migraine, normal pressure hydrocephalus, and Miller Fisher syndrome. In hi

Magnetic resonance imaging in the canine double-haemorrhage subarachnoid haemorrhage model.

In this study, we investigated T2 weighted imaging (T2WI) and T2 values of the cortex, thalamus and cerebrospinal fluid (CSF) of the ventricles in the canine double-haemorrhage subarachnoid haemorrhage (DHSAH) model. T2 values in the cortex increased compared to prescan values from 123.07 +/- 18.72 msec on day 2 to 89.43 +/- 1.98 msec on day 7 (p < 0.05). A trend toward a temporal increase in T2 values was observed in the thalamus, but did not reach significance. The T2 values of the ventricular CSF increased by 102.2% on day 2 and 159.6% on day 7 compared to prescan values. These changes reac

[Frequent neurosurgically treatable diseases in geriatric patients].

Geriatrics and neurosurgery have several points of contact, which require a close cooperation. In this context, diseases frequently affecting old people include chronic subdural hematomas, benign and malignant brain tumors, normal pressure hydrocephalus, spinal compression syndromes, spinal nerve root and peripheral nerve affections. In the case of absolute indications for surgery (e.g., symptomatic subdural hematoma, paraparesis or tetraparesis due to compression of the myelon), no alternative conservative treatment option is available to prevent death or irreversible damage. Such interventio

The glymphatic system and its involvement in disorders of the nervous system.

The central nervous system was thought to be devoid of lymphatics. Recently, the existence of an authentic brain lymphatic system, known as the glymphatic system, composed of paravascular channels penetrating arterial and venous brain vessels and dural lymphatics cleaning the interstitial space, has been demonstrated. Aquaporin-4, located in astrocyte feet attached to the paravascular spaces, plays a key role in the clearance of waste molecules, such as beta-amyloid or tau proteins. The activity of this system is increased during sleep, mainly in the slow wave phase and while sleeping on one s

Intra-bronchial migration of peritoneal catheter of lumboperitoneal shunt.

BACKGROUND: A rare case of intra-bronchial migration of peritoneal catheter of lumboperitoneal (LP) shunt was treated under the bronchoscopic and fluoroscopic observation. CASE DESCRIPTION: A 71-year-old man, who underwent LP shunt installation due to idiopathic normal pressure hydrocephalus a year before, presented with history of high fever and sputum production. Roentgenography and computed tomography of the chest revealed migration of distal end of the peritoneal catheter into the left main bronchus. Migrated catheter was gently extracted through the abdominal wound incision under the bro

Cystic tuberculomas of the brain--two case reports.

A 19-year-old male presented with raised intracranial pressure and a peripherally enhanced multinodular lesion with a large subjacent cyst in the right medial frontal lobe. Surgery revealed a solid tuberculoma and an underlying cyst lined with normal brain tissue. A 22-year-old male with a history of tuberculous meningitis and hydrocephalus presented with an exophytic brain stem and a left temporal tuberculoma, which were excised in two stages. The temporal tuberculoma had a peripherally located cyst that was lined with tuberculous tissue. Intracranial tuberculomas are an important part of the

Laparoscopic placement of ventriculoperitoneal shunts: preliminary report.

We used a laparoscopic technique for the percutaneous placement of the peritoneal end of cerebrospinal fluid shunts in adult patients with obstructive or normal-pressure hydrocephalus. Concurrent with the initial cranial part of the procedure, pneumoperitoneum is established in a routine fashion, and a video-laparoscope and grasping forceps are inserted into the peritoneal cavity. With use of a pacemaker introducer kit, the peritoneal catheter is placed percutaneously under direct laparoscopic vision through a small upper abdominal incision into the peritoneal cavity. At the completion of the

The S-100 protein in cerebrospinal fluid: a simple ELISA method.

A simple ELISA method is described for determinations of S-100 protein concentrations in CSF. The assay has a useful range of 200-3200 pmol/l. The precision of the ELISA was estimated using a pool of CSF. The coefficient of variation was 0.18 within assay, 0.17 between assay and 0.17 between day. The S-100 protein is stable in the CSF as no measurable differences in S-100 concentrations were observed in samples stored at room temperature for 2 days. No correlation between age and S-100 concentration was found when determinations were performed in CSF from neurologically healthy males. Furtherm

Magnetic resonance imaging of an adult with the Dandy-Walker syndrome.

A 30-year-old retired veteran was asymptomatic for two decades; he had carried out normal everyday living activities and was self-supporting. It was not until he was struck by an automobile, which resulted in head trauma, that the Dandy-Walker syndrome was incidentally discovered by computed tomography. Most patients with the Dandy-Walker syndrome or malformation are infants and seldom live into adulthood. Therefore, this patient is one of the very few patients with this malformation who remained in a state of cerebrospinal fluid compensation and lived to adulthood. The head trauma he received

Multiplicity of cerebrospinal fluid functions: New challenges in health and disease.

UNLABELLED: This review integrates eight aspects of cerebrospinal fluid (CSF) circulatory dynamics: formation rate, pressure, flow, volume, turnover rate, composition, recycling and reabsorption. Novel ways to modulate CSF formation emanate from recent analyses of choroid plexus transcription factors (E2F5), ion transporters (NaHCO3 cotransport), transport enzymes (isoforms of carbonic anhydrase), aquaporin 1 regulation, and plasticity of receptors for fluid-regulating neuropeptides. A greater appreciation of CSF pressure (CSFP) is being generated by fresh insights on peptidergic regulatory se

Assessment of cerebrospinal fluid outflow conductance using an adaptive observer--experimental and clinical evaluation.

Idiopathic normal pressure hydrocephalus (INPH) patients have a disturbance in the dynamics of the cerebrospinal fluid (CSF) system. The outflow conductance, C, of the CSF system has been suggested to be prognostic for positive outcome after treatment with a CSF shunt. All current methods for estimation of C have drawbacks; these include lack of information on the accuracy and relatively long investigation times. Thus, there is a need for improved methods. To accomplish this, the theoretical framework for a new adaptive observer (OBS) was developed which provides real-time estimation of C. The

Urogenital dysfunction in neurological diseases.

Neural damage at any level of the neuraxis can lead to urogenital dysfunction, involving the lower urinary tract (LUT) and/or the sexual organs. The LUT consists of the bladder and the urethra (plus the prostate in males); LUT dysfunction can manifest as an underactive bladder, leading to urinary retention, or as an overactive bladder, leading to urinary incontinence. Manifestations of sexual dysfunction include loss of libido and dysfunction of erection, ejaculation and orgasm. In this Review, we address the physiology and innervation of the LUT and the sexual organs and discuss various aspec

The glymphatic system in migraine and other headaches.

Glymphatic system is an emerging pathway of removing metabolic waste products and toxic solutes from the brain tissue. It is made of a network of perivascular spaces, filled in cerebrospinal and interstitial fluid, encompassing penetrating and pial vessels and communicating with the subarachnoid space. It is separated from vessels by the blood brain barrier and from brain tissue by the endfeet of the astrocytes rich in aquaporin 4, a membrane protein which controls the water flow along the perivascular space. Animal models and magnetic resonance (MR) studies allowed to characterize the glympha

[The glymphatic system - an overview].

The glymphatic system as a concept was introduced a decade ago and the knowledge regarding the circulation of the cerebrospinal fluid has since been revised. The research of the glymphatic system is extensive today, but there are many knowledge gaps. The glymphatic system was first investigated in animal experiments, but with the MR techniques available today the glymphatic system has also been shown to exist in humans. Studies suggest that the glymphatic system may be involved in the pathophysiological processes of neurological pathologies, such as degenerative brain disorders and normal pres

Cerebrospinal Fluid Biomarkers for Alzheimer's Disease in the Era of Disease-Modifying Treatments.

Correct in vivo diagnosis of Alzheimer's disease (AD) helps to avoid administration of disease-modifying treatments in non-AD patients, and allows the possible use of such treatments in clinically atypical AD patients. Cerebrospinal fluid (CSF) biomarkers offer a tool for AD diagnosis. A reduction in CSF β-amyloid (marker of amyloid plaque burden), although compatible with Alzheimer's pathological change, may also be observed in other dementing disorders, including vascular cognitive disorders due to subcortical small-vessel disease, dementia with Lewy bodies and normal-pressure hydrocephalus.
