Rescue of peripheral vestibular function in Usher syndrome mice using a splice-switching antisense oligonucleotide.

Usher syndrome type 1C (USH1C/harmonin) is associated with profound retinal, auditory and vestibular dysfunction. We have previously reported on an antisense oligonucleotide (ASO-29) that dramatically improves auditory function and balance behavior in mice homozygous for the harmonin mutation Ush1c c.216G > A following a single systemic administration. The findings were suggestive of improved vestibular function; however, no direct vestibular assessment was made. Here, we measured vestibular sensory evoked potentials (VsEPs) to directly assess vestibular function in Usher mice. We report that

Noisy galvanic vestibular stimulation enhances spatial memory in cognitive impairment-induced by intracerebroventricular-streptozotocin administration.

There are several anatomical connections between vestibular system and brain areas construct spatial memory. Since subliminal noisy galvanic vestibular stimulation (GVS) has been demonstrated to enhance some types of memory, we speculated that application of noisy GVS may improve spatial memory in a rat model of intracerebroventricular streptozotocin (ICV-STZ)-induced cognitive impairment. Moreover, we attempted to determine the effect of repeated exposure to GVS on spatial memory performance. The spatial memory was assessed using Morris water maze test. The groups received 1 (ICV-STZ/GVS-I) o

Suppurative labyrinthitis associated with otitis media: 26 years' experience.

INTRODUCTION: Suppurative labyrinthitis continues to result in significant hearing impairment, despite scientific efforts to improve not only its diagnosis but also its treatment. The definitive diagnosis depends on imaging of the inner ear, but it is usually clinically presumed. OBJECTIVE: To analyze the clinical factors and hearing outcomes in patients with labyrinthitis secondary to middle ear infections and to discuss findings based on imaging test results. METHODS: Retrospective cohort study, based on the charts of patients admitted with middle ear infection-associated labyrinthitis. R

Integration of vestibular and gastrointestinal inputs by cerebellar fastigial nucleus neurons: multisensory influences on motion sickness.

Previous studies demonstrated that ingestion of the emetic compound copper sulfate (CuSO4) alters the responses to vestibular stimulation of a large fraction of neurons in brainstem regions that mediate nausea and vomiting, thereby affecting motion sickness susceptibility. Other studies suggested that the processing of vestibular inputs by cerebellar neurons plays a critical role in generating motion sickness and that neurons in the cerebellar fastigial nucleus receive visceral inputs. These findings raised the hypothesis that stimulation of gastrointestinal receptors by a nauseogenic compound

Management of labyrinthine fistulae in Kyoto University Hospital.

CONCLUSION: In cases of labyrinthine fistulae, we performed complete removal of the cholesteatoma matrix in a one-stage procedure, resulting in a satisfactory bone conduction (BC) hearing preservation rate. Preoperative evaluation of labyrinthine fistulae using high resolution computed tomography (HRCT) detected 86% of cases, and this contributed to favorable results achieved with the surgical treatment of labyrinthine fistulae. We aimed to review cases of labyrinthine fistulae to summarize their outcomes and establish standards of management. METHODS: This was a retrospective chart review of

Application of multivariate statistics to vestibular testing: discriminating between Menière's disease and migraine associated dizziness.

Menière's disease (MD) and migraine associated dizziness (MAD) are two disorders that can have similar symptomatologies, but differ vastly in treatment. Vestibular testing is sometimes used to help differentiate between these disorders, but the inefficiency of a human interpreter analyzing a multitude of variables independently decreases its utility. Our hypothesis was that we could objectively discriminate between patients with MD and those with MAD using select variables from the vestibular test battery. Sinusoidal harmonic acceleration test variables were reduced to three vestibulo-ocular r

Evolution in technique for vestibular schwannoma radiosurgery and effect on outcome.

PURPOSE: To define changes in treatment technique for vestibular schwannoma radiosurgery and to relate them to changes in outcome, a large single institution experience was reviewed. METHODS AND MATERIALS: Two hundred seventy-three patients with unilateral vestibular schwannomas underwent Gamma knife radiosurgery: 118 with computed tomography (CT) treatment planning during 1987-1991, and 155 with magnetic resonance imaging (MR) treatment planning in 1991-1994. Mean treatment parameters differed between the CT and MR groups: minimum tumor dose (D(min)) was 17 vs. 14 Gy, number of isocenters wa

Vestibular function test anomalies in patients with chronic fatigue syndrome.

Chronic fatigue syndrome (CFS) is distinguished by the new onset of debilitating fatigue that lasts at least 6 months, concomitant with other symptoms to be described later. Many CFS patients complain of disequilibrium, yet the exact type of the balance dysfunction and its function and its location (peripheral vs. central) have not been described. Herein we report results of vestibular function testing performed on 11 CFS patients. These results revealed no predominant pattern of abnormalities. Patients typically performed below average in dynamic posturography testing, with a significant numb

[Noninvasive evaluation of intracochlear pressure. II. Study findings in patients with Meniere's disease, fluctuating deep tone hearing and peripheral vestibular vertigo].

The tympanic membrane displacement technique (TMD) is aimed at evaluating intracochlear and intracranial pressure changes non-invasively. Therefore, the present paper describes the findings in patients with Ménière's disease where an increase in volume of the endolymphatic spaces is discussed. It should be investigated to which extent a change in the intracochlear pressure corresponds to different stages of the disease. It could be described that the intracochlear pressure does change over a certain period of persisting Ménière's, but that there is no extensive increase in intracochlear pressu

The approximation of audiometric thresholds by auditory brain stem responses.

Auditory brain stem response (ABR) thresholds for tone pip stimuli were compared with audiometric thresholds for 63 ears to assess the feasibility of deriving audiograms with ABR techniques. Correlations between the two measures were highly significant, indicating a high degree of accuracy in assessing degree and configuration of hearing loss with ABR. Patients with conductive losses showed a simple one-to-one relationship between ABR and audiometric thresholds, while patients with cochlear losses did not. Possibly this is due to the effects of abnormal temporal integration or poor frequency s

Is endolymphatic hydrops, as detected in MRI, a truly cochleocentric finding?

INTRODUCTION: The most common histopathological finding in Ménière's disease (MD) is endolymphatic hydrops (EH), which involves the dilation of the membranous labyrinth. The direct relationship between EH and MD is debated, although EH plays a crucial role in auditory and vestibular functional tests. MRI sequences such as 3D-FLAIR and 3D-real-IR are used to study EH, with the latter being more effective. This study aimed to examine whether the severity of EH detected by MRI is always more pronounced in the cochlea than in the vestibule, indicating a cochleocentric progression of the condition.

Instability in Patients with CANVAS: Can Computerized Dynamic Posturography Help in Diagnosis?

OBJECTIVE: To describe the pattern of dynamic posturography or its role in diagnosis in patients with cerebellar ataxia with neuropathy and vestibular arreflexia syndrome (CANVAS). MATERIALS AND METHODS: We present and describe posturographic data of four patients diagnosed with the CANVAS syndrome in a tertiary hospital. RESULTS: In all patients, the average balance score was diminished. Two patients presented a pattern of visual dependence. The other two showed misuse of three posturography sensory information (visual, vestibular and propioceptive information), specially null use of vestib

Prognostic value of initial electronystagmography findings in idiopathic sudden sensorineural hearing loss without vertigo.

OBJECTIVE: We used electronystagmography (ENG) to characterize recurrent hearing loss and its progression to definite Ménière's disease in patients with idiopathic sudden sensorineural hearing loss (SSHL) without subjective vertigo. METHODS: We reviewed medical records of 1,334 patients with unilateral hearing loss initially diagnosed with idiopathic SSHL between 1985 and 2003 at our university hospital. Of the 1,334 patients, we examined 127 (86 with low-tone and 41 with high-tone SSHL) who underwent ENG within 30 days of the initial diagnosis and who could be followed up during the long ter

Surgical management of the labyrinthine fistula complicating chronic otitis media with or without cholesteatoma.

OBJECTIVES: To present our most recent experience on the diagnosis and treatment of the labyrinthine fistula. The relative data are compared with those of our past experiences adopting the same modality of presenting the results. METHODS: The clinical and surgical records of 334 patients affected with chronic otitis media with or without cholesteatoma were analyzed to evaluate the frequency, site, and size of the labyrinthine fistulae. In this study, a more aggressive strategy was adopted that comprised immediate total removal of the cholesteatoma matrix and the surrounding inflammatory tissu

Clinical features of sudden hearing loss associated with a high signal in the labyrinth on unenhanced T1-weighted magnetic resonance imaging.

We report on five patients with high signals in the labyrinth on unenhanced magnetic resonance imaging who developed sudden hearing loss and vertigo. Weissman et al. (1992) suggested the possibility that such high signals were caused by hemorrhage. We assessed these patients using audiograms, caloric tests, and auditory brainstem responses to investigate the possibility of inner ear hemorrhage. Most of the patients were found to have severe and irreversible impairment of both cochlear and vestibular function. These findings were consistent with the hypothesis that their symptoms were caused by
