Increased vascularization during early healing after biologic augmentation in repair of chronic rotator cuff tears using autologous leukocyte- and platelet-rich fibrin (L-PRF): a prospective randomized controlled pilot trial.

HYPOTHESIS: We hypothesized that arthroscopic rotator cuff repairs using leukocyte- and platelet-rich fibrin (L-PRF) in a standardized, modified protocol is technically feasible and results in a higher vascularization response and watertight healing rate during early healing. METHODS: Twenty patients with chronic rotator cuff tears were randomly assigned to 2 treatment groups. In the test group (N = 10), L-PRF was added in between the tendon and the bone during arthroscopic rotator cuff repair. The second group served as control (N = 10). They received the same arthroscopic treatment without

Outcome of rotator cuff repair in large-to-massive tear with pseudoparalysis: a comparative study with propensity score matching.

BACKGROUND: Active range of motion deficit is one of the alleged negative influencing factors of rotator cuff repair. Recently, with the popularity of reverse total shoulder arthroplasty (RTSA), there is a tendency toward performing RTSA in cases of nonarthritic large-to-massive tears with pseudoparalysis. HYPOTHESIS: Rotator cuff repair in patients with active motion deficit may yield inferior outcome. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Among 195 complete repairs of large-to-massive rotator cuff tears, 35 patients experienced painful pseudoparalysis preoperatively.

The outcome and structural integrity of arthroscopic rotator cuff repair with use of the double-row suture anchor technique.

BACKGROUND: The reported rate of failure after arthroscopic rotator cuff repair has varied widely. The influence of the repair technique on the failure rates and functional outcomes after open or arthroscopic rotator cuff repair remains controversial. The purpose of the present study was to evaluate the functional and anatomic results of arthroscopic rotator cuff repairs performed with the double-row suture anchor technique on the basis of computed tomography or magnetic resonance imaging arthrography in order to determine the postoperative integrity of the repairs. METHODS: A prospective ser

Anatomical basis of latissimus dorsi and teres major transfers in rotator cuff tear surgery with particular reference to the neurovascular pedicles.

Musculotendinous transfers (MTT) of latissimus dorsi (LD) and teres major (TM), either in isolation or combination, have recently been advocated to treat irreparable rotator cuff tears. The purpose of this study was to (1) review the anatomy of the LD and TM neurovascular pedicles, and (2) undertake experimental MTT to humeral insertions of either supraspinatus or infraspinatus to evaluate tension on their nutrient arteries in six positions of the arm. Twenty-six shoulders were studied, 22 of which had previously been injected with red latex. Gross dissection, using 4.3 x magnification when ne

Pre-Exercise Conditioned Pain Modulation Is Associated With Exercise-Induced Hypoalgesia in Patients With Painful Rotator Cuff Tears.

BACKGROUND: Exercise-induced hypoalgesia (EIH) is a well-established phenomenon; however, its magnitude varies among individuals and clinical conditions. In patients with rotator cuff tear (RCT), movement-evoked pain during arm elevation is a key symptom directly linked to functional limitations, making EIH particularly clinically relevant. This study aimed to investigate whether pre-exercise endogenous pain inhibitory capacity, assessed using conditioned pain modulation (CPM), is associated with EIH magnitude in patients with painful RCT. METHODS: Thirty patients with symptomatic RCT perform

Physiological Tensioning During Lower Trapezius Transfer for Irreparable Posterosuperior Rotator Cuff Tears May Be Important for Improvement in Shoulder Kinematics.

BACKGROUND: Lower trapezius transfer (LTT) has been proposed for restoring the anteroposterior muscular force couple in the setting of an irreparable posterosuperior rotator cuff tear (PSRCT). Adequate graft tensioning during surgery may be a factor critical for sufficient restoration of shoulder kinematics and functional improvement. PURPOSE/HYPOTHESIS: The purpose was to evaluate the effect of tensioning during LTT on glenohumeral kinematics using a dynamic shoulder model. It was hypothesized that LTT, while maintaining physiological tension on the lower trapezius muscle, would improve glen

Outcomes of subacromial balloon spacer implantation for irreparable rotator cuff tears: a systematic review and meta-analysis.

BACKGROUND: The management of irreparable rotator cuff tears remains a topic of considerable debate among orthopedic surgeons. Currently, there is little consensus regarding the gold-standard treatment; however, an emerging option involves the use of a biodegradable subacromial spacer. The purpose of this study, therefore, was to systematically review and synthesize the current literature reporting on the clinical outcomes following implantation of a subacromial balloon spacer (SABS) for the treatment of patients with irreparable rotator cuff tears. METHODS: A systematic review of the PubMed

Clinical effectiveness of mini-open superior capsular reconstruction using autologous tensor fascia lata graft.

BACKGROUND: When one is performing superior capsular reconstruction (SCR), graft thickness is an important factor for achieving sufficient glenohumeral stability. However, when a graft of sufficient length and thickness is prepared and inserted into the subacromial space, it is often challenging to secure the field of view arthroscopically. To solve this problem, we devised a mini-open SCR technique. This study aimed to compare the clinical effectiveness of this technique with that of arthroscopic SCR. METHODS: This retrospective cohort study included 46 consecutive patients with Hamada class

Assessment of abduction motion in patients with rotator cuff tears: an analysis based on inertial sensors.

BACKGROUND: Reduced range of motion in the shoulder can be a source of functional limitation. The use of inertial sensors to quantify movement in addition to more common clinical assessments of the shoulder may allow clinicians to understand that they are potentially unnoticed by visual identification. The aim of this study was to generate an explanatory model for shoulder abduction based on data from inertial sensors. METHOD: A cross-sectional study was carried out to generate an explanatory model of shoulder abduction based on data from inertial sensors. Shoulder abduction of thirteen older

Heterogeneous muscle gene expression patterns in patients with massive rotator cuff tears.

Detrimental changes in the composition and function of rotator cuff (RC) muscles are hallmarks of RC disease progression. Previous studies have demonstrated both atrophic and degenerative muscle loss in advanced RC disease. However, the relationship between gene expression and RC muscle pathology remains poorly defined, in large part due to a lack of studies correlating gene expression to tissue composition. Therefore, the purpose of this study was to determine how tissue composition relates to gene expression in muscle biopsies from patients undergoing reverse shoulder arthroplasty (RSA). Gen

Arthroscopic transosseous rotator cuff repair: the eight-shape technique.

All-arthroscopic anchorless transosseous suture techniques combine the advantages of the open transosseous repair with the benefits of arthroscopic technique. However, all the techniques described until now are very complex, difficult to reproduce and associated with an increased surgical time. The authors developed a novel all-arthroscopic anchorless transosseous suture technique easy to perform and to reproduce. This procedure maximizes the tendon-footprint contact area obtaining both medial and lateral fixation without using any device, employing only 1 suture tape so to avoiding the risk o

Clinical evaluation of nonarthritic shoulder pain: Diagnosis and treatment.

Shoulder pain and dysfunction is a complex problem frequently encountered by primary care physicians. Common nonarthritic conditions seen in the primary care setting include rotator cuff syndrome, impingement, posttraumatic stiffness, adhesive capsulitis, and instability. A thorough history and physical examination can aid in the diagnosis of many common shoulder complaints. Pain and instability are the most common shoulder complaints. Pain that is sharp or burning is commonly radicular in origin, whereas pain caused by tendinitis is often dull, diffuse, and aching. Instability is frequently f
